Please review the following information carefully. The application for admission to the WSU nursing program is attached. This application is for the Fall 2016 Semester Admission only. All application materials must be received no later than February 1, 2016 by 4:00 p.m. Be sure to fill out your information accurately especially your WSU address. An will be sent to your WSU to verify that your application has been received. If you do not receive an by January 29, 2016, you will need to contact our office by 4:00 p.m. on February 1, 2016 to verify that your application has been received. No applications will be accepted after the deadline. Breakdown for Admission Criteria: 60% prerequisite GPA (calculated on the 3 most recent natural sciences, not including nutrition, 3 most recent social sciences, English and statistics), 20% TEAS score, 15% starting coursework at WSU or Rochester Community & Technical College (RCTC), and 5% not repeating or withdrawing from any prerequisites. ** All students admitted to the Nursing: Generic Option for Fall 2016, must complete all prerequisites, including the requirement for certification as a nursing assistant, listed as active on a Nursing Assistant Registry within the United States before courses begin for Fall For more information, please see attached letter in the application packet. Reminders for applicants: 1. Review admission policies and procedures to be sure your file is complete and you are ready to apply. Complete the Application Readiness Checklist at The checklist is designed to be completed on the computer. Print out the completed checklist, sign, attach and submit with your application for admission. You need not print the checklist in color. 2. Applicants for admission must be declared as nursing candidates (PNUG or PNGE) as of the application due date. 3. Students who signed the Preferred Admission Contract as first-year WSU students and meet the contract requirements will have first priority for admission to the nursing program. 4. Students with credits earned at other colleges or universities, including college credits earned while in high school and credits for CNA courses completed at community and technical colleges, must verify that all transcripts have been received by WSU and that all completed courses are reflected on the student Degree Audit Report (DAR) as of the application due date. The Department of Nursing Undergraduate Progression Committee will meet in March to review applicant files and determine who will be admitted to the nursing program. By mid-april, each person applying to the nursing program will receive a letter regarding his or her admission status at his or her permanent address in official university records. 4 Department of Nursing STUDENT REVIEW OF NURSING ADMISSION PROCEDURES/POLICIES Please return this signed form with your admission application. SIGNATURE REQUIRED The following policies and admission procedures pertain to the Fall 2016 nursing admission. Note that these policies are also found in your WSU catalog and the letter accompanying your nursing application. You are responsible for meeting all admission criteria and making sure that your file is complete. To be considered for admission to the WSU Nursing Program, you must: 1. be accepted to Winona State University with a declared nursing candidate major (PNUG or PNGE) prior to applying for admission to the nursing program 2. have a cumulative GPA of 3.30 or above 3. have completed at least 45 credit hours 4. have completed at least 4 of 5 natural sciences, 3 of 4 social sciences, Statistics, and English at time of application (Natural and social science prerequisites must have been completed within the last 5 years. English and statistics must have been completed within the last 10 years.) 5. have a plan for completing all prerequisite courses; (NOTE: All prerequisites must be completed with a C or higher before starting the nursing major in the fall.) 6. complete the Test of Essential Academic Skills (TEAS) test prior to application. (The TEAS may be taken only once, and should be taken the semester of application to the nursing program.) 7. submit transcripts for all college credits completed, including CNA courses completed at community or technical colleges, so that all credits completed are reflected on your Degree Audit Report by the application due date 8. submit your completed application form by 4:00 p.m., Monday, February 1, Your application must be received (not postmarked) by the application due date. Late applications will not be accepted. For the Fall semester 2016 nursing admission process, priority consideration will be given to students who are enrolled in and meet the criteria of the preferred admission contract. The preferred admission contract is not a guarantee of admission. To continue in the preferred admission contract, the student must: 1. Be qualified to begin college-level (i.e., non-remedial) science, mathematics/statistics, and English courses as required by the nursing major. If the student takes MATH 050 or English 099, this contract is void. All 100-level university courses are acceptable and not considered remedial. 2. Meet with a WSU nursing advisor every semester to develop a plan for courses. 3. Meet any University or program GPA and/or grade requirements as stipulated in the University catalog and/or schedule of classes. 4. Successfully complete an average course load of 15 credits each fall and spring semester. The student must have completed at least 45 credit hours prior to application to the nursing major. 5. Have completed at least 4 of 5 natural sciences, 3 of 4 social sciences, Statistics, and English at time of application. 6. Complete all prerequisite courses with a grade of C or better. 7. Not have withdrawn ( W ) or repeated any nursing prerequisite course once enrolled at WSU. 8. Apply for admission to the nursing major after his or her third semester of coursework at Winona State University. 9. Earn a cumulative GPA of at least 3.50 at the time of first application to the nursing major. The GPA is not rounded up. 10. Earn a Test of Essential Academic Skills (TEAS) score of proficient, advanced or exemplary. This test must be taken during the sophomore year. The results must be received before application to the nursing major. The TEAS may be taken only once. Students may apply to the nursing program only once under the preferred admission status. I have reviewed the Nursing program admission criteria. I know that I am responsible for monitoring my admission status, including submitting required transcripts and evaluations of transfer credit. 5 DO NOT RETURN THIS PAGE USE THIS WSU NURSING APPLICATION TRACKING FORM to track your application for Fall 2016 For students with transfer credits, including PSEO credits: You must submit official transcripts for all college level courses, including CNA courses completed at community or technical colleges for college credit, to the WSU Admissions Office (not the WSU Nursing Department) as soon as possible, and no later than January 1, 2016 so that your credits can be evaluated for transfer and included on your Degree Audit Report as of the application due date. If you have credits from other schools within the Minnesota State Colleges and University (MNSCU) System, WSU is able to process a copy of transcripts electronically. Students are simply asked to complete the online request form available at If you have previously submitted your transcripts and all courses are reflected on your DAR, you do not need to resubmit transcripts. For all applicants: Instructions for accessing your Degree Audit Report (DAR) online Go to the Winona State home page: Click on Resources in the top menu bar, then on Find a Course In the course search screen, click on Login under Student e-services in the left menu. You will login using your StarID and password. If you have difficulty in logging in, click the Need Login Help? link and then the link to the StarID Self Service page to activate your StarID or reset your password. Click on Grades & Transcripts in the left menu. Click on Interactive Degree Audit Report Click on link to review. Verify that all of your transfer credits are shown on your DAR. If something is missing, you may need to contact Admissions to verify that your transcript has been received, and to get more information about when it will be processed and included in your DAR. Application must be received (not postmarked) by 4:00 pm, Monday, February 1, Received from WSU Nursing Department that application has been received by January 29, Received letter from WSU nursing program regarding admission status [should receive by 1 st or 2 nd week of April; if not, contact the Nursing Department]. If accepted to the nursing program, your Confirmation of Admission must be returned by the due date.

6 To: Applicants for admission to the Generic Option of the Nursing Major Fall 2016 All students admitted to the B.S. major in Nursing: Generic option for Fall 2016 must complete an approved nursing assistant training program that meets state and federal requirements to work in certified nursing and boarding care homes and be listed on a Nursing Assistant Registry within the United States. This means that: All students admitted to the Nursing: Generic Option for Fall 2016, must complete all prerequisites, including the requirement for certification as a nursing assistant, and be listed as active on a Nursing Assistant Registry within the United States. A copy of your CNA certificate should be submitted to the nursing department before courses begin for Fall semester As you select a nursing assistant course: Be sure that upon completion of the course you are eligible to take the Certification Exam for Nursing Assistant. Courses that meet this requirement are, as noted above, approved nursing assistant training programs that meet state and federal requirements for employment in certified nursing and boarding care homes. After you successfully complete the exam, you will be a certified nursing assistant and your name will appear on the Nursing Assistant Registry in the state in which you completed the exam. Students are responsible for all costs, registration and testing fees, and course arrangements.
