Growing Need for Local Services In 1988 there was a moratorium on new construction for on-reserve Personal Care Homes by the Government of Canada. First Nation Elder population expected to grow 230% over next 20 yrs with even greater need for care & support services.
Elders our Spiritual Leaders Elders are the heart of each community. Without these Spiritual & Cultural Leaders we are lost.
Caring for our Elders Living at Home known as Aging in Place is what most people want for their later years. Effective Assisted Living & Home Care Services should: - Strive to fit into daily routines that the Elder can rely upon - Deliver tailored needs that cannot wait for another day.
The Community Alert & Response for Elders (CARE): a Solution by First Nations for First Nations CARE is designed to help our Elders age at home with support provided through CARE Assistive Technologies. Keeping our elders in our communities contributes to preserving our rich cultural history and future.
CARE Offers Peace of Mind CARE is a Simple & Reliable Automated Call System. CARE gives peace of mind with the security that the service is checking in on our Elders on a regular basis. CARE provides reassuring calls made as often as required; updating on the Elder s day-to-day status; notifying family & caretakers.
CARE More Than a Call System CARE is more than a call system, it is a lifeline that will help to keep these Spiritual & Cultural leaders, who are so central to First Nation identity, where they belong residing safely and independently in their communities in their own homes.
CARE Bridges Gaps CARE bridges the gaps that otherwise challenge the ability for the Elder to age at home & in the community. Considered a non-medical social program, CARE does not duplicate existing services, rather it augments them. It does not replace the need for professional health or other services & may not be for everyone.
CARE Proven Technology The CARE service is a daily checkup call service that contacts Elders (on the days & times they specify) & greets them with a series of specifically recorded messages (you can even record your own message if you'd like!). The automated calling service is supported by proven computer-based technology.
Tailored Fit to YOUR First Nation! Messaging can be designed to address every individual needs & issues to deliver prompting reminders to: change their foot dressing related to diabetic foot ulcers, reminders to eat or take medications, reminder to check blood glucose levels to call in to Nursing Station or Health Care facility, reminder to exercise & more.
Benefits to Elders No direct cost to Elders who use the service or their network of family & caregivers Easy setup & maintenance free - CARE services works with any phone No additional hardware or software required Promote compliance & improve health outcomes
Benefits to First Nations & Health Service Delivery Organizations Keep your Elders safe & secure in their homes & a part of the community as long as possible Messages tailored to meet individual needs of each Elder Easy to get started & requires low-cost to establish & maintain Integrated delivery of tele-care services to your communities services & key contacts
Benefits to First Nations & Health Service Delivery Organizations Potential to manage staff resources & redirect elsewhere in the system More personal freedom for Elders & reduced burden placed on caregivers Helps staff, family & caregivers stay connected Reduced acute hospital admissions Increased support for post hospital discharge
Benefits to First Nations & Health Service Delivery Organizations Managed telephony technology service with low maintenance cost Enjoy a closer connection with the community & Elders your organization serves with bulletins & personalized messages Direct ability to shape the service to your community s needs (in any language), specific reminders for each Elder We work with you to create awareness & support for this Elder Assistive Technology service
CARE is the Solution CARE is a reliable, professional & proven service that is persistent and doesn t forget. CARE is the Solution
We ask you to join CARE. Step into a new dimension of community care & bring culturally-based & effective Aging Services into the home of your Elders. CARE Developed by First Nation People for First Nation People. Meegwetch
