1 STATE OF UTAH DEPARTMENT OF COMMERCE DIVISION OF OCCUPATIONAL AND PROFESSIONAL LICENSING NEWSLETTER March 1, The following actions have been taken by the Division following hearings conducted before an Administrative Law Judge and the respective Licensing Committees or Boards or have been entered by Stipulation or Memorandum of Understanding from February 1, February 29, PHYSICIANS/SURGEONS *CHOWDHURY, Arif A., MD License Nos /8905 Docket No Layton, Utah Date: February 7, 2012 *CURRIER, Nathan Robert, MD License Nos /8905 Docket No Salt Lake City, Utah Reinstatement Date: February 21, 2012 *SAZANI, Thomas Anthony, MD License No Docket No Orcutt, California Date: February 7, 2012 Respondent engaged in unprofessional conduct with respect to a patient in that he did not practice to the appropriate standard of care concerning the patient. Also Respondent's staff erroneously gave one patient another patient's medical records. Respondent's licenses to practice as a physician/surgeon and to administer and prescribe controlled substances were placed on probation for two years with conditions. The probation on Respondent's licenses to practice as a physician/surgeon and to administer and prescribe controlled substances was terminated and said licenses were reinstated with full privileges. Respondent's license to practice as a physician/surgeon in California has been disciplined in November 2009 and June Respondent admitted that the conduct alleged in Exhibits A and B (the California Orders) constitute unprofessional and/or unlawful conduct in this state. Respondent's license to practice as a physician and surgeon was revoked with a stay of enforcement on the revocation. Said license was placed on probation with conditions until the administrative probation in California is terminated.

2 DENTISTS/DENTAL HYGIENISTS *BATEMAN, Alphalus Harold License Nos /8903 Docket No Salt Lake City, Utah Date: Respondent engaged in unprofessional and unlawful conduct when he ordered quantities of a controlled substance which was shipped to his residence for his own use. Respondent did not possess a valid prescription for the controlled substance from a prescribing practitioner nor was Respondent lawfully administered the controlled substance. Respondent voluntarily surrendered his licenses to practice as a dentist and to administer and prescribe controlled substances. Respondent may not reapply for licensure for a period of five years and completion of conditions outlined in the *MYERS, John W., DDS License No Docket No Draper, Utah Reinstatement Date: February 17, 2012 ADVANCED PRACTICE NURSES None in February 2012 REGISTERED NURSES *CADY, Troy Kenneth License No Docket No Syracuse, Utah Reinstatement Date: February 23, 2012 *CHRISTIANSEN, Karen C. Respondent failed to comply with probationary conditions that existed on her license. Respondent's license to practice as a registered nurse was revoked with a stay of enforcement on the revocation. Said license is suspended for a period of at least six months. During the suspension period, Respondent will remain subject to the probationary conditions and restrictions identified in the May 2009 Order in Case No. DOPL Respondent failed to comply with conditions of a diversion agreement which existed on his license. Respondent's participation in the Utah Professional Recovery Assistance Program (diversion) and his diversion agreement were terminated immediately. Respondent's license to practice as a registered nurse was revoked with a stay of enforcement on the revocation. Said license was placed on probation for five years with conditions and restrictions. The suspension on Respondent's license to practice as a registered nurse was terminated and said license was placed on probation for five years subject to the conditions and restrictions identified in the July 27, 2011 Stipulation and Respondent no longer wants to comply with probationary conditions that exist on her license. Respondent voluntarily surrendered her license to practice as a registered nurse. A license to practice as a journeyman electrician was issued and said license was placed on probation for two *SHORT, Jason Terry Apprentice Electrician License No Docket No Salt Lake City, Utah Respondent engaged in unlawful conduct when he acted in a construction trade beyond the scope of licensure he held as an apprentice electrician. Respondent's license to practice as an apprentice electrician was revoked effective the date of the Respondent was also ordered to pay a $1,000 fine within 30 days. *SUPERIOR HARDWOOD & TILE, LLC License No Docket No Cedar City, Utah Respondent's license to practice as a contractor was placed on probation for eight *THERMAL FLOORS, LLC License No Docket No Midvale, Utah Respondent failed to replace its qualifier. Date: February 22, 2012 *COLTON, Wendy Dawn Private Probation Provider License No Docket No American Fork, Utah Reinstatement Date: February 8, 2012 The probation on Respondent's license to practice as a private probation provider was terminated and said license was reinstated with full privileges. *COX, Gary L. Vocational Rehabilitation Counselor License No Docket No Syracuse, Utah Respondent engaged in unprofessional conduct in his practice as a vocational rehabilitation counselor when he made comments to a female client. Respondent voluntarily surrendered his license to practice as a vocational rehabilitation counselor. Respondent may not reapply for this type of license for a period of five years. Respondent's license to practice as a cosmetologist/barber was reinstated and said license was placed on probation for two years with conditions and restrictions. Respondent engaged in unprofessional conduct and violated NASW Code of Ethics in her practice as a licensed clinical social worker when she engaged in a relationship with a former client. Respondent's license to practice as a licensed clinical social worker was revoked with a stay of enforcement on the revocation.
