Congratulations to Ms. Clare Mac Gabhann and all the staff who organised the 2 nd Nurse & Midwife Medicinal Product Prescribing Conference held in Dublin Castle on the 27 th of February. This was a great success. The newly launched publications and the presentations given on the day are available on the website: Congratulations to Ms. Susan Moore, Clinical Nurse Specialist Stoma Care, Sligo Regional Hospital on winning the Poster Competition Prize sponsored by Microsoft Ireland. I am pleased to inform you of the establishment of the Strategic Advisory Group on Hospital Groups. This Strategic Advisory Group will have a key role in providing objective advice and expertise to the Department of Health and the Minister, on the implementation of Hospital Groups. I have been appointed to join this group and this appointment will be for three years. The first meeting took place on the 7 th of February, The inaugural meeting for the National Workload and Workforce Planning review group has taken place. The project will utilise the Midwifery Workforce Planning tool Birthrate Plus (BR+) to review staffing and skill mix levels in a selection of sites across the country. It is envisaged that this may be used to benchmark the staffing levels in all Maternity Units nationally in the future. I will co-chair this group with Ms. Sheila Sugrue, National Lead Midwife. The Office of the Nursing and Midwifery Services Director is commencing a national project to shape the future role of the Registered Nurse -Intellectual Disability (ID). This project will take into account the significant changes in the provision of services for people with ID, which have occurred over the last number of years. January marked the launch of the Post Graduate Certificate in Nursing & Midwifery Programme for graduate staff nurses and midwives. This is the first time in Ireland that new graduates are offered direct access to this high level qualification at such an early stage in their career. In April, we look forward to Midwives Week hosted by NMBI and I would like to take this early opportunity to wish everyone a very happy Easter Holiday. 2 P A G E 2 Corporate ONMSD News National Clinical Programme for Palliative Care Developments and Initiatives Palliative Care Competency Framework A competency framework has been developed which is both core and discipline specific and appropriate to all levels of palliative care specialisation. The framework is intended to enhance knowledge, understanding and skills and will assist with practice needs assessment and evaluation and will inform curricula and future education programmes. Nursing, midwifery and health care assistants are represented within the document with discipline specific indicators. A launch is planned for Quarter 2 (Q2) in A final draft is available on request. Palliative Care Needs Assessment Guidance A guidance document has been developed to assist with the comprehensive assessment of palliative care needs for persons with a life limiting diagnosis, in all settings. An education programme is currently being developed to support implementation of the guidance document with an on-line component. The document will be available by Q2 in 2014, on our website: Rapid Discharge Planning Pathway Following extensive consultation, a rapid discharge planning document has been developed to support the smooth and seamless transition of care from hospital to community for persons who have expressed a wish to die at home. The document details the pathway, an action checklist, an algorithm, a template letter, frequently asked questions (FAQ) and a PowerPoint presentation. The document and presentation are available on the Palliative Care Programme website at: For further information, contact: Ms. Lorna Peelo Kilroe, Nursing Lead, National Clinical Programme for Palliative Care, ONMSD by lorna.peelo@hse.ie or by Phone: ONMSD National Guidance on Venepuncture and Peripheral Intravenous Cannulation The Office of the Nursing and Midwifery Services Director (ONMSD) is leading on the development of national guidance to support all healthcare staff who undertake venepuncture and peripheral intravenous cannulation. A national multi-disciplinary working group has been established and is chaired by Mr. Patrick Glackin, Interim Area Director of NMPD, HSE West. The purpose of this guidance is twofold: 1. To provide governance to include all relevant competent practitioners within the multidisciplinary team to undertake venepuncture and/or peripheral intravenous cannulation. 2. To provide guidance to healthcare staff on the management of venous access for all patient groups, including those with difficult venous access. The guidance will complement existing national and local guidelines in respect of venepuncture and peripheral intravenous cannulation. 3 P A G E 3 Corporate ONMSD News Nurse and Midwife Medicinal Product Prescribing Update The second nursing and midwifery medicinal product prescribing conference was hosted by the ONMSD on Thursday, the 27 th of February. The conference held in Dublin Castle was widely acknowledged by the 280 delegates as a great success. The ministerial address was given by Mr. Alex White, TD, Minister of State for Primary Care. The conference, opened by Dr. Michael Shannon, Nursing and Midwifery Services Director, who congratulated all nurse and midwife prescribers on their new role and achievement, saying The prescribing initiative is first and foremost about patients and about how we as nurses and midwives provide care, I am pleased to tell you that in the seven years since the initiative began; nurses and midwives have written 132,000 prescriptions. Two international speakers shared their experiences in the United Kingdom and Finland: Dr. Karen Stenner, Lecturer/Researcher on Nurse Prescribing, University of Surrey and Ms. Merja Merasto, President, Finnish Nursing Association. Ms. MacGabhann also launched three new publications: 1. Nurse and Midwife Prescribing Data Collection System Review 2. Clinical Nurse/Midwife Specialist Registered Nurse Prescriber Survey 3. Nurse and Midwife Medicinal Product Prescribing Toolkit Clare MacGabhann, Interim Director of Nursing and Midwifery, provided an update on the initiative. A total of 1,057 candidates have been funded by the ONMSD over seven years, to undertake the education programmes. Currently, there are 668 nurses and midwives registered as RNPs with the Nursing and Midwifery Board of Ireland (NMBI). Susan s poster was titled Clinical Audit on Medication Prescribers Knowledge of the Impact of Ileostomy Formation on the Absorption of Medication. Organising Committee of Conference: Clare MacGabhann, Annette Cuddy, Rose Lorenz and Sandra Kennedy Mr. Colm O Gorman, the guest speaker, delivered a very interesting presentation focusing on health care and equity of access for all with a national strategic focus. The closing remarks were given by Dr. Ambrose McLoughlin, Secretary General, Department of Health, who commended the success of this initiative and remarked on the very positive impact of nurse and midwife medicinal product prescribing. 4 P A G E 4 Corporate ONMSD News Productive Operating Theatre (TPOT ) Learning Sessions The Productive Operating Theatre (TPOT ) is a national quality improvement initiative and work stream of the national anaesthesia and surgery programme. The national team, Martha Ni Chuanaigh and Grace Reidy, with the support of Lorraine Murphy, have developed a learning session package to support TPOT teams measure for improvement. Measurement for Improvement is critical to understanding variation in processes and the impact of changes. The learning session package comprises of two full day workshops. Two progress support teleconferences are facilitated between workshops. The learning session objectives are organic in so far as the needs of the participants are the foremost determinants of focus and emphasis. Workshop one was hosted at Sligo Regional Hospital (SRH) on the 4th February TPOT multidisciplinary teams from Letterkenny General Hospital, Mayo General Hospital, Portiuncula Hospital, Sligo Regional Hospital and University Hospital Galway participated at the workshop. Participants for TPOT Workshop at SRH Reducing the delay in actual theatre session start time was the focus of practical work as all participating TPOT teams requested support on this topic. Teams worked together to problem solve, to develop aim statements, measurement plans, operational definitions and to plot and interpret measurements. Participating teams returned presentations with their own crafted problem and aim statements, operational definitions and measurement plans to the TPOT national team. This work was then critically analysed in a safe forum where learning and experiences were shared amongst participating teams. Realistic expectations of work to be achieved were agreed before convening for workshop two to be hosted by Mayo General Hospital on 2nd of April Sharing is an important element of learning and the simplicity of measuring for improvement is the crux underpinning this learning session package. It is the TPOT national team s intention to facilitate Measurement for Improvement Learning Sessions in all HSE groups. 5 P A G E 5 Midwives Week 2014 Corporate ONMSD News Midwives Week April 28th-May 2nd 2014 The Nursing and Midwifery Board (NMBI) will be celebrating the recognition of midwifery as a separate profession by hosting A Midwives Week from the 28th of April to the 2nd of May The theme for the week is "Promoting Safe Care for Mother and Baby". The purpose of the celebration is to heighten awareness amongst the public of the importance of midwives within the healthcare system. The week will provide the opportunity to inform and educate members of the public about important midwifery issues and to promote the critical role of the registered midwife in the healthcare system. Midwives are making improvements and providing vital services in hospitals, clinics, communities, universities and other settings across Ireland. From care of mother and baby in the community to the hospital, to research institutions, to determining national health policy, the midwifery profession is meeting the health care needs of Irish mothers and their babies. NMBI have the following plans to celebrate Midwives week: 1. Midwifery Students Debate The Motion is Ireland is a breastfeeding friendly Country Date: Monday 28th of April. For further information, contact: Catherine Rooney, NMBI by crooney@nmbi.ie or by Phone: Post Graduate Certificate in Nursing and Midwifery Programme for Graduate Staff Nurses and Midwives The National Post Graduate Certificate in Nursing & Midwifery Programme for graduate staff nurses and midwives was launched in January This programme is offered at level 9 on the NFQ, accredited by NUI, RCSI and NMBI with 30 ECTS credits awarded on successful completion. This is the first time in Ireland that new graduates are offered direct access to this high level qualification at such an early stage in their career. This level 9 programme will open doors for future education and career development giving graduate staff nurses and midwives an advantage, compared to other graduates worldwide. The programme is being delivered over two years in three modules, using blended learning which is a mixture of face to face and online lectures. Critically for the first time in Ireland, this programme offers graduates a unique opportunity to consolidate their undergraduate learning experience in a structured and supportive environment, while building on their competencies and providing the foundation for lifelong learning for the next stage of their career. 6 Focus on HSE West New Registered Advanced Nurse Practitioner Post in the North West P A G E 6 The North West Regional Rheumatology Service, Our Lady s Hospital, Co. Leitrim, have been working over the last few years to develop and establish a Registered Advanced Nurse Practitioner (RANP) post in Rheumatology. The Director of Nursing, Assistant Director of Nursing, Consultant Rheumatologists, NMPD North West Project Officer and the wider multidisciplinary team and key stakeholders have been involved. The Nursing Midwifery Board of Ireland (NMBI) carried out a formal assessment of the proposed role in January This was an all day meeting, attended by multi-disciplinary representatives from the North West and further afield. The post has been successfully accredited and included in the formal register of RANP posts held by NMBI. The aim of the RANP Rheumatology role is primarily to work with patients who are newly diagnosed with rheumatoid arthritis. The RANP s caseload consists of the clinical management of this specific group of patients, at an advanced nursing level, for a maximum period of one year from date of referral by the Consultant Rheumatologist. This is an important nursing post for patient care, their families and ultimately their quality of life. It is important to note that an average of one patient is diagnosed with Rheumatoid Arthritis per week. The development of this new nursing role in the North West is a great achievement for the nursing profession and will impact positively on service delivery. The concept of the role is linked directly to the objectives of the National Clinical Programme for Rheumatology. The UK Lean Healthcare Academy Awards Productive Series Sligo Regional Hospital (SRH) has won the 2014 United Kingdom Lean Healthcare Academy Award in the Productive Series International Section. The conference and awards ceremony was held in Leeds in February Their entry, 'Room to Care, a Productive Ward Project' achieved first place. Using Productive Ward (PW) methodologies, staff in SRH redesigned the admission process for patients. They secured a dedicated room, that incorporated all the equipment and documentation required to enable a streamlined approach to the admission process. HSE Staff at the UK Lean Healthcare Academy Awards 2014 Congratulations to the Midland Regional Hospital, Tullamore, Productive Ward Team who were also finalists in the international section. This is a great achievement for both Sligo Regional Hospital and the Midland Regional Hospital and a tribute to all of the team members for their work and commitment to promoting safe and quality care delivery to patients. Over one hundred entries went through the judging process, resulting in twenty four finalists, who presented their projects on the awards day. It is a tremendous achievement to have HSE Productive Ward sites as Finalist/Award winners in this prestigious awards ceremony for a 2 nd consecutive year. Other news on the Productive Series in HSE West A Productive Ward - Train the Trainer facilitation programme and Productive Ward module implementation training (MIT) for staff from HSE West and Mid West was completed in January New Productive Ward sites have also completed MIT, these include: St John's Surgical Ward and the Antenatal Maternity Ward, both in Portiuncula Hospital, Ballinasloe and the Medical Assessment Unit and Outpatients Department in Roscommon Hospital. The UL Hospitals have completed the Productive Ward readiness assessment in order to commence PW in the Postnatal wards in the Maternity Hospital, Limerick and the Trauma and Medical Wards in the UL Hospital, Limerick. The UL Hospitals are now in the process of identifying a local facilitator to progress the Productive Ward across the hospitals group. 7 P A G E 7 Focus on HSE South Health Care Assistants Graduate with Health Service Skills Award Mr. James O Shea, Director of the RCNME, HSE South acted as Master of Ceremonies at the latest graduation of the Health Service Skills Programme. The main theme from his address was the encouragement to continue studying throughout life and further enhancing and developing their vital role as health care assistants (HCA). Mr. Mark White, Director, HSE South NMPD, placed special emphasis on three key aspects to enhance the development of their role: 1. remain compassionate 2. continue their journey of life long learning 3. remain visible within the healthcare multidisciplinary team Ms. Claire Tully, Director of Nursing & Midwifery, Waterford Regional Hospital, highlighted the major changes in the health service and detailed opportunities and challenges which have presented for health care staff and encouraged the participants to remain vigilant in supporting person centred care. This cohort represents the latest number of graduates from a programme designed for HCAs and support-staff, which has evolved since In 2013, the national group of CNMEs were charged with the process of applying to FETAC/QQI for validation of the Health Service Skills programme. Metrics provides a method of measuring nursing care by using indicators to measure nurse delivered outcomes and patient experiences. Ms. Mandie Sunderland, Chief Nurse of the Pennine Acute Hospitals, NHS Trust gave an overview of establishing and implementing metrics into various care settings and presented evidence on the successful introduction of metrics to improve standards of care in a UK project hospital site. Ms. Mandie Sunderland with presenters at Information Session, Cork Ms. Eithne Cusack, NMPD Director, Dublin North highlighted the collaborative aspect of the development and implementation phases of metrics in Dublin North, Dublin North East and North West Regions. Ms. Anne Gallen, NMPD Director North West presented finding from the HSE North West Nursing and Midwifery Metrics Evaluation Research Report (2013). Findings indicate an increased transparency in the performance of individual wards, units and services. It is hoped this will encourage individuals and teams to regain control of the quality of nursing and midwifery care. A question and answer session with Margaret Nadin, Project Officer, NMPD Dublin NE, Mary Owens, DON and Fiona Willis, A/Clinical Development Co-ordinator, Mallow General Hospital enabled a series of interesting questions to be contextualised and explored. NMPD Cork/Kerry have sought expressions of interest from services, detailing their service requirements and staff commitment to expedite the implementation of metrics on a phased basis in the coming months in HSE South. 8 P A G E 8 Focus on HSE Dublin Mid Leinster The DECIDER Programme at the Triogue Community Mental Health Centre, Portlaoise Laoise/Offaly Mental Health Services The DECIDER Programme was introduced as a therapeutic programme in 2013, to the Triogue Community Mental Health Centre, Portlaoise. It is based on a combined Cognitive and Dialectic Behavioural Therapy approach to teaching life skills. It has been shown to be particularly effective in working with clients with impulsive behaviours and borderline personality disorder. The group takes place once per week for one and a half hours and runs for twelve weeks. DECIDER is designed and delivered visually, no note taking, no reading the skills taught are displayed as pictures, drama and music. The rationale behind each picture and the life skill it represents is explained by the facilitator. The lifejacket metaphor symbolises our personal ability to inflate our own survival kit in life. The programme is future focused, delivered in a very interactive and fun manner, utilising life situations and exploring options to deal with these situations. DECIDER participants will remember the skill as it is presented visually, humorously and simply to allow association of the picture presented and the life skill which they need to remember in times of distress and impulsivity, helping to lead a more skilful life, less impulsive life. A group of three staff from the Portlaoise Day Hospital service attended training which was funded through Nursing and Midwifery Planning Development (NMPD) Tullamore, HSE Dublin Mid Leinster. Staff developed an understanding of the programme, learned the evidence base supporting it, the benefits for the client group identified and how to make an informed decision regarding it s suitability as part of the overall programme at the Day Hospital. DECIDER is a cost and time effective intervention. It reduces the impact of impulsive behaviour on resources. The DECIDER programme has been successfully integrated into the Day Hospital schedule of treatments and interventions available to clients, with a wide presentation of mental heath problems. It is highly transferable to anxiety management groups with a selection from a menu of 32 skills available as part of the programme. The STOPP sign is an anagram used for helping with Distress Tolerance, one of the elements of the programme. S- Stop T- Take a breath O- Observe how you feel P- Pull back P- Practice what works The DECIDER programme and skills are also useful and aids problem solving focus and increased personal autonomy in one to one counselling sessions. Initial 2013 courses have been audited and results have shown evidence of a reduction in symptoms of global distress, anxiety and depression. There also has been marked increase in client attendance for the whole of the course and in completing the DECIDER. This is significant as poor attendance and non completion are a common feature in most mental health related courses. 9 P A G E 9 Focus on HSE Dublin North East Nursing and Midwifery REACH: Research Excellence Across Clinical Academic Healthcare Project In Dublin North, the NMPD have commenced a nursing & midwifery research programme to build clinical academic research capacity within clinical nursing and midwifery services. This initiative known as the Nursing and Midwifery REACH Project proposes to address the recommendations related to research skill development made from the Scape Study (2010) that evaluated Clinical Nurse and Midwife Specialist roles and Advanced Nurse and Midwife Practitioner roles in Ireland. The research programme is a partnership initiative between the HSE and the School of Nursing and Human Sciences, Dublin City University. A researcher has been appointed to the NMPD Dublin North to develop research capacity of nurses and midwives in healthcare services. The aim of the programme is to lead and facilitate relevant quality research that informs and enhances patient outcomes, practice development and healthcare policy. The research programme will enhance and develop the research role, profile, skills, research experience and output of clinical nursing and midwifery staff by supporting nurses and midwives in research progression. Support is being provided on specific research skills such as research proposal development, study design, data collection and data analysis, sourcing funding, publication of results, and translation of research to practice. This support is being facilitated through one to one and collective research meetings, workshops, presentations and seminars. As part of the Nursing and Midwifery REACH Project, nurses and midwives are invited to participate in and showcase their research outcomes at the Nursing and Midwifery Clinical Research Conference on the 12th of June, 2014 in the School of Nursing and Human Sciences, DCU. For further information, contact: Linda Nugent, NMPD, Dublin North by linda.nugent@hse.ie or by Phone: Leadership and Management Educational Programme For Clinical Nurse/Midwife Managers in Dublin North East The primary role of the Clinical Nurse and Midwife Manager (CNM/CMM) is one of clinical and professional leadership within the nursing and midwifery team. The Leadership and Management Educational Programme for CNM/CMM 2 in Dublin North East has provided a great learning and sharing experience for the twenty four participants from across all disciplines of nursing and midwifery in the region. This geographical spread and discipline mix helped to generate a great deal of discussion about the challenges faced in the different service areas. One participant described it as being great to be able to Learn about other staff and their issues and to realise that no matter what discipline of nursing and midwifery you are in that everyone faces challenges. This realisation helped to deepen the participants awareness of the impact of the role of nurses and midwives on the healthcare experiences of our patients and their families. The programme was designed by Karen Maher from the Health Management Institute (HMI) and consists of four main modules: People Management, Building Resiliance, Employment Law and Attendance Management. Each participant on the programme was given the opportunity to nominate a mentor to work with them through the programme. This mentoring process provides great support for the manager even after the programme has been concluded. Participants on this programme obtain a People Management, Level 6 QQI academic award. The programme is accessed utilising an e-learning suite and participants are assigned a group moodle account to support them in the programme. One of the key components of the programme is to inspire in each of the nurse/midwife managers, the ability to have a strong belief in themselves and their capabilities. Front line decisions are critical and impact on the quality of care a patient receives. Participants on the Programme for CNM/CMM 2s It is essential that all CNM/CMM 2s are resilient, strong and able to put the needs of their patients and staff first. 10 P A G E 1 0 National Leadership & Innovation Centre for Nursing & Midwifery National Leadership Masterclasses Seventy-five Directors and Assistant Directors of Nursing & Midwifery attended a series of Leadership Masterclasses as part of their ongoing leadership development programme with the Institute of Leadership RCSI. The programme, commissioned by Dr. Michael Shannon, Director, Office of the Nursing and Midwifery Services, HSE, is facilitated by the National Leadership and Innovation Centre for Nursing and Midwifery (NLIC). The aim of this Masterclass series was to enable participants to network at a strategic level, with key decision makers in the healthcare system. Guest speakers described the strategic context in the Irish and UK health systems and how nurse and midwife leaders can respond effectively to challenges both professionally and personally. (L-R) Ms. Tina Joyce, Ms Mary MacMahon, Dr. Michael Shannon, Mr. Tony O'Brien, Ms. Cora Lunn, Ms. Theresa Keane, Mr. Gearoid Hardy. Mr. Tony O'Brien, Director General of the Health Service (HSE), detailed his vision for the HSE, the 2014 Service Plan, reform and finance and what leaders in the system must be prepared for. He posed a number of challenges for the Directors including patient safety and leading integration of services and budgets. HSELanD is an online resource designed to support the professional development of staff working in health services in Ireland. The service is available to staff in the Health Service Executive, voluntary and private health sector. Since its launch in 2007, 49% of the 90,000 users registered are from nursing and midwifery services. In 2013, over 14,000 nurses and midwives registered with HSELanD, equating to a 47% increase from previous years. While activity has increased across the hubs, the Nursing and Midwifery Practice Development hub is one of the busiest hubs with over 10,000 registered users, which is 14% of all HSELanD users. Responding to these challenges, Dr. Michael Shannon discussed the key issues affecting nursing and midwifery policy at a national level with particular emphasis on workforce planning. Ms. Avilene Casey, Liaison Officer, Special Delivery Unit, DoH and President of IADNAM, discussed health policy in Ireland and gave examples of strategies that Directors can use to successfully influence policy at local and national level. Dr. Peter Carter, CEO, Royal College of Nursing, detailed the current issues facing Directors of Nursing in the UK, especially in light of findings from the Berwick and Francis Reports. Ms. Emily Logan, Ombudsman for Children, took us through her personal journey from Children s Nurse to Ombudsman for Children. She explored transitions, the importance of personal learning and transferable skills. In a thought provoking and humorous presentation, Senator Marie Louise O Donnell discussed how to influence and communicate ideas effectively through preparation and distillation. Mr. Michael Comyn, Founder of the Fearless Organisation, described practical ways in which leaders can build and enhance their resilience. Projecting a positive personal and corporate image was the subject of Ms. Ruth Murphy s session which included the effective use of personality and style. This master class series was very positively evaluated by participants.
