3 Instructions on how to use this monitoring diary This diary is to help you keep a day-to-day record of your carbohydrate portions, insulin doses and blood glucose levels. By completing all this information over a number of consecutive days you should start to see a pattern to your blood glucose levels. This pattern can then help you to decide what, if any, changes to your insulins are required. To get the most out of using this diary we recommend that you: 1. Attend a carbohydrate counting group to learn how to estimate your carbohydrate portions accurately. 2. Test your blood glucose before each meal and at bedtime. 3. Record all the times that you eat, test or inject accurately. 4. Record any important comments about illness, exercise etc which may affect your test results. 5. Record hypos - any blood glucose level below 4 mmol/l. 6. Bring this diary to diabetes appointments with the nurse, dietitian or doctor so you can work together to review the patterns in your diary.
