2 WELCOME! Good afternoon (evening) folks, I m Dr...., your host for this surgical procedure. Welcome to the Oklahoma State Fair and the Oklahoma Veterinary Medical Association s Surgical Suite. All members of the surgery teams, doctors and technicians have graciously donated their time and talents for this educational endeavor. The members of this surgical team are: Note: You do not have to use this booklet word for word, and you may add information you feel necessary. This booklet is just a guide. 3 Ovariohysterectomy The female patients are required to have this surgery performed before they can be adopted from the Oklahoma City Animal Shelter. This procedure called an ovariohysterectomy (spay) not only prevents this patient from having puppies, and reduces pet overpopulation, but minimizes the number of animals that have to be euthanized. It also improves her health in other ways. 1. Prevents an infection of the uterus 2. Eliminates ovarian cancer 3. Reduces the risk of breast cancer If you will direct your attention to the visual aids (overhead, chart, slides, screen), I will explain (walk you through) (guide you) through the procedure. I will also guide you through the surgery as it is performed. I will be happy to answer your questions upon completion of the procedure. 4 The Preparation Procedure 1. The patient fortunate to be selected for this surgery is admitted to the preparation room. 2. If the patient exhibits anxiety or worry, a pre-anesthetic medication is administered by the doctor. 3. An IV catheter is place in the front leg and an IV drip started. 4. A medication is administered to relax the patient. An endotracheal tube is placed in the trachea (windpipe) by the doctor. A. This allows the patient to breathe O2 and anesthetic freely. B. The windpipe is the tube that runs from the throat to the lungs. 5. A pain medication is given. 6. When the patient is fully anesthetized, the surgery technician prepares the patient for surgery in the surgery prep area. A. Expresses the bladder B. Shaves the surgery site and surrounding area and prepares the area where the doctor will make the incision. C. The area is scrubbed with a surgical scrub soap at least three times. 7. The doctor and the surgery technician continually monitor the patient s vital signs physically and with instruments. **Explain what monitoring equipment is being used. The doctor scrubs caps, masks, gowns, gloves and boots assisted by the surgery technician. 8. When the patient is fully prepared, she is moved from the surgery preparation area to the surgery room and placed on the surgery table on her back with her tummy up. She is made comfortable and secure on the table insuring all of the monitoring devices and IV fluids continue to be in place. 9. Keep in mind the doctor and the surgery technician continue to monitor the patient s vital signs physically and with instruments. It is time to mention that we are now moving into a sterile environment - the surgery room. All of the apparel, instruments, drapes, etc. have been sterilized. 10. We are now in the surgery area everything has been done to create a sterile environment. Page 4 of 17

5 A. You will notice the doctor is dressed in sterile garb. B. You will also observe the surgery technician is dressed in sterile attire. The Surgery Procedure 1. The surgery technician opens the first layer of the surgery pack and presents the surgeon items he or she might request. 2. The doctor towels and drapes the prepared incision site. 3. The surgeon makes an incision into the abdomen exposing the abdominal area. 4. Controlling the small amount of hemorrhage (bleeding) that may occur. 5. The doctor and the technician continue to monitor the vital signs physically and with instruments. 6. The surgeon using a small instrument (called a spay hook) delivers one horn of the uterus through the incision. With his fingers the surgeon identifies the ovary and frees the ovary from its tissue attachments. The ovary is then delivered through the incision. 7. The vessels to the ovary are ligated (tied) to prevent bleeding when the vessels are severed to remove the ovary from the abdomen. The vessels and tissue are then severed (cut) to release the ovary and it is removed from the abdomen. 8. The second horn of the uterus and ovary are treated in the same manner. 9. Now they have both ovaries and both horns of the uterus out of the abdomen. 10. In order to complete the ovariohysterectomy they must remove both ovaries, both horns of the uterus and part of the body of the uterus. 11. They now expose both horns of the uterus with the ovaries attached anterior (front end) and move back (posterior) until they expose an area where the horns of the uterus divide. 12. Another ligature (tie) is placed posterior (behind) the bifurcation (where the two horns of the uterus divide). This ligates (ties) the vessels in this area and closes the opening of the body of the uterus. 13. The surgeon then severs (cuts) the area anterior (in front) of the Page 5 of 17

6 ligature (tie). The ovaries, both horns of the uterus, and a portion of the body of the uterus can be removed from the abdomen. 14. The incision (hole) in the abdomen is ready for closure. 15. The incision is closed in two or three layers. The first and second layers with absorbable sutures. The skin with removable sutures. 16. The surgery is now complete. The patient is moved to the recovery area and monitored until the tracheal tube can be removed and recovery is achieved. 17. When the patient is mobile, it is safe to move her to a normal hospital room. 18. The patient may be dismissed the same day to be observed by the parents (owners) or spend one night in the hospital for observation and pain medication administered if required. 19. Questions? 7 Orchiectomy The male patients are required to have this surgery performed before they can be adopted from the Oklahoma City Animal Control facility. This procedure is called an orchiectomy (castration). It not only prevents this patient from producing more puppies, and reduces pet overpopulation, but minimizes the number of animals that have to be euthanized. It also improves his health in other ways. 1. Prevents an infection of the testicles. 2. Eliminates testicular cancer 3. Reduces the risk of aggressive behavior 4. Reduces risk of prostate problems If you will direct your attention to the visual aids (overhead, chart, slides, screen), I will explain (walk you through) (guide you) through the procedure. The surgeon brings the two testicles out through the incision. 7. The vessels to the testicles are ligated (tied) to prevent bleeding when the vessels are severed to remove the testicles from the scrotum. The vessels and tissue are then severed (cut) to release the testicles and they are removed from the scrotum. 8. Now they have both testicles out of the scrotum. 9. In order to complete the orchiectomy they must remove both testicles. 10. The surgeon then severs (cuts) the testicles and removes them from the scrotum. 11. The incision (hole) in the scrotum is ready for closure. 12. The incision is closed in two layers. The first layer is closed with absorbable sutures. The skin is closed with removable sutures. 13. The surgery is now complete. The patient is moved to the recovery area and monitored until the tracheal tube can be removed and recovery is achieved. 14. When the patient is mobile, it is safe to move him to a normal hospital room. 15. 10 Trivia and Fun Fact Filler (use if you wish or provide your own) 1 Each day 10,000 humans are born in the US - and each day 70,000 puppies and kittens are born. As long as these birth rates exist, there will never be enough homes for all the animals. As a result, every year 4 to 6 million animals are euthanized because there are no homes for them. 2. Two dogs and their offspring, if not spayed or neutered can produce 67,000 puppies in 6 years. Supporting the Pet Overpopulation Fund will help reduce the number of stray and unwanted pets. Find out how you can help by visiting the Oklahoma Veterinary Medical Association s website 3. An estimated 100,000 unwanted stray animals are killed in Oklahoma each year because there are not enough homes for adoption. 4. An effective method of reducing pet overpopulation is to provide surgical sterilization for pets 5. Surgical gut suture is erroneously called catgut, a slang derivative of kitgut. A kit was a small three-stringed fiddle which used gut strings. Kit strings were an early form of suture, but were manufactured from sheep, not cat, intestinal submucosa. After washing, the submucosa is tanned with or without chromium salts being added. After tanning, the fiber is twisted, but twisting results in a rough or lumpy surface which would cause tissue drag. To prevent drag, the twisted is polished to smooth the surface. Polishing lessens drag, but also removes suture substance creating foci where the cross-sectional mass is reduced and the suture weakened. 6. In 1960 there were only 18 Veterinary Colleges in the U.S. - Today there are 28, graduating about 2,100 students a year, with more than 8,500 students enrolled. Approximately 75% of entering students in US Veterinary schools are women. Oklahoma has a veterinary school, it is located at Oklahoma State University. 7. OSU Veterinary college opened in 1951 with 26 students and has graduated 3322 total veterinarians as of May OSU veterinary school accepts approximately 80 students per year. Over 400 students apply each year. Of the 80 accepted, 56 (70%) are Page 10 of 17

11 Oklahomans. This year approximately 76% of OSU veterinary students are female 9. The DVM program is a four year program. Before practicing veterinary medicine, graduates must also pass national and state licensing exams. In Oklahoma, DVM's must renew their license each year and must have a minimum of 20 hours continuing education each year. 10. In 2004 there were 8.8 first year vet students per million population in the USA 11. The state of Oklahoma currently has 1343 practicing & licensed veterinarians and newly emerging support group of 354 registered technicians! 12. Veterinarians are in many aspects of our lives: Small animal or companion animal Large animal which covers horses and food animals Federal and State public health Poultry (chicken/turkey/fowl etc) medicine Zoo and/or state wildlife medicine Marine or aquatic parks and oceans Research and/or lab animal medicine Academic or teaching positions Toxicologist - a lot like the CSI involvement! 13. If you have an interest in working on a professional level with animals please see us after the surgery. We can give you phone numbers or contact information for veterinary technology which is a 2-year program or the 4-year doctorate program at Oklahoma State University College of Veterinary Medicine. 14. When is the usual time to spay or neuter my dog? 5-6 months 15. Neutering is often recommended for a male dog that is aggressive. 16. How long is the gestation period of dogs? 2 month days 17. I d like to have my pet spayed or neutered but I m worried about the anethesia. How risky is it? Placing a pet under anesthesia is a very common concern of Page 11 of 17

12 owners. Although there is always a slight risk involved, the anesthetics currently used by veterinarians are very safe. Many veterinarians use equipment that monitors heart and respiratory rates during surgery to ensure that their patients are doing well under anesthesia. Thus, the medical benefits of having your pet spayed or neutered far outweigh the slight risk involved with undergoing anesthesia. Consult your veterinarian if your are concerned about this aspect of the procedure. 18. Will neutering them change their personalities? Any change will be for the better. After being altered, your pet will be less aggressive toward other dogs or cats, have a better personality, and will be less likely to wander. Spraying (urine marking), which is often done by dogs and cats to mark their territory, diminishes or ceases after pets are altered. 19. How long do they have to stay in the hospital? Just for the day, they do not have to stay overnight. Anesthetics are much better than they used to be, and pets are awake and stable by the afternoon of the surgery. They are much less stressed at home than they would be in the hospital overnight surrounded by strange animals and sounds. They will get more attention at home than they would when we all go home for the evening. It also is one way that we keep costs down, by avoiding the expenses associated with overnight care. 20. What s the big deal about spaying & neutering? Pet over population! Most people don t realize how serious this problem is. To say that there are too many pets and not enough homes doesn t convey the magnitude of the problem. The leading cause of death in pets in the United States is not disease or illness or injury.. it s being killed in our nation s pounds and shelters. Five million dogs, cats, puppies and kittens are killed just because there aren t enough homes to go around. That s about five percent of the entire pet population each year. So PLEASE, PLEASE.. spay and neuter your pets!! Encourage everyone you know to do the same. 21. I want my children to witness our pet giving birth. Pets often have their litters in the middle of the night or in a place of their own choosing. Because pets need privacy when giving Page 12 of 17

13 birth, any unnecessary intrusion can cause the mother to become seriously upset. These intrusions can result in an unwillingness to care for the offspring or in injury to the owners or to the pet. 22. Is the expense for the surgery really worth it? Yes! This is a one-time expense that can dramatically improve your pet's quality of life. If you are still uncertain whether or not to proceed with the surgery, consider the expense to society of collecting and caring for all the unwanted, abused, or abandoned animals being housed in shelters most with little chance of finding permanent homes 23. Having your pet spayed or neutered is a part of responsible pet ownership and an important investment in your pet's long-term good health. 24. Communities spend millions of dollars to control and eliminate unwanted animals. Irresponsible breeding contributes to the problem of dog bites and attacks. Animal shelters are overburdened with surplus animals. Spay and Neuter Convenience to Owners Eliminates estrous or "heat" periods; no bloody discharges. Eliminates the scent that attracts annoying males. No need to confine your female while in heat. Reduces aggression against other animals Reduces the annoying and embarrassing urge of male dogs to "mount" children and adult's legs. Reduces the risk of prostate problems and testicular tumors later in life One male running loose for just a few hours can impregnate many females adding to the serious problem of unwanted puppies Better Health For Your Pet Eliminates all the problems and potential risks involved in pregnancy and birth. 14 Eliminates the common problem of cancer of the uterus as the pet grows older. Decreases the possibility of mammary tumors. Helps Decrease The Pet Overpopulation By not bringing more unwanted puppies and kittens into the world. Rids you of the worry of what to do with unplanned litters of puppies and kittens. Eliminates Sexual Frustration Decreases your pet's desire to roam in search of a mate; decreasing the problems with a free-roaming animal. Lets your pet relax and enjoy being part of the family. MYTHS She should have one litter first. NOT TRUE It is actually better for her not to have a litter or a heat period before being spayed. Spaying will hurt her. NOT TRUE Spaying is safe and relatively painless operation done by a registered veterinarian. Your pet will appreciate the freedom from sexual frustration or repeated parenthood. I will find good homes for the puppies or kittens. NOT TRUE You may be able to place your puppies or kittens, but are they in "good" homes and "permanent" homes? And remember each time you place one of your puppies or kittens, somewhere else an animal is being killed because there was no home for him. Spaying will make your pet fat. NOT TRUE Spaying will not make her fat and lazy. Too much food and not enough exercise is the main cause of obesity. Neutering will take away the "guard dog" instincts. NOT TRUE Neutering a dog does not reduce its ability as either a guard or watch dog. They will still be as protective of their territory as they were before the surgery. Neutering makes pets fat. 16 Pet Overpopulation Fund FAQ s What is the Pet Overpopulation Fund? It is a fund that was established by the 2004 State Legislature to render financial support for pet sterilization for low income families and to provide educational programs on responsible pet ownership. Where can I find information on the Pet Overpopulation Fund? Rules governing the Pet Overpopulation Fund can be found in Title 35 Oklahoma Department of Agriculture, Food, and Forestry; Chapter 15. Animal industry. Sub chapter 12. Pet Overpopulation Fund. (35: ). You may also contact the Administrator; Oklahoma Veterinary Medical Association, P. O. Box 14521, Oklahoma City, OK How is the Pet Overpopulation Fund funded? The Pet Overpopulation Fund is supported by the purchase of Animal Friendly special license plates, state income tax refund check off program, and by donations. How can I purchase an Animal Friendly License plate so I can support this program? Applications for Animal Friendly license plates can be obtained from the Oklahoma Tax Commission s special plate division or from the Oklahoma Veterinary Medical Association s (OVMA) web site; What is the cost of the Animal Friendly License plate? The cost for an animal friendly license plate is thirty five ($35) in addition to the regular license fee. Twenty ($20) will be contributed to the Pet Overpopulation fund. What constitutes a low income owner? Any Oklahoma resident that is participating in a state assistance program. Will owners income be verified to qualify in the low income pet sterilization program? Yes. Applicants will be qualified by verifying proof of a Medicaid Card, Social Security Award Letter, or an Electronic Balance Transfer (EBT) Card, which verifies eligibility in the Food Stamp, Women s, Infants & Children(WIC), Temporary Assistance for Needy Families(TANF) or Day care programs. How can owners apply for the low income sterilization program? Applicants will have to obtain and complete an application form. Page 16 of 17

17 Applications can be obtained from the OVMA or the OVMA Web Site; Are all veterinarians eligible to participate in the surgical program? No. Only veterinarians that are licensed in Oklahoma and are members of the Oklahoma Veterinary Medical Association are eligible to participate. How do veterinarians volunteer to participate? Eligible veterinarians must submit an application. Applications can be obtained from the OVMA Web Site, What organization will be administering the program? The Pet Overpopulation Fund will be under the auspices of the Oklahoma Department of Agriculture, Food, and Forestry and they have contracted with the OVMA to administer the program. Will the owner be required to sign a surgical consent form? Yes. A standard surgical consent form will be included on the application for surgery and must be signed and dated by the animal s owner or agent. What if the owner requests a rabies vaccination? If a vaccination is requested, the participating veterinarian will agree to give the vaccination for five ($5) dollars. This will be an added benefit to the owner because of the public health significance of Rabies. How does the low income family select the veterinarian for the surgical procedure? Once the applicant has been approved, they will receive an approval form and a list of participating veterinarians in their area. It will be the applicant s responsibility to contact the participating veterinarian, make a surgical appointment, and receive pre-surgical instructions. How soon does the applicant have to schedule surgery? Sterilization surgery must be performed within fifteen (15) days of the application s approval date. Can the surgical procedure be delayed? The procedure can be delayed only if the participating veterinarian determines that a delay is necessary for medical reasons. Appropriate written documentation with an explanation must be provided to the administrator. Page 17 of 17
