1 REVIEWING YOUR PAYMENT PROTECTION INSURANCE. To help us review the sale of your Payment Protection Insurance (PPI) please complete all of the sections below, to the best of your knowledge. You must provide evidence of the condition(s) you have suffered such as confirmation from your doctor or hospital records at the time of sale. D7 If your claim was declined due to a pre-existing medical condition please tell us about this and provide us with any evidence you may have.
