1 To: Full-time Employees From: Patrice Masterson Director of Human Resources Date: October 12, 2015 Re: Open Enrollment and Benefits Information The 2015 open enrollment period for Georgia Perimeter College s benefits plans will be held from November 2 - November 13, If an employee wants to keep the same benefits coverage from 2015, he/she will not be required to make new elections and his/her 2015 elections will roll over to 2016, except Flexible Spending Account elections. As always, a Flexible Spending Account election is required each year. Tobacco re-certification will not be required this year unless an employee makes a change during open enrollment. Information about most benefits plans may be obtained from the campus information meetings, the USG Human Resources website, as well as the GPC Human Resources website. According to the Affordable Care Act, all individuals and their family members must have health insurance and will be required to provide proof of insurance when they file their taxes. If one does not have health insurance, they will be required to pay a penalty. Employees may obtain health insurance from their employer, a spouse s employer or through the Healthcare Marketplace. Open enrollment is the only time you may enroll in GPC/USG health insurance unless you experience a family status change mid-year. Please keep this in mind as you make decisions about your benefits. Please note that the University System of Georgia healthcare plan meets the affordability requirements under the Affordable Care Act. Therefore, generally, employees eligible for the USG healthcare plan will not be eligible for tax credits or subsidies in 2016 through the Health Insurance Marketplace created under the Affordable Care Act. A summary of your present coverage may be found online through Employee Self Service at Reviewing this information will allow you to make necessary decisions concerning your benefits. Changes made during this open enrollment period will become effective January 1, An open enrollment worksheet will be mailed to your home to assist you in preparing to make benefits changes online through Employee Self Service. 2 The GPC website for open enrollment may be found at The following open enrollment information may be found on this website: USG Comparison Guide USG/GPC Benefits Summary GSU/GPC Voluntary Benefits Information and Enrollment Forms Open Enrollment Letter Open Enrollment Instructions Human Resources will hold campus information meetings to describe the changes to health insurance plans and to describe the new voluntary benefits plans as well as to answer your questions. Each session will begin with a 60 minute presentation on the benefits plans that will be in place in Plan Year 2016 and the new voluntary benefits plans followed by time for questions. Sessions are scheduled as follows: Campus Alpharetta Clarkston Decatur Dunwoody Lakeside Newton Date October 15 October 28 October 29 October 27 October 22 October 21 Time 2:00-3:30 3:00-4:30 2:00-3:30 2:00-3:30 10:00 am - 11:30 am 2:00-3:30 Location AA1140 CL1100 SF2100 NC1100 Lakeside #4 1N2210 Please consider the following information as you make decisions about your benefits: Healthcare Plan Changes In 2016, the University System of Georgia will continue to offer 4 plan options to our active employees and their dependents and our pre-65 retirees and pre-65 retiree dependents. The most significant change to the USG healthcare plans is that 65 and older retirees and 65 and older retiree dependents will be enrolling in their supplemental healthcare coverage through the Aon Retiree Health Exchange. USG will be contributing $2,736 annually to a Health Reimbursement Account (HRA) for each eligible retiree and/or dependent enrolling in coverage through the exchange. Retirees will be able to use HRA funds to reimburse themselves for monthly premiums and other eligible medical and pharmacy plan expenses. An administrative manual which will include eligibility rules, retirement processing rules and funding process will be available in the next few months. We are adding a few new programs for active employees with very few changes to the healthcare plan designs. Additions to the BCBSGa healthcare plans:

3 LiveHealth Online this is an online doctor visit with Rx capability. Co-pay for online visit is $15 for the Comprehensive Care and BlueChoice HMO plans. The Consumer Choice HSA plan will have a reduced online visit amount prior to a participant satisfying the deductible. CVS MinuteClinic discounted office visits A clinic visit will be a $15 co-pay processed through pharmacy benefits. The Consumer Choice HSA plan will have a reduced visit amount prior to a participant satisfying the deductible. Changes to our healthcare plans and other benefits: Decrease in the in-network family out-of-pocket maximum on the Consumer Choice HSA plan from $7,000 to $6,850 to comply with ACA requirements New rate tiers for the pre-65 retirees and pre-65 dependents will be published in the 2016 Comparison guide separately; 65 and older retirees will be required to enroll their pre-65 dependents in coverage during Open Enrollment; 12% increase in the dental premiums for both the high and base plans Continue to move towards defined contribution premium model in which the employer cost will be the same for each plan Tobacco certification will default to the prior year election. If the employee makes a change during open enrollment, he/she will be prompted to certify his/her tobacco use and for dependents 18+ if enrolled in a USG healthcare plan Domestic partners and domestic partner dependent children will no longer be eligible for coverage due to the new same sex spouse ruling. Health Savings Account annual family contribution limit increase from $6,650 to $6,750 Kaiser Sr. Advantage healthcare plan will no longer be a healthcare option; pre-65 Medicare eligible retirees and/or pre-65 Medicare eligible dependents will be enrolled in the Kaiser HMO plan with active employees For Plan Year 2016, the University System of Georgia will continue with the 4-tier premium structure for our active employees enrolled in a USG healthcare plan. The four tier designations are: Employee only, Employee + One Child, Employee + Spouse, and Family. The dental and vision plans will continue to use the 4-tier premium structure as follows: Employee only, Employee + Child(ren), Employee + Spouse, and Family. This year, our Open Enrollment will be a passive enrollment for benefits and the tobacco surcharge. If an employee does not want to make any changes or new elections for open enrollment, their current coverage will remain the same for Also, the tobacco surcharge will remain the same as the current election if the employee does not need to change the tobacco status in If the employee does make changes and is enrolled in a USG healthcare plan, the tobacco certification will need to be completed in the enrollment system. As always, a Flexible Spending Account election is required each year. CVS/Caremark will remain the Pharmacy Benefit Manager for all of the self-insured healthcare plans administered by BCBSGa (Consumer Choice HSA, Comprehensive Care, and BlueChoice HMO). We will also be adding the following programs for Specialty Drugs and Injectable Drugs. Advanced Control Specialty Formulary: The Advanced Control Specialty Formulary is an expansion of CVS s current step therapy program for specialty medications. Specialty

4 medications are used to treat complex conditions and may require injection, special handling and/or monitoring. Some examples of Specialty medications are those used for conditions such as Rheumatoid Arthritis, Multiple Sclerosis, Hepatitis C, and Cystic Fibrosis. The therapy classes chosen for Advanced Control Specialty have multiple specialty drugs available that have similar safety and effectiveness. In these select specialty drug classes, employees may be required to try a preferred medication before a non-preferred medication in the class. If necessary, the employee s doctor can submit a request for coverage of a non-preferred product through a medical necessity review. Site of Care Alignment: Site of care alignment preserves member access to quality specialty infusion care at lower cost location. This strategy analyzes medication costs for clinician-infused drugs and identifies opportunities to achieve lower costs by shifting members to alternative sites of care. These infused drugs can continue to be billed and paid under medical, or coverage may be moved to the pharmacy benefit. Employees on BCBS Plans will not receive a new ID cards in 2016 unless the employee changes to a different plan or adds dependents. Employees enrolled in the Kaiser HMO will receive new ID cards. As a reminder, employees and non-medicare eligible retirees enrolled in a BCBSGa plan will not receive a separate card for their pharmacy benefit. Medical and pharmacy information is included on one card. Pre-65 Medicare eligible retirees enrolled in the Comprehensive Care plan will receive a separate ID card for their enrollment in SilverScripts, the USG Medicare D pharmacy benefit. The HSA match will remain the same in 2016 with a maximum match limit of $375 for individual and $750 for family coverage. The HSA maximum annual contribution limit for family coverage will increase by $100 to $6, Healthcare plan premiums Overall USG healthcare premiums will increase an average of 6.7%. Because we are gradually moving to a defined contribution pricing model, the percentage increase for 2016 is different by plan and are as follows: BlueChoice HMO 7%, Kaiser HMO 4%, Comprehensive Care 1.8%, and Consumer Choice HSA 20% 2016 USG Voluntary Benefit Plans Information Dental Plan Delta Dental Plan benefits remain the same as Premiums will be increasing 12%. This increase is a result of high utilization of each dental plan. Vision Plan - EyeMed No changes to the plan benefits or premiums. Life Plan Minnesota Life No changes to plan benefit or premiums. Employees will be allowed to increase 1x salary up to the lessor of 3x salary or $500,000 without EOI. This is a good time to make updates to beneficiaries. Short and Long Term Disability - MetLife No changes to the plan benefits. Long Term Disability premiums will increase 15%.

5 Flexible Spending Accounts and Health Savings Accounts US Bank No changes to the plan benefits or account fees. LifeStyle Benefits No changes to the plan benefits or premiums GSU and GPC Voluntary Benefits Plans Information We are very excited to announce the new voluntary benefit options for Georgia Perimeter College and Georgia State University. These important benefit options will be available starting on January 01, Group Critical illness insurance supplements your health insurance, providing a lump-sum payment if you are diagnosed with a serious illness such as certain cancers, heart attack, or stroke. Amounts of $10,000, $20,000 or $30,000 in coverage are available for you and your dependents. This benefit is insured by MetLife. Group Accident- This coverage complements your health insurance by providing a lump-sum payment for a covered event as a result of an accident that can be used as you see fit to help with out-of-pocket expenses, such as deductibles, copays and out-of-network care, or everyday expenses, like credit cards, child care and more. This benefit is insured by MetLife. Whole Life Insurance- Whole Life Insurance may be a great supplement to your employer-offered plan because it helps protect your loved ones should something happen to you for your entire life, not just while you're working. This benefit is insured by MetLife. EPIC Hearing Plan- EPIC s Hearing Service Plan offers you a national alliance of independent ear physicians and audiologists dedicated to high-quality hearing care. Your EPIC benefit ensures substantial savings between 30% and 60% on name-brand hearing aids and products to protect and improve your hearing. **** All the insurance products mentioned above are payroll deducted and you must complete a paper application to enroll. They are not available on the standard USG electronic enrollment site. Representatives will be available to discuss these services during the open enrollment period******* Additional Direct Bill Benefit Options MetLife Group Auto and Home- Provides a Group discount for Auto and Home coverage VPI- Veterinary Pet Insurance by Nationwide- Provides coverage for Accidents, Illnesses and Surgeries and provides a group discounts for your dogs and cats WHAT EMPLOYEES NEED TO DO -Make any changes to benefits in ADP. -Submit appropriate dependent documentation to Human Resources when it is requested. -Enroll or re-enroll in a Flexible Spending Account (FSA) if you wish to start or continue participation. -Opt in for text messaging-instructions on USG website. -Attend a Campus Benefits Meeting. -Review Open Enrollment materials.
