1 Earn up to $200 for Permitting Your Heating Ventilating and Air Conditioning (HVAC) Equipment SoCalGas and Southern California Edison are working together to offer rebates for toward your heating and air conditioning (HVAC) system replacements authorized though your local building department. You may be eligible to receive $200 for each HVAC system permitted in your home. When you go through the permitting process you get peace of mind of knowing that your HVAC system is properly and safely installed and is operating efficiently.

2 How to Apply: Check your eligibility by referring to the terms and conditions below. Obtain a permit. After your HVAC system is installed by a licensed contractor, schedule an appointment with your local building official to inspect your installation and finalize your permit. Permits must be finalized between September 1, 2014 and December 31, 2015 ( Term ) to be eligible for rebates. Term duration and eligibility requirements are subject to change. Ensure your HVAC permit has been finalized by your participating building department before submitting your application. Complete the application. To be eligible for both rebates, you must have both a SoCalGas and an SCE account, installation address and have installed both a natural gas-fueled furnace and AC system. Customers with only a SoCalGas account are only eligible for a rebate for the installed natural gas-fueled furnace. Customers with only an SCE account are only eligible for a rebate for the installed AC system. All system installations must be permitted through your local building department to qualify for rebates. Provide signatures on the application from yourself and the licensed contractor who performed the installation. Make copies of your SoCalGas and/or SCE bill, your finalized permit, and your contractor invoice to accompany your application. Make additional copies of all documents for your records. Mail your completed application and installation and permitting documentation soon. Rebates are awarded on a first-come, first-served basis. Postmark date deadline is January 31, 2016, but may be subject to change. 5 Terms & Conditions I have read, understood, and agree to the terms and conditions set forth below. I certify that the information I have provided is true and correct and the product(s) for rebate is/are installed and operational and meet(s) the eligibility requirements set forth in this application. Signature: 1. To be eligible for a rebate in accordance with this application, I must be a customer of SoCalGas and/or SCE with an active meter, and I must install a qualified product(s). All references to the term install, installation or similar phrases shall mean that the product is completely installed and is entirely functional and operational. 2. I understand that I must complete a separate application for products installed at each individual address and Service ID number. Permits must be approved by a participating building department between September 1, 2014 and December 31, 2015 ( Term ) to be eligible for rebates. The Term may be extended upon approval by the California Public Utilities Commission (CPUC). 3. Rebate offerings and rebate amounts may change without notice during the Term. Products for resale, products rebuilt, rented or leased less than five years, received from warranty or insurance claims, exchanged, or won as a prize, and new parts installed in existing products, do not qualify for any rebate. These terms and conditions may be modified or terminated without prior notice. Failure to submit a complete application may result in delay or rejection of a filed application. Complete applications must include all required application information, required signatures, proof(s) of purchase and other required documentation for all products as referenced in this application. Original applications will become the property of SoCalGas AND/OR SCE. Neither SoCalGas AND/ OR SCE nor its authorized third party will be responsible for items lost or destroyed in transit through the mail or electronic medium. 5. I will allow, if requested, a representative from SoCalGas AND/OR SCE, the CPUC, or any SoCalGas AND/OR SCE authorized third party, reasonable access to my property to verify the installed product before a rebate is paid. I understand a rebate will not be paid if I refuse to allow access within 30 days of a request by SoCalGas AND/OR SCE for such verification, or its authorized third party may contact the product vendor and/or installer, if needed, to verify purchase and/or installation and may provide my name and/or address to third parties to complete this verification. 6. I certify that I have installed product(s) in accordance with all applicable federal, state and local laws, building codes, manufacturer s specifications, and permitting requirements. If a contractor performed the installation or improvement, the contractor holds the appropriate license for the work performed. 7. I understand the rebate amount cannot exceed the purchase price of the product, nor can it include taxes or shipping costs. SoCalGas AND/OR SCE reserves the right to limit the number of products rebated. 8. SoCalGas and SCE make no representation of warranty, and assume no liability with respect to quality, safety, performance, or other aspect of any design, system product or appliance installed pursuant to this agreement, and each of SoCalGas and SCE expressly disclaims any such representation, warranty or liability. I agree to indemnify SoCalGas and SCE, their affiliates, subsidiaries, parent companies, officers, directors, agents, and employees against all loss, damage, expense, fees, costs and liability arising from any claims related to any products installed or services performed during the installation or maintenance of such products. 9. If I am a tenant, I am responsible for obtaining the property owner s permission to install product(s) for which I am applying for a rebate.
