(1.3) What is it trying to achieve (intended aims/outcomes)? The purpose of the policy is to enable nurses to discharge patients who meet the clinical criteria for nurse facilitated discharge, thus ensuring the highest quality, most timely and safest possible discharge for the patient. (1.4) How will the proposal be implemented? This policy will be disseminated through directorate structures. (1.5) Are there any Section 75 categories (see list in 2.1) which might be expected to benefit from the intended policy/proposal? 2
