2 The times they are a-changin Bob Dylan An additional 30 million people are likely to be added to the insurance pool by 2015 many of whom will be served by community health centers Accountable care organizations established through the ACA are already in place and more are likely to come providing coordinated care Value-based purchasing hospital providers scored and rewarded or penalized on the quality of care including discharges and readmissions.

3 And Still More Changes By 2016, offices of health practitioners are likely to be among the leading providers of healthcare (Council of Economic Advisors) 78 million baby boomers will increase demand for healthcare at the time that many in healthcare professions are also retiring. Electronic records and additional health information technology will become more widely adopted Billing and coding will become more difficult and will more heavily influence reimbursement rates with ICD 10 coding changes

4 The Front Line of Health Care Front line workers - community health workers, hospice aides, medical assistants, home health aides, nursing assistants, technicians, EMTs, etc. account for half of the 12 million people employed in healthcare Two-thirds have an annual family income of less than $40,000 a year Predominately female Have less than a Bachelor s degree

5 Demand for Front-Line Workforce Are the fastest growing segment of the healthcare workforce and will increase under the numbers will increase under health reform Personal and home care attendants projected to be the nation s second-fastest growing occupation through 2016 Home health aides projected to be the nation s third-fastest growing occupation through 2016 in Louisiana a 4% annual demand increase between now and 2018 Medical Assistants a 31% increase in demand by 2012 and a 3% annual demand in Louisiana through 2018

6 A Need for Better Training for Front-Line Professionals A growth in the elderly especially the oldest (85 and over) and frailest will require increased skill levels Those living with physical and mental disabilities are more likely to be living at home brought about through policy changes An increased emphasis on quality of care especially readmissions to hospitals and coordinated care Increased reliance on technology Ability to work in teams Improved critical thinking skills Needs Curator monitors a diverse set of data streams and determines appropriate care interventions Community navigators who are culturally attuned to manage care and finances Pattern Sensor identifies subtle variations in routine pattern mood change, physical activity suggesting in-person support is needed

9 Reasons for Coding Concerns ICD-9-CM diagnosis codes are 3-5 digits in length and number over 14,000 ICD-10-CM diagnosis codes are 3-7 characters in length and total 68,000 ICD-9-PCS procedure codes are only 3-4 numbers in length and total approximately 4,000 codes The ICD-10-PCS procedure codes are alphanumeric, 7 characters in length, and total approximately 87,000

10 CMS Recommended Refresher Education for Coders and Training Biomedical sciences anatomy, physiology, pathophysiology Medical terminology Pharmacology No training should take place before 6-9 months of implementation At least 2 full days for ICD-9-CM coders At least 50 hours (in addition to refresher courses) for ICD-10- PCS coders

11 Need for Coders BLS estimates a need for an additional 37,700 coders nationwide by 202 ad 21% increase A definite need for more than just on the job training In Louisiana an estimated annual growth of 1.6% -- one caveat this number comes from the Occupational Forecast in 2010 before we knew the full extent of ICD- 10

12 Health Information Technology Need more than a computer guy requires a new breed of professionals May require post-secondary education from certificate level to Ph.D. Professional who can integrate internal IT infrastructure with external networks and personal health technologies Visual Analytics Expert translates raw data into compelling infographics to make information meaningful Web developers, network administrators, and software engineers Privacy specialists

15 Food for Thought: Building the Workforce Provide career paths throughout the organization Establish human resource policies that encourage education and training tuition assistance, coaching, release time, onsite training Expand the responsibilities of front-line workforce Develop industry partnerships Don t exclude anyone we have a workforce shortage not a people shortage Explore various funding sources including federal workforce investment funds, community college funding, incumbent worker training

16 One Success Story Southeastern Arkansas College Fast Track to Health Credentials 95% of entering students required remedial work in reading, math or other basic skills some tested as low as 4th grade reading level Southeastern developed a fast track for LPNs and other allied health program compresses time required to complete basic education requirements to one semester of intensive work Following the one semester is an accelerated, intensive year program in nursing and/or allied health complete with intensive coaching and support from the college 96% completed the work and 80% passed the National Council Licensure Exam for Practical Nurses.
