1 BOOKING FORM - Individuals and families The Holy See has confirmed that the Holy Father, that Alvaro Del Portillo will be beatified on September 27, 2014 in his native city of Madrid. Faithful from all over the world are expected to participate. Plenia Travel would assist you in the organization of your trip. In order to book your journey properly, please kindly fill up this form and send it to dalvarobeatification@plenia.es.you would be contacted back with the information relevant to your request until all the process is completed to secure your bookings. HOW TO PROCEED TO COMPLETE YOUR BOOKING 1) We kindly ask you to fill this Booking Form on a computer, avoiding hand-writing. 2) Send us your Booking Form by e mail to dalvarobeatification@plenia.es 3) Once we have received your Booking Form, we will contact you and send you the pro forma Invoice indicating the total amount due, besides the deposit amount needed to secure your booking. We will send you as well all bank details you might need if you choose to pay via bank transfer. You may also pay with any credit card (Amex credit cards will have a surcharge of 3%). 4) The deposit amount to secure your booking will be $300, 00 per person and it must be paid within 3 days of the moment that you have received the pro forma invoice. 5) Once we have received your deposit amount, you will have 30 days to make the final payment of the rest of the amount due, via bank transfer or credit card. (Amex credit card will have a surcharge of 3%). 6) In case you want to book your flights through us, we will kindly ask you to scan and send us your passport before June 30 th so we can issue your tickets. The passports that you submit must be valid for at least 6 months. Plenia Travel is not responsible if any official documents are not valid for any reason. It will be your responsibility to get information regarding your passport at the appropriate embassy for your trip to Spain. a. 5 TERMS & CONDITIONS For the purposes of the foregoing General Conditions, it is understood as informative brochure the documents to which these are incorporated; ass program/offer the description of the combined travel contained in the brochure constituting the object of the Combined Travel Contract and as consumer/user/customer, the physical or juridical person buying or committed to buy the combined travel. Information about the program/offer contained in the brochure is binding for the organizer and the retailer, unless that any of the following circumstances arises: - That changes in such information have been clearly communicated in writing to the consumer and user before entering into the contract and such possibility had been object of express mention in the program/offer. - That modifications are produced afterwards, by previous agreement in writing between the contracting parties Legal frame applicable to the contract of combined travel and acceptance of General Conditions. The foregoing General Conditions are subject to what is established in the Legislative Royal Decree 1/2007 dated on November 16th, through which Merged Text of the General Law for Defense of Consumers and Users and other complementary laws is approved. The foregoing General Conditions, incorporated as signed by the contracting parties in all combined travel contracts, as well as the particular conditions agreed in the contract or included in the travel documentation provided simultaneously to the subscription of the contract, are binding for the contracting parties Organization. Organization of this combined travel has been performed by LUZZIA AGENCIA DE VACACIONES, S.L., with address at street Velázquez nº 98, Madrid (Spain). C.I.C.M.A nº Price Price of Combined Travel includes. a) Transportation, when such service is included in the program contracted, with the type of transportation, characteristics and category indicated in the

6 contract or in the documentation delivered to the consumer at the time of subscribing it. b) Lodging when such service is included in the program contracted, in the establishment and with the food regime indicated in the contract or in the documentation delivered to the consumer at the time of subscribing it, or in other similar in case of replacement requested by the consumer and accepted by the organizer. c) Rates or taxes of the hotel establishments and indirect taxes -Added Value Tax (AVT), Canaries General Indirect Tax (I.G.I.C.), etc.-, as applicable. d) Technical assistance during the travel, when this service is specifically included in the program hired. e) All other services and supplements concretely specified in the program hired or expressly indicated in the combined travel contract. f) Management expenses 3.2. Revision of prices. Prices of the combined travel has been calculated based on exchange rates, transportation rates, cost of fuel and applicable rates and taxes at the date of issuance of the program or posterior to it, in which case, had been made public in a printed manner. The aforementioned price will be able to be revised, but only to incorporate variations of the price of elements indicated (price of transportation, cost of fuel, rates and taxes and exchange rates). These modifications will be notified to the consumer, in writing or through any other means allowing to have a record of the communication performed, being able the consumer, when the modification performed is an increase in an amount larger than twenty percent of the total of the combined travel, to desist of the travel without any penalty, or as well accept the modification of the contract. The revision of increasing prices performed 20 days immediately before the date of departure of the travel will be void Special offers. When as a consequence of special offers of last hour or equivalent the hiring of a combined travel is performed at a different price of the one expressed in the catalog, services comprised in the price will only be those specified in detail in the offer, even when such offer makes reference to any of the programs described in such catalog, as long as such remission is performed for the exclusive purposes of general information of the destination Exclusions. Price of the Combined Travel does not include:

7 Visa endorsements, airport rates, and/or entry and exit rates, vaccination certificates, "extras" such as coffee, wines, liquors, mineral water, special food regimes -even in the supposition of full or half pension, unless expressly agreed in the contract otherwise-, washing and ironing of clothes, optional hotel services, and, in general, any other service not expressly included in the item "The price includes" or not specifically detailed in the program, in the contract or documentation delivered to the consumer on subscribing it Excursions or faculty visits. In the case of excursions or faculty visits not hired at the origin, it must be taken into account that these do not form part of the combined travel contract. Its publication in the program/catalog only has informative nature and the price is expressed with the indication of "estimated". Thus, at the time of hiring it at the destination, variations on their costs can be produced, altering the estimated price. Such excursions will be offered to the consumer with their specific conditions and definitive contract in an independent manner, not guaranteeing the possible performance of the same until the time of their hiring Tips. Tips are not included within the price of the combined trip. In the case of cruises, a complementary contribution which is usually, although in a mistaken manner, referred as tip is not included in the price of the travel, and which amount is in function of the duration of the trip and has as sole addressee the service personnel, regarding to which the customer is warned at the beginning of the trip that they must assume the commitment to deliver it at the end of the travel. 4. Manner of Payment. Inscriptions and reimbursements. A deposit of $ 300 per person, is payable together with a completed and signed booking form (deposit is not refundable in any case) Upon receipt of this the Company will forward our confirmation invoice. The aforementioned confirmation will be communicated to the customer as soon as possible in writing or by any means allowing having a record of the communication performed. Since that moment, the combined travel contract, as well as the general conditions to which it is adhered, is of forced compliance for both parties. If any of the services could not be confirmed, the organizer would propose an alternative which will be able to be accepted by the customer. In case that the customer rejects it, such thing will comprise the complete reimbursement of the amount provided, equal to forty percent of the total of the combined travel without any penalty.. The rest of the contract price (except for the booking amount) shall be paid no later than 30 days after the deposit amount has been transferred. If payment is not received on the due date the holiday or travel arrangements will be liable to cancellation which will lead to loss of deposit. In the supposition that by special

8 circumstances (such as demands of the suppliers of the different services hired) the payment of the rest of the price had to be performed in a time previous to the term indicated, such circumstance will be notified to the customer in writing or by any other means allowing to have a record of the communication performed, being the customer obliged to the payment as long as bonds and documentation needed for the travel are delivered at such time. If the payment of the total travel price is not performed in the conditions indicated, it will be understood that the consumer desists from the travel requested, applying to it the conditions foreseen in the following item. All reimbursements proceeding by any concept will always be formalized through the Retail Office where the inscription was performed, not making any reimbursement by services not used voluntarily by the consumer. 5. Waiving by the consumer. At any time, the user or consumer can waive the services requested or hired, having the right to the reimbursement of the amounts which it has paid, but it shall indemnify the organizer for the concepts indicated as follows, unless that the termination is caused by a force majeure: 1. In the case of loose services: The totality of the management expenses, plus the cancellation expenses, if these were incurred. 2. In the case of combined travels: a) Management expenses b) Cancellation expenses, if any. c) A penalty, consisting on: - $ 300 if the termination is produced with more than ninety days of anticipation to the date of beginning of the travel. - 35% between the forty-five and ninety days before the beginning of the travel % if the termination is produced with less than forty five days before the departure. If the customer does not arrive at the departure, they will not have the right to any reimbursement of the amount paid, except by agreement between the parties otherwise. In the case that any of the services hired and canceled is subject to special economic conditions of hiring, such as freight of airplanes, vessels or special rates,

9 cancellation expenses by termination will be established in accordance to the conditions agreed by both parties Transfer of booking. The consumer will be able to transfer its booking in the combined travel to a third person, as long as this one meets all conditions required for the aforementioned travel, having to communicate it in writing to the organizer with a minimum anticipation of 35 days to the date of beginning of the travel, unless the parties agree a shorter term in the contract. Either the person transferring its booking in the combined travel as well as the transferee will jointly respond before the organizer for the payment of the travel price as well as additional and justified expenses that could arise by such transfer Cancellation of travel by not reaching the number of persons inscribed in the minimum foreseen. In the cases that the organizer conditions, and it expressly specifies it, the feasibility of the offer of combined travel to have a minimum of participants and since such number is not reached, the cancellation of the travel is produced, the user will have the right to the reimbursement of the total price or the anticipated amounts, without they can claim any amount in concept of indemnification, as long as the organizer had notified to them in writing with a minimum of ten days of anticipation to the date foreseen for the beginning of the travel Modifications and terminations of the contract by cause attributable to the organizer or travel cancellation. The organizer commits to provide its customers the totality of the services hired contained in the program/offer which originated the combined travel contract, with the conditions and characteristics stipulated, all of that in accordance to the following extremes: a. In the supposition that, before the departure of the travel, the organizer is obliged to modify in a meaningful manner any essential element of the contract, including the price, it shall make it immediately of the knowledge of the consumer, either directly, when also acting as retailer, or as well through the respective retailer in the other cases. b. In such case, and except that the parties agree otherwise, the consumer will be able to opt between terminating the contract without any penalty or accepting a modification of the contract in which the variations made and their repercussion in the price are indicated. The consumer shall communicate the decision adopted to the retailer or, in its case, to the organizer within the three days following to the notice of the modification referred in item a). In the supposition that the consumer does not notify its decision in the terms indicated, it will be understood that it opts by the termination of the contract without any penalty

10 c. In the supposition that the consumer opts to terminate the contract, under the coverage foreseen in item b), or that the organizer cancels the combined travel before the date of departure agreed, by any reason which is not attributable to the consumer, this one will have the right, since the time in which the contract termination is produced, to the reimbursement of all amounts paid, with adherence to the same, or as well to the performance of another combined travel with equivalent or higher quality, as long as the organizer or retailer can propose it. In the supposition that the travel proposed is of a lower quality, the organizer or retailer shall reimburse to the consumer, as proceeding in function of the amounts already paid, the price difference, with adherence to the contract. In any case, the consumer and user will be able to demand the reimbursement of the amounts paid to the entrepreneur to whom it was paid, who shall reimburse them in a maximum term of 30 days staring since the notice of the consumer of its choice by the termination or since the determining circumstances of the cancellation are produced. c. There will not exist obligation to indemnify in the following suppositions: 1. When the cancellation is caused since the number of persons inscribed for the combined travel is lower to the one requested and it is communicated in writing to the consumer before the deadline established for such purpose in the contract. 2. When the cancellation of the travel, unless in the supposition of excess of booking, is due to reasons of force majeure, understanding for such those circumstances foreign to the party that invokes them, abnormal or unforeseeable, which consequences could not be avoided, in spite of having acted with due diligence Consequences of lack of provision of services. In the case that, after the departure of the travel, the organizer does not provide or proves that it cannot provide a major portion of the services foreseen in the contract, it will adopt the proper solutions to continue the travel organized, without any supplement of the price for the consumer, and, in its case, will reimburse to this one the amount of the difference between the services foreseen and those supplied. If the consumer continues the travel with the solutions given by the organizer, it will be deemed that it expressly accepts such proposals. If the solutions adopted by the organizer are unfeasible or the consumer does not accept them by reasonable motives, the first shall provide to the latter, without any supplement of price, an equivalent transportation means to the one used in the travel to return to the place of departure or to any other agreed by both, without detriment of the indemnification proceeding in its case. In case of claim, the retailer or, in its case, the organizer shall work with diligence to find proper solutions,

11 10. - Obligation by the consumer to communicate any non-compliance in the execution of the contract. The consumer is obliged to communicate any non-compliance in the execution of the contract as soon as possible in writing or in any other manner which can be recorded, to the organizer or retailer and, in its case, to the provider of the service dealt with Expiration of actions. The term of expiration of actions derived from the rights recognized in the Legislative Royal Decree 1/2007 dated on November 16th, regulating the Combined Travels will be of two years Responsibility General. The organizer and retailer of the combined travel will respond before the consumer, in function of their corresponding obligations by their respective management scope of the combined travel, of the proper compliance of the obligations derived from the contract, independently that these must be performed by themselves or other service providers, and without detriment of the right of organizers and retailers to act against such service providers. The organizer and retailer of the combined travel will respond from damages suffered by the consumer as consequence of the non-execution or defective execution of the contract. Such responsibility will end when any of the following circumstances occur: 1. That defects observed in the execution of the contract are attributable to the consumer. 2. That such defects are attributable to a third party foreign to the supply of the services foreseen in the contract and having an unforeseeable or unavoidable character. 3. That the defects mentioned are due to reasons of force majeure, understanding for such those circumstances foreign to the party that invokes them, abnormal or unforeseeable, which consequences could not be avoided, in spite of having acted with due diligence. 4. That the defects are due to an event that the retailer or, in its case, the organizer, in spite of having placed all needed diligence, could not foresee or avoid. Nevertheless, in the suppositions of exclusion of responsibility by arising any of the circumstances foreseen in numbers 2, 3 and 4, the organizer and retailer who are part of the combined travel contract will be obliged to provide the necessary assistance to the consumer who is in difficulties Limits of repair by damages.

12 As for the limit of repair by the damages resulting from the non-compliance or faulty execution of the services included in the combined travel, provisions of the International Agreements on the matter shall be followed Delimitation of combined travel services Travel by airplane. Presentation at the airport. In travels by airplane, presentation at the airport will be performed with a minimum of anticipation of three hours in international travels and two hours in national travels on the official hours of departure, and specific recommendations indicated in the travel documentation provided on subscribing the contract shall be followed in every case. It the hiring of separate services, it is recommended that the customer confirms again the departure time of the flights with forty-eight hours of anticipation Hotels General. Quality and contents of the services provided by the hotel will be determined by the official tourist category, if any, assigned by the competent organism of its country. Given the laws in force on that regard, establishing only the existence of individual and double rooms allowing that in some of the latter a third bed can be provided, it will always be deemed that the use of the third bed is made with the knowledge and consent of the persons occupying the room. This tacit estimation derives from the certain circumstance of having been previously warned, as well as the room being reflected as triple in all print-outs of bookings provided to the consumer on paying the installment, in the contract and the tickets and/or travel documentation simultaneously delivered at the signature of the same. Likewise in the cases of double rooms for use of up to four persons, with four beds, when specified as such in the offer of the program/brochure. The usual time for check-in and check-out at the hotels is in function of the first and last service to be used by the user. As a general standard and unless expressly agreed otherwise in the contract, rooms will be able to be used since 14 hours of the arrival day and shall be vacated before 12 hours of the day of departure. When the service hired does not include the permanent accompaniment of a guide and in the supposition that the user foresees its arrival to the hotel or apartment booked in dates or times different to the ones indicated, it is convenient, in order to avoid problems and misunderstandings, to communicate as soon as possible about such circumstance to the organizer or retailer, or to the hotel or apartment directly, according to each case.

13 Likewise, the user must consult the organizer, at the time of booking, the possibility to take animals, since these are not generally admitted in hotels and apartments. In the supposition of having confirmed the admittance of animals and they pretend traveling with them, such circumstance shall be indicated in the contract. Lodging service will imply that the room is available at the corresponding night, understanding as provided independently that, by circumstances proper of the combined travel, the check-in time in the same is produced later than initially expected Other Services. In flights which arrival to the destination is performed after hours, the first service of the hotel, when this one is included in the offer of the program/brochure, will be dinner. Likewise, in flights which arrival to the destination is performed after hours, the first service of the hotel will be the lodging. It will always be understood as direct air trip that one which documentary support is only one flight ticket, independently that the flight performs any technical stop Supplementary Services. When users request supplementary services (for instance, room with sea view, etc.) which cannot be definitively confirmed by the organizer, the user will be able to opt to definitively desist from the supplementary service requested or keep its request waiting that such services can be ultimately provided. In the supposition that the parties had agreed the previous payment of the price of supplementary services which finally cannot be provided, the payment made will be reimbursed by the organizer immediately at the waiving of the service by the consumer or returning from the travel, either the user had opted by the waiving in the provision of the supplementary service requested or had kept the request Special economic conditions for children. Given the diversity of the treatment applied to the children, depending on their age, the services supplier and the date of the travel, it is recommended to consult always the scope of the special conditions existing and that every time will be object of concrete and detailed information and will be gathered in the contract or in the travel documentation delivered at the time of its signature. In general, as for lodging, these will be applicable as long as the child shares its room with two adults. Regarding to lodging of minors abroad, the information provided in a timely manner will be followed for each case and which can be recorded in the contract or in the travel documentation delivered on subscribing it.

14 14. - Dates and delays. In the case that expenses by loss of the fight or delay of this one are originated, due to bad weather, bad atmospheric conditions or any other cause, ISHAV ARCTICO will not be liable for such expenses, including lodging and food expenses originated. In such case, the traveler will be able to request damages and detriments to the transportation companies implied. Delays in the beginning of the travel due to strikes of personnel of air companies, closure of airports by weather conditions, delays in the flight schedules or any other meteorological or logistics cause that the guide or the organization deem that can affect the safety of the group or any of the participants, etc.,, and/or any other cause foreign to the organization, will not be able to be reason of noncompliance of the foreseen travel schedule Passports, visa certifications and documentation. All users, without exception (including children), shall have their corresponding valid personal and family documentation, either passport or National Identification Document (D.N.I.), according to the laws of the country or countries visited. Obtaining visa certificates, passports, vaccination certificates etc. will be at the charge of the users, when these are required by the travel. In case that the grating of visa is rejected by any Authority, by particular causes of the user, or their entrance is rejected in the country by lacking of the requirements demanded, or by a defect in the documentation demanded, or not bearing the same, the organizer waives every responsibility by facts of this nature, being at the charge of the consumer any expense arising, applying in these circumstances conditions and standards established for the suppositions of voluntary waiving of services. Likewise, it is reminded to all users, and especially to those who are nationals of States member of the European Union, that they must be assured, before starting the travel, to have complied all standards and requirements applicable in matter of visa certifications in order to enter without problems to all countries to be visited. Travelers younger than 18 years old must have a permit in writing signed by their parents or custodians, foreseeing that this one can be requested by any authority Additional information to be provided to the consumer. At the time of formalizing the contract, the organizer or retailer will inform the consumer about the specific documentation needed for the travel chosen and about the possibility of faculty subscription of an insurance covering expenses of cancellation and/or an assistance insurance covering the expenses of repatriation or transportation in case of accident, disease or death. Likewise, the organizer will inform the consumer about the implicit risks and sanitary formalities for the destination and the travel hired, in compliance to the General Law of Defense of Consumers and User Validity. Validity of the program will be since January 1st, 2014 until December 31st, 2014.
