1 Description of the EPSC EXCEL PSI Reporting Form Georg Suter, SWISSI Process Safety GmbH, Basel, This EXCEL Reporting sheet for Process Safety incidents has been prepared by SWISSI on behalf of EPSC and Clariant. It is not the intention of EPSC or the PSI Working Group to declare this as a mandatory standard tool at all. The tool should rather support those companies which are looking for a flexible and low-cost instrument to fulfil their reporting duties. It was a deliberate decision to keep the calculations as simple as possible and as open as possible, such that adaptation to specific company need can be made by anybody who has some experience with EXCEL programming. Although much more elegant programming would be possible by using macros, this option has not been used because often EXCEL sheets with macros are not allowed in company networks. The descrpition below refers to the full version. Much simpler sheets have been created with limited functionality, e.g. finding the reporting limit for a single substance or for a mixture. Thees sheets are simple subsets of the full version. Currently the database contained in the sheet is a combination of the substance list in the UN Orange Book and the official substance GHS classification list of the EU. There is an offer from ECOMED to deliver a database of substances, which would allow direct links from CAS numbers to GHS and UNDG classes and thus to threshold limits. The EPSC PSI Working Group has decided to leave it to the individual members whether they want to buy this or any other database. SWISSI would provide support to integrate any other database into the tool. (in complex cases at cost). You will receive two EXCEL Workbooks with an example filled in. You may play around and enter own examples. The open versions gives on the sheets FORM and Subst 1 more insight in the used algorithms.

2 Structure The Workbook has a hierarchical structure and consists of the following sheets: (dashed = hidden for the user, accessible for the manager of the reporting system) PARAMETERS Look-Up Tables for predefined text and phrases FORM Overview, Summary Combining LoPC and Consequences Applying the mixing rule (summation of %) HELP To see the procedure for Determination of threshold quantity Subset for Mixtures SUMMARY To get all entered values in a table form Subst 1 5 Release details Determination of threshold quantity Subset for one Substance CAS Finder To find a CAS number based on a substance name LIST Look-Up Tables for threshold quantities based on classification

3 Parameters The following lists can be predefined individual for each company: Sites Countries Regions Business Units Lists The following lists with lookup tables are used. They allow to derive the threshold limits based on classification data.
