Application Checklist Complete this checklist and bring this page with you at the time you apply for a permit. Failure to submit any of the following may delay the application and permit approval process. The purpose of this checklist is to provide assistance to permit applications but does not address all the requirements in the Ontario Building Code and other applicable law. PROJECT ADDRESS: Has the proposed construction been through a preliminary review or approval with Planning? 2 Notes to table on required documents: 1. The letter of authorization form is required only if the person signing the application is NOT the building/property owner. 2. Separate roof truss and floor joist layouts are required by manufacturer if not proposing conventional framing. 3. A structural engineer is required where proposed work does not fall under the design limitations nor meets the residential tables of the Ontario Building Code. Note: Architects and/or Engineers are typically required for larger multi-residential projects. Have these professionals complete a Commitment to General Reviews by Architect & Engineers form. Personal Information: Personal information contained in this permit application and schedules is collected under the authority of subsection 8(1.1) of the Building Code Act, 1992, and will be used in the administration and enforcement of the Building Code Act, Questions about the collection of personal information may be addressed to: a) the Chief Building Official of the municipality or upper-tier municipality to which this application is being made, or, b) the inspector having the powers and duties of a Chief Building Official in relation to sewage systems or plumbing for an upper-tier municipality, Board of Health or Conservation Authority to whom this application is made, or, c) Director, Building and Development Branch, Ministry of Municipal Affairs and Housing 777 Bay Street, 2 nd Floor, Toronto, M5G 2E5 (416) Forms: 1) For the purpose of the Designer Form, individual means the person referred to in Clause (1) of Division C, Article of Division C, and all other persons who are exempt from qualification under Subsections and of Division C. 2) Schedule 1 is not required to be completed by a holder of a licence, temporary licence, or a certificate of authorization, issued by the Ontario Association of Architects. Schedule 1 is also not required to be completed by a holder of a licence to practise, a limited licence to practise, or a certificate of authorization, issued by the Association of Professional Engineers of Ontario. 3 Drawings Two copies of each of the following drawings are required for a complete application: Site Grading and Services Drawing Detail Provide a drawing which shows all existing services and details existing grading which also shows all proposed alterations. Site Plan based on a Legal Survey Provide a drawing based on a legal survey which shows property lines, setbacks from property lines to existing and proposed building(s), distances between buildings and parking details, including all the dimensions. Plans Foundation plans, floor plans, building elevations, building sections, wall sections, mechanical ventilation design and specifications. Provide enough information to demonstrate compliance with the Ontario Building Code. Roof Truss Truss drawings, engineered by a truss manufacturing company (if not showing conventional framing on plans). Floor Design Floor joist layout drawing, engineered by a joist manufacturing company (if not showing conventional framing on plans). Approval Process Upon receipt of a complete application, building staff will review the proposed work and within 10 business days (for house category) or within 15 business days (for small building category) advise that: The application is approved and that the permit is available to pick up at reception upon payment of the building permit fee; or The application proposes work which does not comply (detailed reasons will be provided to the applicant or the designer). Other Approvals: The following may be required before a building permit can be issued: Mississippi Valley Conservation Authority approval - if near the river Health Unit approval for private services only Planning approval must conform to the Development Permit By-Law Geotechnical Report by Soils Engineer ** Materials should not be ordered and work shall not begin until a building permit has been issued **

4 Inspection Guidelines The following will help you complete your inspections successfully. Site Copy Drawings Once a permit is issued you will receive a building permit and your site copy drawings & documents. All site copies must be on site at time of construction and at time of the inspection. Your building permit is to be posted on site in a place visible to the public. Booking Inspections The permit will indicate which inspections will be required and the number to book an inspection prior to the work being covered. Please refer to the civic address, the permit number and provide a contact number when calling for an appointment. The Province requires a Municipality to provide an inspection within 48 business hours of the request. AIR BARRIER INSPECTION STAGE IS REQUIRED TO BE INSPECTED BEFORE COVERING. REFER TO THE INSTALLATION REQUIREMENTS OF YOUR AIR BARRIER PRODUCT AND THE OBC. Occupancy Inspection The following are required prior to receiving an occupancy permit: Hydro Final Certificate of Inspection (Electrical Safety Authority ESA-SAFE) All professional certificates (if any) of on-site review for soils Structural Review Letter for on-site installation of Professional Engineer s design (where required) Mechanical Ventilation Design Summary, Heat Loss Calculation & Mechanical Layout Payment of Development Fees. Copy of any certifications required by Part 12 Energy Resource Conservation Confirmation of insulation in attic space, walls, above grade and basement walls. Note: No deposit will be refunded if the developer allows the building to be occupied prior to correcting identified deficiencies and receiving an occupancy permit. Final Inspection The following are required prior to closing a permit file and releasing the building permit deposit: Copy of as built survey showing foundation location and setbacks (if applicable) Confirmation that altered grading complies with approved grading plan (by Engineer in subdivision) Note: No deposit will be refunded if a final inspection report has not been successfully completed within one year from the date of permit issuance. Responsibilities The owner or contractor is responsible for booking inspections and for providing revised drawings (where changes or modifications are made to the designs already submitted). Additional permit fees may apply for revisions to approved drawings. 6 Elevation Worksheet FOR INFILL LOTS ONLY Civic Address - The following worksheet may be used to help set the elevation of your slab and finished grade. Setting the correct slab elevation will ensure the building services will have proper drainage to the Town s service at street level (service elevations may be obtained from the Public Works Department). Setting the correct finished grade will ensure surface drainage is directed away from the building and not adversely affecting neighbouring properties. The elevations calculated on this form may be transferred to a proper site plan.

Designers (names of designers who are responsible for the building code design and whose plans accompany the permit application) Architectural Mechanical Form authorized by OHBA, OBOA, LMCBO, February 6, 2012 ***************************************************************************************************************** GUIDE TO THE ENERGY EFFICIENCY DESIGN SUMMARY FORM The Energy Efficiency Design Summary form summarizes the compliance path used by a house designer to comply with energy efficiency requirements of the Ontario Building Code. This form is completed by the person responsible for the energy efficiency design of the project, and must be submitted with the building permt application. The information on this form MUST reflect the drawings and specificiations being submitted, or the building permit will be refused. Refer to Supplementary Stanard SB-12 for details about building code compliance requirements. Further information about energy efficiency requirements for new buildings is available from the Provincial Building Code website at or the Municipal Building Department. As of January 1, 2012 a house designer must use one of four energy efficency compliance options in the building code: 1. Comply with the SB-12 Prescriptive design tables 2. Use the SB-12 Performance compliance method, and model the design against the prescriptive standards 3. Design to Energy Star standards, or 4. Evaluate the design according to EnerGuide technical procedures and achieve a rating of 80 or more.

16 COMPLETING THE FORM B. Compliance Options Indicate the compliance option being used. SB-12 Prescriptive requires that the building conforms to a package of thermal insulation, window and mechanical system efficiency requirements set out in Subsection of SB-12. Energy efficiency design modeling and testing of the building is not required under this option. SB-12 Performance refers to the alternative method of compliance set out in Subsection of SB-12. Using this approach the designer must use recognized energy simulation software (HOT2000 V9.34c1.2 or newer), and submit documents which show that the annual energy use of the building is equal to a prescriptive package. Energy Star houses must be designed to Energy Star requirements and be labelled on completion by Enerquality or other agency. The Energy Start BOP form must be submitted with the permit documents. EnerGuide80 houses are validated by NRCan authorized energy advisors and must achieve a rating of 80 or more when evaluated in accordance with EnerGuide administrative and technical procedures. C. Project Design Conditions Climatic Zone: The number of degree days for Ontario cities is contained in Supplementary Standard SB-1. Windows, Skylights and Glass Doors: If the ratio of the total gross area of windows, sidelights, skylights and glass doors to the total gross area of walls is more than 17%, higher efficiency glazing is requried. If the ratio is more than 22% the SB-12 Prescriptive option may not be used. The total area is the sum of all the structural rough openings. Some exceptions apply. Refer to of SB-12 for further details. Fuel Source and Heating Equipment Efficiency: The fuel source and efficiency of the proposed heating equipment must be specified in order to determine which SB-12 Prescriptive compliance package table applies. Other Building Conditons: These construction conditions affect SB-12 Prescriptive compliance requirements. D. Building Specifications Thermal Insulation: Indicate the RSI or R-value being proposed where they apply to the house design. Under the SB-12 Prespective option, RSI 3.52 wall insulation is permitted in certain conditions where other design elements meet higher standards. Refer to SB-12 for further details. E. Performance Design Summary This section is not required to be completed if the SB-12 Prescriptive option is being used. AIR TIGHTNESS REQUIREMENTS FOR NEW HOUSES All houses must comply with increased air barrier requirements in the building code. Notice of air barrier completion must be provided and an inspection conducted prior to it being covered. A blower door test to verify the air tightness of the house must be conducted during construction if the NRCan EnerGuide80 option is used, of if the SB-12 Performance or Energy Star options are used and an air tightness of less than Pa in the case of detached houses, or Pa in the case of attached houses is necessary to meet the required energy efficiency standard. ENERGY EFFICIENCY LABELING FOR NEW HOUSES Energy Star and EnerGuide issue lables for new homes constructed under their energy efficiency programs. The building code does not regulate new home labelling.

21 TOWN OF CARLETON PLACE URBAN FORREST COMMITTEE GUIDELINES & STANDARDS FOR TREE PLANTING AND CONSERVATION PLANS The Town shall require Conservation Plans and Tree Planting Plans for all development including residential, commercial, and industrial uses. Conservation Plan: The conservation plan will have a preliminary assessment by a qualified professional (certified arborist, registered professional forester or other qualified professional), which will determine stands of trees or individual trees on the property which warrant protection. This plan should consider such matters as: The existing health of the tree, grouping of trees or woodlot, hackberry and the quality of such Its degree of sensitivity to grade changes, drainage disruption, changes in water table or any other factors, which may affect the trees Measures that can be taken to protect the trees (tree wells) If trees cannot be protected, consider the reasons why Opportunities for tree planting to mitigate loss of tree or forest cover. The conservation plan will identify how these trees will be protected both above and below ground, as it is important to protect the root systems from soil compaction. The following measures will be undertaken to protect these trees: 1. The identified tree or trees to be protected will be fenced off, a minimum, to the drip line (furthest point of extension of branches) to protect the roots from soil compaction. 2. Above ground utilities shall avoid, where possible, the crowns of the trees. 3. Below ground utilities shall avoid where possible damaging the root system of trees. If utilities are to be placed below ground they are to be placed directly under the tree so not to damage the fine root hairs of the extended root system. 4. Tree roots that will be damaged must be cut cleanly to avoid ragged edges so they will heal properly. If exposed, they must be moistened immediately and covered with moist material. 5. No equipment, trucks or storage of supplies shall be inside the fenced area. 6. No grading shall take place around the protected tree or trees. In short, the professional should be asking these questions: 1. Are there trees that can be protected due to size, rareness or are they a healthy stand that would add to the community. 2. If trees are going to be protected how will this be done during construction and after the project is complete. 3. If trees cannot be protected why not. 4. If trees cannot be protected what is the mitigating measure going to be for loss of trees, (i.e. enhanced tree-planting program).

False Spira lovely creamy-white plumes of flowers, grows quickly, will grow anywhere good for holding soils on banks. Spireas- Bridal Wreath (taller 5ft, white flowers), Goldflame, not Goldmound (3 ft. pink flowers, very attractive leaves all year) Anthony Waterer (3.5 ft. dark leaves, lovely deep pink flowers) Potentillas- wide variety, 3 feet, white or yellow flowers, very hardy Purple Leaf Sandcherry- can get tall, responds to pruning, but will grow into a nice shrubby tree if not pruned, maroon leaves. Nanking Cherry- will get quite large- up to 7 ft. - but easily pruned, round growth habit, white blooms, red berries. Burning Bush 4-5 ft., red fall colour, branches break in very heavy snow. Flowering Dogwood shiny red berries through fall and early winter. Crimson autumn colour. Tree Planting Plan for Developers: The planting plan will identify where additional trees are to be planted, which species and sizes of trees to be planted and how these trees will be planted and maintained. The planting plan will identify: 1. Where trees will be planted: - The site plan must identify where trees will be planted. - At least one tree shall be planted for each residential lot developed unless a large number of trees have been removed for the development then an enhanced tree planting program will be undertaken. - Industrial and commercial development site plans shall incorporate multiple trees. - Prior to planting the developer must identify the location of underground utilities; present, planned and potential future locations. 2. Species and size of trees to be planted: - Trees will be from seed from plant hardiness zone 4b, 4a or 5a or seed zones 35 and The developer will plant a 60mm (2.5 ins) caliper deciduous tree or a conifer tree minimum height 2.0 m. - To avoid monocultures at least 4 deciduous and 1 conifer species will be selected from the list (Table 1) and approved by Town staff.

Tree planting: The International Society of Arborists, the Canadian Nursery Trades Association or Landscape Ontario standards of planting and maintenance are to be followed: - Excavate to a depth 200mm deeper than the height of the root ball, with a width 750 mm greater than the root ball. - Loosen the planting hole to a depth of 200mm - Loosen burlap and cut away minimum at least 50% of the burlap without disturbing the root ball (if in a wire basket cut away as much of the wire basket while the tree is in the hole) - Place plant material to a depth equal to the depth they were originally growing in the nursery. - Tamp soil around the root system in layers of 150 mm to eliminate air pockets. When 2/3 of the planting soil has been placed, fill the hole with water. After the water has penetrated into the soil, complete backfilling. - Build a 100mm deep saucer around the outer edge of the hole to assist with watering. - The hardwood trees will be staked following International Society of Arborist standards. - The trees will be mulched to a depth of 10 mm filling the saucer leaving 50 mm free around the trunk to avoid trunk rot. - The trees will be watered one week after planting and every 2 weeks thereafter, pending weather conditions, until the area developed is no longer the responsibility of the developer.
