1 Use of valid ICD 9 codes, billed with appropriate and corresponding CPT codes, benefits providers by facilitating treatment authorization and claims payment. The use of valid and appropriate codes also supports the necessity of the services in cases of file review, audit or litigation. Based on ChiroCare's commitment to program integrity and excellence, only valid and appropriate ICD 9 codes will be accepted. Submission of terminated or invalid ICD 9 codes will result in processing and/or payment delays. To help simplify the coding process a list of commonly used and accepted ICD 9 codes is provided below. This list is not intended to represent all ICD 9 codes accepted by ChiroCare however it can be used as a guide in your coding process. In addition to the validity of ICD 9 codes being reported, they must support the type and level of all services being billed. For example, if a patient has been diagnosed with multiple conditions requiring treatment in several spinal and/or extremity regions the diagnosis code(s) reported on the claim form must represent each of the different body regions requiring treatment. In addition, to remain consistent with the ICD 9 CM Official Guidelines for Coding and Reporting, please list the diagnosis/condition primarily responsible for the treatment in the primary position on the claim form. ChiroCare further recommends that the secondary, tertiary and quaternary diagnosis positions on the claim form be used to report additional conditions/regions that are being treated.
