1 Dear Braiding our Resources 2012 Participant, Welcome to The Culinary Institute of Canada s Customized Culinary Boot Camp for Early Childhood Development! We are delighted that you have decided to join us in what is promised to be a fun culinary experience that sends you on a taste journey around our island. Be prepared to discover your hidden kitchen talents! To ensure a FUN and SAFE environment for all of our camp participants, we ask that you review and familiarize yourself with the enclosed information about our camp. This information outlines important camp details and a number of forms that must be completed and brought with you to the first day of camp. In order to participate in all activities, each camp attendee must complete the required forms prior to the first day of camp. Once again, on behalf of Holland College and the Culinary Institute of Canada, we look forward to sharing our culinary culture with you while making great friends along the way. 2 Camp Beginning/Ending Times Camp Start time/end Our boot-camp begins at 1:00 pm at the Culinary Institute located on 4 Sydney Street, Charlottetown and we ask that everyone arrive between 12:30-12:45pm.The camp will end at, approximately, 3:45-4:00pm. What to Expect Our boot camps are designed to provide a fun, unique and memorable learning experience. We encourage you to bring your camera to capture the memorable moments here at The Culinary Institute of Canada. What to wear - Please make sure that you wear light clothing and comfortable shoes because you will be wearing the chef's whites and on your feet a good portion of the day. What not to wear - open sandals, shorts, dangly jewelry, and ipod or mp3 players. Boot-Camp Contacts If you are going to be late, or away due to illness, etc. please contact Lindsay Arsenault before 8:15 AM at (902) Student Conduct Behavioral Expectations Camp participants are expected to conduct themselves in a mature, co-operative, respectful manner while attending a camp at the Culinary Institute of Canada. Students will be held responsible to the head chef instructor and staff for their behavior and conduct while in attendance at our facility Defiant, Offensive or Dangerous Behavior Behaviors that are deemed unacceptable will result in expulsion from your boot camp. A refund for the camp will not be given. These include:

3 Preventing or interfering with the camp experience of others Threats, harassment, intimidation, abuse, or fighting Being under the influence of or in the possession of drugs or alcohol Use or possession of weapons or harmful objects Vandalism of the Culinary Institutes property Using obscene or abusive language Failure to cooperate with staff or being disrespectful to staff Endangering oneself, other students or staff Please contact our office, if you would like to report any form of harassment or discrimination during the course of the camp. (902) Vandalism and Theft Boot camp participants involved in any theft, loss of, or damage to the Culinary Institute of Canada s property are financially responsible. All instances of vandalism and theft are reported and investigated. Care of Personal Property The Culinary is not responsible for the loss and damage of personal property. Camp participants are advised to leave items of value at home. Cell Phones/Electronics Cell phones and other electronic devices must be turned off during class time. Appropriate cell phone etiquette is expected in all situations.

4 WAIVER OF LIABILITY AND INDEMNITY AGREEMENT READ CAREFULLY BEFORE SIGNING This document relates to: Boot Camp: Customized Culinary Boot Camp for Early Childhood Development May 4, 2012 Participant s name: X Whereas Holland College and the Culinary Institute of Canada provides an opportunity for X to participate in Customized Culinary Boot Camp for Early Childhood Development Boot Camp; which consists of culinary activities. Unfortunately, sometimes the chance of risk of injury is inherent with respect to physical activity. Accordingly, all participants must reduce their chance of injury. The safe conduct of any group activity, such as those conducted at Holland College and the Culinary Institute of Canada, relies upon the individual actions of each member of the group. The participant has an obligation to refrain from dangerous or disruptive activity that might endanger the participant or any other member of the group. In consideration of being allowed to participate it is important that the participant understands and agrees that: 1. I/We are fully informed or otherwise aware of and fully assume any and all risks or property in connection with the participant s participation in the Camp (including but not limited to damage and loss of property, bodily injuries, medical treatment and death). The participant is in good physical and mental health and does not have any physical conditions, which could affect their ability to participant in the Camp. 2. I/We understand that the participation in the Camp is discretionary and that the I/We may choose not to participate at any time. 3. I/We fully and forever RELEASE, WAIVE DISCHARGE, AND COVENANT NOT TO SUE, Holland College and the Culinary Institute of Canada, its governors, directors, officers, employees, agents and volunteers (all of whom are hereinafter collectively referred to as the Releasees ) and to release the Releasees from any and all demands claims, actions, suits, damages, losses, liabilities, costs and expenses arising, directly or indirectly, in connection with participation in the Camp, form any cause whatsoever (including but not limited to, damage or loss of property, bodily injuries, medical treatment and death) 4. I/We shall INDEMNIFY AND HOLD HARMLESS the Releasees for and form any and all demands, claims, actions, suits damages, losses liabilities, cost and expense, arising directly or indirectly as a result of the participants intentional or negligent acts or omissions from any cause whatsoever (including but not limited to damage and loss of property, bodily injury, medical treatment and death) whether or not foreseeable or contributed to by the negligent act or omission of the Releasees or others. 5. I/We shall fully comply with all applicable laws, Holland College and the Culinary Institute of Canada policies in the Camp. If the participants participation in the Camp is at any time deemed, detrimental to any other participant s as determined by Holland College and the Culinary Institute of Canada in its sole discretion, I/We understand that (i) I/We may be expelled from the Camp without Holland College and the Culinary Institute of Canada incurring any liability, (ii) I/We may incur

5 liability to Holland College and the Culinary Institute of Canada under this Agreement, and (iii) I/We may be subject to further disciplinary action by Holland College and the Culinary Institute of Canada. 6. I/We have read and fully understand this Agreement, (ii) intend that this Agreement be legally binding upon and enforceable against the participant and family members, estate, heirs, and legal representatives, (iii) intend that this Agreement inure to the benefit of Holland College and the Culinary Institute of Canada and, (iv) confirm that I/We are entering into this Agreement voluntarily and of our judgment. 6 Privacy Release I hereby give permission for Holland College and the Culinary Institute of Canada to use pictures of myself/my child/children for use in their brochures, for marketing purposes and to be used on the website. I also agree that the visual images and or audio recordings that have been made by my child/children become the property of the Holland College and the Culinary Institute of Canada. I give Holland College and the Culinary Institute of Canada the absolute and irrevocable right to the images and recordings. I release and forever discharge Holland College and the Culinary Institute of Canada, its employees, agents or assignees from any and all claims and demands arising out of or in conjunction with the use of the visual images and/or audio recordings.
