Mission of the Graduate Program The mission of the graduate program in the Department of Health and Human Physiology (HHP) is twofold: 1. To prepare future academicians for positions in similar departments in colleges and universities by providing high-quality, cost-efficient and internationally recognized graduate educational programs in: a. The scientific study of physiological and behavioral processes as they interact with social and cultural forces to impact health. b. Physiological sciences as they relate to the function of the human organism as an integrated system of molecules, cells, tissues, and organs, in health and disease. 2. To prepare students for leadership positions as clinicians, therapists, or educators in academic, clinical, community-based and other professional settings with special emphasis on Clinical Exercise Physiology and Athletic Training. Overview The Department of HHP was founded in August 2010 as a merger of the formerly independent departments/programs of Health and Sport Studies, Leisure Studies, and Integrative Physiology. Previously, each of these entities had their own graduate programs. Thus, the new Department of HHP inherited the following programs: Furthermore, some faculty within these independent units joined another department (American Studies) or left the University since the merger. In addition, a new tenuretrack faculty member has joined the department and two new tenure-track faculty lines have recently been approved. As a result, the various graduate programs in the new Department of Health and Human Physiology (HHP) are not well-integrated with each other, are no longer in line with the mission of the HHP graduate program, and some focus areas within the currently existing programs are no longer covered by the expertise of the faculty of our department. Thus, there is a need to restructure the overall HHP graduate program. The PhD programs in Exercise Science and Health and Sport Studies are currently being phased out. The MA in Leisure Studies will remain a separate program. While this program is currently being evaluated in regards to its best design and content, it will not be discussed further in this proposal. Thus, this proposal concerns exclusively the MS in 2

Exercise Science, the MA in Health and Sport Studies, and the PhD in Integrative Physiology. Programs Affected by this Proposal This proposal concerns exclusively the following graduate programs, currently housed in the Department of HHP: MS in Exercise Science (currently only the focus area in Athletic Training is offered) MA in Health and Sport Studies (currently only the subtrack in Psychology of Sport and Physical Activity is offered, the subtracks in Athletic Administration and Sport Studies are in the process of being phased out). PhD in Integrative Physiology (current focus areas include: Anatomy, Biomechanics, Exercise and Integrative Physiology, and Neural Control of Movement) Proposed Changes The accompanying figure illustrates the major aspects of the proposed restructuring. We propose: (1) to rename and restructure the existing MS in Exercise Science to a MS in HHP, (2) to suspend admission and ultimately close the MA in Health and Sport Studies, and (3) to rename and restructure the PhD in Integrative Physiology to a PhD in HHP. 3

We propose to streamline our Graduate Program by suspending admission and ultimately closing the MA in Health and Sport Studies and by renaming and restructuring the existing MS in Exercise Science and PhD in Integrative Physiology. These changes are designed to accomplish the mission of our Graduate Program and to better integrate the Graduate Program into the overall educational objectives of our department. Specifically, we propose the following changes: 1. Suspend admission and subsequently close the MA in Health and Sport Studies 2. Rename the MS in Exercise Science to MS in HHP and add two formal subtracks that are designed as terminal MS degrees (without thesis). The two formal subtracks are: a. Clinical Exercise Physiology (CEP, new subtrack) and b. Athletic Training (AT, previously a focus area in the MS in Exercise Science). The major focus area of the MS in HHP besides the two formal subtracks is designed to prepare students for a doctoral degree program in research areas that reflect the expertise of the faculty of the department. This curriculum requires a formal thesis. 3. Rename the PhD in Integrative Physiology to PhD in HHP with no formal subtracks. Focus areas will reflect the expertise of the faculty of the department. Rationale for the chosen name of the revised MS and PhD programs: We propose to name both the MS and the PhD programs as MS/PhD in HHP. This name is identical to the department name that was carefully chosen after thorough on-campus discussion. At the time the department name was chosen, representatives from various colleges, as well as the Provost s Office, agreed that this name does not overlap with existing programs on campus and best reflects the mission of the new Department of HHP. Based on this previous campus-wide discussion we strongly feel that naming the revised MS and PhD programs according to the department name will likewise avoid overlap with existing graduate programs and best reflect the educational mission of our department. 4

Integration of the Revised Graduate Program The revised graduate program in the Department of HHP is designed to integrate the various educational programs from undergraduate to the master s and doctoral levels into a hierarchical structure as shown on the left. For example, students in the BS in Leisure Studies or Athletic Training may continue their education in one of the terminal master s degree programs before entering the professional world. On the other hand, undergraduate students interested in an academic career may opt for the MS in HHP with thesis to prepare themselves for our (or another) PhD program. We strongly feel that this hierarchical architecture of our educational programs not only prepares students optimally for taking on positions in the professional world, but also helps in recruiting the highest qualified students from our undergraduate program to our own graduate programs. Thus, the integration of the various educational programs within our department is expected to improve placement of our graduates and to reduce attrition of students away from our department and The University of Iowa. Rationale for Proposed Changes MA in Health and Sport Studies: This MA is a remnant of the previous (and now closed) Health and Sport Studies program. The MA consists of three official subtracks. Two of the subtracks (Athletic Administration and Sport Studies) are currently being phased out. The only subtrack currently offered within the MA in Health and Sport Studies is Psychology of Sport and Physical Activity (six students enrolled as of spring semester 2012). The expertise of the faculty who joined HHP from this former program is not in the area of Psychology of Sport and Physical Activity but in the area of Health Promotion with a focus on physical activity and health outcomes. Thus, we propose to also phase out this subtrack and to subsequently close the MA in Health and Sport Studies once all currently enrolled students have graduated. The existing expertise in Health Promotion will be integrated into the revised MS in HHP as well as into the PhD in HHP (described in detail below). MS in Exercise Science: This MS program is currently limited to the focus area in Athletic Training (AT) with six students enrolled (as of spring semester 2012). The AT program within the department of HHP is a highly successful program. For example, the BS in AT has been accredited continuously since 1974 from the Commission on Accreditation of Athletic Training Education (CAATE). Likewise, the focus area in AT within the MS in Exercise Science is a well-established program of highly selected students who are all certified Athletic Trainers. Most of the students are funded by 5

Graduate Assistantships through the Department of Athletics. To streamline the overall graduate program within the department, we propose to change the name from MS in Exercise Science to MS in HHP and to establish two official subtracks within the MS that will be offered without thesis. One subtrack will represent the previous focus area in AT and the other subtrack will be a newly developed subtrack in Clinical Exercise Physiology. In addition to these two subtracks, students can chose to pursue the MS in HHP with thesis. The MS in HHP with thesis is designed to prepare students for a doctoral degree program in research areas reflected by the expertise of the faculty of the department. MS in Health and Human Physiology: The mission of the graduate program of HHP includes academic research-oriented and more practical health-professional-oriented educational goals. To reflect these goals and the overall mission of the graduate program, we propose to restructure the MS into two official subtracks without thesis that will have the character of a more terminal graduate degree and a curriculum with thesis that serves to prepare students with BA/BS degrees for a doctoral degree program. Thus, we propose the following new structure for the MS in HHP: A. Subtracks without thesis, designed to serve as a terminal degree: 1. Clinical Exercise Physiology (CEP) 2. Athletic Training (AT) B. Curriculum with thesis 3. designed to prepare students for a doctoral degree program Our department receives an increasing number of inquiries from students seeking admission into a graduate program that prepares them for a professional career in the broad areas of allied health. This trend is in line with the increasing enrollment numbers in our undergraduate programs in HHP (520 UG students in the BA plus 380 UG students in the BS degrees), which clearly reflect the emerging interest in this area. In addition, some of our undergraduate students (BS in Human Physiology) who initially have a pre-medical or pre-physical therapy interest start looking for alternative career opportunities once they are in their Junior or Senior year. These students typically seek a Master s program that prepares them for certification exams in the areas of Athletic Trainer, Clinical Exercise Specialist (CES), or Registered Clinical Exercise Physiologist (RCEP). Based on the many inquires that we regularly receive from students interested in these areas and based on the increasing numbers of employment opportunities in the various areas of allied health in the State of Iowa and the US, we feel that there is a need at The University of Iowa for an MS program that serves this population of highly motivated students. For example, as of June 2011, there are 3,596 individuals in the US who hold the CES certificate and 853 with RCEP credential from the American College of Sports Medicine (ACSM). The RCEP certification requires an MS/MA degree. State licensure of CEP is enacted in Louisiana and bills are in process in other states (e.g., Massachusetts, North Caroline, and Utah). While CEP certification does not require a graduate degree, in all states offering licensure in CEP a graduate degree in the area of Exercise Science/Physiology is required. The subtrack in Clinical Exercise Physiology is specifically designed to fill this need. 6

In the past we had exceptionally well-qualified undergraduate students with GPAs of 3.5 or better who enrolled in our BS program in Human Physiology specifically because they were interested in a research career. Other students who initially joined the BS program as pre-medical or pre-physical therapy students became interested in research careers while participating in our honors program. Those students are typically not yet ready to directly enter a PhD program. The MS in HHP with thesis curriculum is designed to prepare such students from within The University of Iowa or from other US or international institutions for a doctoral degree program. Furthermore, we anticipate that the MS in HHP with thesis will also serve as a feeder into our own PhD program. PhD in Integrative Physiology: The new Department of HHP inherited this PhD program from the previous (and now closed) Department of Integrative Physiology. As of spring semester 2012, five students are enrolled in the PhD in Integrative Physiology. Reflecting the expertise of the faculty of the Integrative Physiology department at the time the PhD program was designed over two decades ago, this program offered focus areas in Anatomy, Biomechanics, Exercise and Integrative Physiology, and Neural Control of Movement. Over the past several years, a number of faculty members left the department (primarily via retirement), while faculty from the Health and Sport Studies program brought in new expertise in the area of health promotion that is not reflected in the current focus areas of the PhD in Integrative Physiology. As a result, the current PhD in Integrative Physiology no longer reflects the overall expertise of the faculty of the department and is no longer in line with the mission of the graduate program. Thus, we propose to rename the PhD in Integrative Physiology to PhD in HHP and to restructure the PhD so that it will better serve the mission of the graduate program, reflect the overall expertise of the faculty of the department, and be better integrated into the overall design of the educational program of the department. There will be no official subtracks in the restructured PhD program. However, focus areas will reflect the expertise of the faculty of the department. PhD in Health and Human Physiology: This program will be specifically designed to: (1) better serve the mission of the graduate program to prepare future academicians for positions in related educational programs and (2) be better integrated into the overall educational programs of the department. We envision the revised PhD program in HHP as an integral part of the overall educational mission of our department. For example, we expect to recruit highly qualified students from the MS in HHP with thesis to the PhD program. Furthermore, the integration of the MS and PhD programs with the various undergraduate programs within our department may attract even more qualified undergraduate students to our Bachelor s programs. For example, freshman students may specifically select the BS in Human Physiology because it provides them with the opportunity to continue their education in the MS and subsequently in the PhD program. Thus, the revised graduate program may not only benefit from our strong undergraduate program, but the undergraduate program may also be strengthened by the revised graduate program opportunities. 7

New Graduate Level Courses To enrich the departmental offerings in graduate level courses we will: Develop two new courses Add graduate-level components (such as discussion sessions) to existing higherlevel undergraduate courses and offer them as co-existing courses with different course numbers for undergraduate and graduate students. Develop a Clinical Exercise Physiology Internship (described under Health and Human Physiology Internship Program ) Newly-developed courses Professor Gary Pierce, who joined our department in the fall of 2011, will develop a new course in Clinical Exercise Physiology. This course will be designed as an advanced undergraduate course with a special section for graduate students. The course will be designed to provide students with an understanding of recent advances in exercise physiology for clinical populations. Particular emphasis will be placed on the acute and chronic responses to exercise in patients with cardiovascular, pulmonary, and metabolic diseases. Specific topics to be addressed include pathophysiology of disease processes, clinical assessment of disease severity, diagnostic exercise and pharmacological testing, and exercise rehabilitation in clinical populations. In addition to these topics, the graduate portion of the class will also include topics in basic and intermediate electrocardiography and selected advanced topics in 12-lead electrocardiogram (ECG) and rhythm strip interpretation, case studies, and laboratory sessions in 12-lead ECG preparation. To complement the Integrative Physiology (27:241) and Motor Control (27:314) graduate seminars, Professor Kathleen Janz will develop and offer a new graduate seminar in Health Promotion. This seminar will address current topics in the areas of physical activity, diet, and health outcomes including clinical and community interventions. The Department of Health and Human Physiology is also actively involved in two of the University s new cluster hire programs the Obesity Initiative and Aging Mind and Brain Initiative. There are currently two tenure-track faculty searches being conducted in HHP associated with the Obesity Initiative, and it is anticipated that these faculty, and the Department s affiliation with these initiatives, will provide additional graduate curricular and training opportunities that fit extremely well with the revised graduate programs. New graduate-level sections for existing higher level undergraduate courses Several higher-level undergraduate courses offered by the department of HHP cover content that is highly relevant for the different graduate programs within the department. These are courses taught by tenure-track faculty within the department and cover topics 8

related to the expertise and research interests of the respective faculty. We propose to offer some of these classes as 200-level graduate courses. For example, the Cardiovascular Physiology lecture course (27:145) would also be offered as 27:245 (Cardiovascular Physiology for Graduate Students). Graduate students enrolled in the 200-level courses would participate in the formal lectures of the corresponding undergraduate courses 1 and in addition would also participate in weekly graduate seminars or discussion sessions. Topics addressed in the lecture portion of the course will be discussed in more detail and in greater depth. The format will include discussion of current literature in that field, guest lectures by researchers working in the respective areas, or hands-on demonstrations of research techniques. The following is a list of courses that we propose to offer at both the undergraduate and graduate levels: Course # Course Title SH Offered taught by 27:138 Exercise Testing & Prescription I: Metabolic Health 4 Fall Dr. Janz 27:141 Exercise Physiology 3 Fall Dr. Kregel 27:145 Cardiovascular Physiology 3 Spring Dr. Stauss 27:148 Physiology of Aging 3 Fall Drs. Collectively, these three courses will form the new HHP Internship Program. While the Practicum (27:152) and Internship (27:191) in Health Promotion are primarily designed for undergraduate students in the BA in HHP, the Practicum (27:152) will also be made available as an elective for graduate students in the revised MS in HHP 2. The new CEP Internship (27:2xx) will be designed primarily for graduate students in the MS in HHP (CEP subtrack) and will provide a learning environment outside the classroom and laboratory for the transfer of concepts, skill sets, theories and models to the professional practice setting. While meeting the academic needs of the student, the internship will also establish a closer relationship with clinical 1 Students who already took the class as an undergraduate student may enroll for 1 SH of Problems (27:200) and participate only in the graduate-level component of the course. 2 This option is primarily for students who want to use this course number to accumulate practical experiences required for certain certifications. 9

practitioners, provide a conduit for the exchange of ideas and practices among the faculty, the students, and practitioners, and provide service to the wider community. In doing so, the internship program will complement the mission of the Graduate Program of the Department of HHP. Distribution of Total Hours for Practica/Internships: For each SH of practicum (27:152) credit a student must commit 40 contact hours. The new Internship in CEP (27:2xx) will be offered for 3 SH and will require 60 contact hours per SH (180 contact hours total). The distribution of the total number of hours during the semester is to be predetermined by the organizational supervisor (practitioner) and the student and then reported to the departmental advisor/mentor, who develops a schedule for progress reports and evaluations. Compensation and Benefits: Graduate students in the MS in HHP (CEP subtrack) will earn 3 SH credit for the CEP internship (27:2xx) and may repeat the practicum (27:152) for up to 2 SH total (counting towards elective courses). In addition, graduate students derive significant career growth opportunities during these experiences. Students are expected to bear all expenses incidental to living in the area of the internship and to make satisfactory housing and commuting arrangements in order to carry out the assignments. However, many agencies and organizations offer to assist students through minimal salary, honorarium stipends, housing, commuter expenses, insurance, membership benefits, professional meeting expenses, and other benefits. Benefits such as these may be offered to the student. Practica are less time-intensive and, therefore, are typically completed earlier in the student s program and carry no financial benefits. University Liability Insurance Program: The University of Iowa provides a Student Malpractice Blanket Liability Insurance Program for students in practica and internships that are required as part of their academic program. Some organizations require the intern to hold professional liability coverage beyond that provided by the University. Professional liability coverage is available through many sources with varying coverage and costs. Collaborating Centers for the new Internship in CEP: We will capitalize upon the infrastructure and collaborations already established for the Practicum (27:152) and Internship (27:191) in Health Promotion. These existing collaborations include: UI Student Health Service/Health Iowa UI Cardiac Rehabilitation (CHAMPS) UI Respiratory Rehabilitation Johnson County Public Health UI Wellness 10

In addition, we consider exploring the following potential opportunities: UI Reaching Euglycemia and Comprehensive Health (REACH) Diabetes Prevention Clinic UI Pulmonary Infections (Breathing Services and Pulmonary Rehabilitation) Clinic Cardiac Rehabilitation Program, Mercy Medical Center, Iowa City, IA Non-invasive Vascular Care Services, Mercy Medical Center, Iowa City, IA Respiratory Care/Pulmonary Rehab Program, Mercy Medical Center, Iowa City, IA Diabetes Education Program/Pregnancy Diabetes Prevention Program, Mercy Medical Center, Iowa City, IA Cardiac/Pulmonary Rehabilitation Program, Mercy Medical Center, Cedar Rapids, IA Cardiac Rehab Program, Nassif Heart Center, St Luke s Hospital, Cedar Rapids, IA Research internships (e.g., human research studies conducted at the UI Cardiovascular Center) 11

Curriculum for MS in HHP Clinical Exercise Physiology (CEP) Subtrack The CEP subtrack within the MS in HHP is designed as a terminal Master s degree that will prepare students for professional employment by providing a solid academic education and specific skill sets in various areas related to clinical exercise physiology. In addition, internships in areas such as cardiac rehabilitation, pulmonary rehabilitation, diabetes prevention and management, noninvasive cardiovascular testing (e.g., exercise testing, echocardiography, Holter ECG), fitness evaluation, exercise prescription, health maintenance, health risk identification, and health education are an important component of the curriculum. As a terminal degree program, the CEP subtrack will be offered as an MS degree without thesis 3. We anticipate that most students in the CEP subtrack strive for professional certification from the American College of Sports Medicine (ACSM) in areas such as Clinical Exercise Specialist or Registered Clinical Exercise Physiologist. A large variety of elective courses will provide students with flexibility to tailor their individual curriculum to best prepare them for their future career goals and areas of certification. Most of the professional certifications require a certain number of hours of clinical/practical experience (e.g., Clinical Exercise Specialist : 500 hours; Clinical Exercise Physiologist : 600 hours) in cardiovascular, pulmonary, metabolic, orthopedic/musculoskeletal, neuromuscular, or immunological/hematological areas. To help students to cover a portion of this requirement, we will design a new Internship in CEP (27:2xx) as described under Health and Human Physiology (HHP) Internship Program above. These Internship opportunities will offer graduate students practical experiences that form a cornerstone of the curriculum of the CEP subtrack. Faculty members supporting the program: We will start this program with a small number of students and increase enrollments as we gauge our faculty numbers and their contributions. The course instructor has been contacted to resolve this issue. 16

Curriculum for MS in HHP with Thesis The MS in HHP with thesis is designed to academically prepare students with BS or BA degrees for doctoral programs in the broad areas of health promotion and human physiology. Because this curriculum is designed to prepare students for a doctoral program it will be offered only as MS with thesis. Health promotion is distinguished by its focus on physiological and behavioral processes as they interact with social and cultural forces to impact health. Scholarship in this area advances the current understanding of physical activity behavior and health outcomes associated with this behavior through the evaluation of theories that predict behavior, the modeling of physical activity patterns associated with health outcomes, and the testing of interventions that promote physical activity and improve quality of life. Students with a special interest in the area of health promotion may elect to focus on acquiring the analytical and research skills necessary to go on to doctoral programs. Human physiology is the study of how the human body responds to external and internal stimuli, including physical exercise and stress, environmental conditions, aging, and disease. Scholarship in this area requires in-depth knowledge in the natural sciences, including biology, chemistry, biochemistry, biomechanics, mathematics and statistics, physics, anatomy, biomechanics, neuroscience, and physiology. Students with a special interest in the area of human physiology will acquire knowledge of the interrelated functions of human organ systems and gain hands-on skills in experimental techniques in the areas of biomechanics, cardiovascular physiology, and/or exercise physiology using human subjects, experimental animals, isolated tissues, or cell culture systems. 18 Prerequisite Coursework: Students entering the MS in HHP with thesis must hold a BS/BA degree with a minimum GPA of 3.0. Basic knowledge in the areas of anatomy, physiology, and statistics are required. Therefore, courses in anatomy and physiology with laboratory sections as well as coursework in statistics are prerequisites for the MS in HHP with thesis. Typically, these prerequisites are met by a total of 8 SH of anatomy and physiology (3-SH lecture and 1-SH laboratory component for anatomy and physiology, respectively) and a 3-SH introductory statistics course 6. Total Semester Hour Requirements: 30 semester hours of graduate coursework which includes a maximum of 4 semester hours of Thesis MS (27:404). Required courses: All graduate students in the MS in HHP with thesis are required to take the following coursework: Course # 4 Course Title SH Offered One Advanced Statistics course, e.g.: 3 171:162 Design & Analysis of Biomedical Studies 3 Fall 22S:148 Intermediate Statistical Methods 4 Fall/Spring One Research Methods course, e.g.: 3 169:205:001 Research Methods and Leisure Behavior 3 Fall 07P:220:001 Quantitative Educational Research Methodologies 3 Spring 07X:150:001 Introduction to Educational Research 3 Fall/Spring Graduate Seminar Courses (4 enrollments at 1 SH each) selected from: 27:241 Integrative Physiology Seminar 1 Fall/Spring 27:314 Seminar in Motor Control 1 Fall/Spring 27:2xx Seminar in Health Promotion 1 TBD 650:270:0002 Principles of Scholarly Integrity (takes 2 semesters to complete) 0 Fall/Spring 27:404 Thesis MS maximum of 4 SH Fall/Spring 6 Students who lack some prerequisite coursework may be allowed to make up their deficiencies during the first two semesters. This make-up coursework will not count towards the total 30 SH requirements. 18
