Over 25 successful surgeries in two days. Odiley Festus The 21st of July began like every ordinary day but for those in the Eyecare sector it was a day of history as it was the day the Femto Laser technology was introduced to Sub-Saharan Africa through the Restore Vision International Femto laser Lasik Centre in Lagos Nigeria. Dr. Asharaf Bassiouni and Dr. Moones Fathi Abdelrehim were guests of the Restore Vision Centre between July 21 to 1st of August Dutrring this time their visit was part of the events that led to the successful set up of the Restore Vision Centre in 22 Idowu Taylor Street, Victoria Island, Lagos Nigeria The surgeries were carried out by Dr. Moones and Restore Vision Centre Ophthalmologists from the Eye Foundation Centre who had earlier undegone training in Femto Laser technology outside the country. Their visit also had a lecture delivered by Dr. Moones Fathi Abdelrehim at the Bruce Spivery Auditorium as the Eye Foundation Hospital at 27 Isaac John Street, Ikeja G.R.A Lagos on Tuesday 29th July The topic was titled: An overview of Refractive Surgery The Femtosecond Laser Experience. The invited guests were introduced into the indices and advantages of the new femto laser technology and was well attended. Dr. Moones explained that the technology was the latest technology anywhere in the world and was guaranteed to cure eyecare challenges through surgery without the aid glasses. All attention was moved to the Restore Vision Laser Lasik Vision Centre where surgeries took place on patient s queueing up and having their surgeries performed by the combined team of Dr. Moones and The Restore Vision Nigerian Team ophthalmologists. At the end of the surgeries many patients were marveled that what was once regarded in wishful thinking meaning the possibility to see without glasses was not possible and affordable here in Nigeria. Over 25 patients had their surgeries at the Restore vision Centre and all were successful. Photo: CHIMEX STUDIOS News in Brief CONs Training. On the 8th of May, 2014, Friends of Vision (FOV) and Community Ophthalmic Nurses (CON) gathered at Eye Foundation Centre, Ijebu for meeting. Participants came from all the major divisions of Ogun State (Remo, Ijebu, Yewa and Egba) for update on their activities, challenges as well as training for the FOVs and CONs. Fellowship Training This quarter, three visiting doctors undergoing training especially in the areas of machine handling and procedures. Alfred Sommers Visit The Team of Prof. Alfred Sommer (of the Johns Hopkins Bloomberg School of Public Health) and Mr. Ganesh Babu and Dr. R. D Ravindran of the Aravind Lions Institute of Ophthalmology were our guests as they were in the Lagos and Ijebu centers of the Hospital to meet and train the fellows of the Hilton Cataract Initiative. The visit was a part of the assessment of the Hilton Cataract Initiative which was to see a large number of cataract surgeries performed by 5 selected fellows from Eye June-August 2014 EFH GroupNews 1
2 News in Brief continued Foundation team. Their visit in Nigeria was from 27th June- 1st July. They also particpated in the Eye Camps held Ogun State and our Ijebu Hospital in Ijebu-Imushin. The visit which spanned two locations namely the Isaac John Head office and the Ijebu Centre. Prof Sommer was very impressed with his first visit to Nigeria and he remarked that the professionalism and hospitality of the Eye Foundation Team members for exceeded his expectations. He promised to try and make more visits in the nearest future. HCI onsite visit and Workshop in Nairobi The Eye Foundation Centre team were in Nairobi, Kenya for an onsite visit and workshop put together by the HCI and LA- ICO team to assess work done so far by the partner hospitals for the Hilton Cataract Initiative. Presentations were given by the participants, assessments were made, challenges were raised and tackled and action plans were drawn for a more effective implemmentation of the project. Festus Odiley, Eunice Ogunkelu The Eye Foundation Team was in Abia state as part of capacity building partnership agreed by Eye Foundation Hospital Group and Abia State Eye Center. The Eye Foundation Team was in Abia State between 24th to 28th June, The three day strategic meeting was an opportunity for Dr. Adekunle Hassan and the Eye Foundation Team to synergize with the Abia State Team Led by Pof. Augustus Mbanaso, Chief Medical Director, Dr. Chima Anya, Head Ophthalmologist Abia State Eye Center and his team. The Abia state Eye center was the venue of the meeting as an inspection of the facilities and staff procedure. Mrs. Halimat Bogunjoko trained the nurses on the ethic and standards of the theatre and nurses conduct and etiquette. The optical department and other areas were addressed in a meeting with the commissioner of Health and the Chief medical director in attendance. The meeting ended with both parties agreeing to continue to work together to achieve the goal of the partnership which is to train the needed manpower to effectively provide quality eyecare service to the people of the South East. The four day meeting witnessed Eye screening and surgeries with the Eye Foundation and Abia State Team working together to have 100 cataract surgeries performed. It was a wonderful experience as many patients who had lost hope of ever seeing were surprised that after a surgery of 20 minutess they were able to see and appreciate the wonders of nature. Some broke down in tears of appreciation. The meeting was part of the series of capacity building a partnership with Eye Foundation to help Abia State build a world class Eye Facilty that would cater for the eyecare needs of the south eastern part of the country. Prior to this meeting, key staff from the newly founded Abia Eye Centre were guests of the Eye Foundation Hospital on a capacity building training. The training which commenced on the 19th of May 2014 lasted nine days officially coming to an end on the 27th of same month. The trainees which include an Ophthalmologist, Optometrist, Accountant, Matron, Admin officer, IT personnel, Ophthalmic nurse and an Opthalmic techinician were taken through all the processes required to run an eye clinic successfully by various heads of departments. There were sessions for group work, where individuals went to their various departments after each days training and meetings. Lecture materials/ power points were given to individual members of the Team, including a book titled Ophthalmic Assistants Textbook. Protocols for each unit were handed over to Dr Anya Kalu(Ophthalmologist). On the last day, the Team was addressed by Dr O. Oderinlo and Dr O. Okonkwo who encouraged them to let all they have learnt be part of them as they start up their Clinic. June-August 2014 EFH GroupNews 3
4 HCI UPDATE Understanding Femto Laser. What is Femtosecond LASIK? The Femto second laser is a technique used to perform the first step in lasik creating a corneal flap preparing the eye for the second step, where an Excimer laser is used on the cornea to correct refractive error and improve vision. How does Femto laser work? The Femto second laser uses tiny, very rapid pulses of laser to create a corneal flap. Each pulse of laser passes through the layer of the cornea and forms a microscopic bubble at a specific depth and position determined by the doctor, creating a uniform layer of bubble beneath the corneal surface. The doctor creates a corneal flap by gently separating the tissues where those bubbles were formed. The corneal flap is folded back so the doctor can perform Ezcimer Laser treatment that can also be customized to the person s own cornea. What is the difference between a corneal flap created with femto second laser and one created with microkeratone? The Microkeratome is a handheld instrument, which moves back and forth to create a cut in the cornea. A microkeratome is only capable of making a single, one dimensional cut across the cornea. 5 using the femto second lasik including: Safety: As there is no actual cutting of the corneal, but only a layer of gas bubbles, no damage occurs if the procedure has to be aborted at any stage. The procedure can be reapplied either during the same session or after few minutes. If the surgeon decides to postpone the procedure, the gas bubbles are absorbed and the patient returns to normal in few hours.whose diet are rich in these antioxidants have a much lower risk of developing AMD. Accuracy: The surgeon can determine with a range of only few microns, the exact thickness of the flap. This allows the surgeon to correct higher errors in patients with thin corneas and yet maintain a safe residual thickness of cornea. Ability to tailor the procedure to your eyes: The femto second lasik gives your doctor the ability to tailor the dimensions of the corneal flap based on whats best for your eye. Everything from the diameter of your flap to the angle of its edges can be precisely determined. This is important because everyone s eyes are shaped a little differently. Having a cornea flap thats individualized to the patient contributes to excellent prospective outcomes. In addition a corneal flap created with femto second lasik also locks back into position after the LASIK procedure is performed making it more stable especially for those who are actively involved in contact sports. No Pressure on the eye: Patient maintains fixation throughout the whole procedure with no pain or momentary blackout as with the microkeratome. He graduated from Obafemi Awolowo University Ile- Ife with a Bachelor of Medicine Bachelor of Surgery (MBChB) degree in 1993 and completed his residency training in 2002 as one of the pioneer Eye Foundation Hospital residents. He is a Fellow of the Royal College of Surgeons of Edinburg Scotland, Member of the Royal College of Ophthalmologist, UK and Fellow of the West African College of Surgeons. He was trained at the prestigious Tennents Institute of Ophthalmology, Western Infirmary-Glasgow, Scotland, under Professor C M Kirkness and has attended several International training courses spanning over Europe, the United States of America and Asia. He has over 75 publications, academic seminar and scientific papers as well as research work to his credit. As a clinical researcher his area of interest includes ocular neovascular diseases as well as management and outcomes of retinal detachments. 12 Understanding Femto Laser continued Not affected by corneal curvature or high astigmatism: This also allow the surgeon to do Lasik in patients with very steep or very flat corneas that would, otherwise have been impossible to be treated safely with standard Microkeratome Lasik. Better vision: More patients in clinical studies achieved 20/20 (6/6) vision or better with the femto second lasik than in cases with mechanical Microkeratome. Better Quality of Vision: Because of all these advantage and ability to combine a customized flap with a customized Excimer laser treatment, more patients observe better quality of vision with less glare and good night vision. How long does it takes to create a flap using the femto second lasik? The creation of the flap itself takes only about seconds per eye, including preparation time, the entire LASIK procedure takes about 10minutes for both eyes. FLEx is the similar to LASIK, in that a flap is created, lifted and reflected back. However, instead of the refractive correction done with the excimer laser, the femtosecond laser also performs a refractive lenticule together with the flap. This refractive lenticule is peeled off the corneal bed, rather than being ablated with the excimer laser. FLEx offers all the safety of Femtosecond LASIK, with a more elegant work flow and more stable results. SMILE(Small Incision Lenticule Extraction): SMILE takes FLEx even further. Instead of creating and lifting a flap, a refractive lenticule is carved with cornea, and removed through a small incision. SMILE is the top of the ReleX technique. It offers many advantages to both the patient and the doctor. It is quick extremely safe, maintains the corneal intergrity and gives predictable and stable results, with the least patient discomfort Is the femto second lasik method right for me? If you are looking for a safe, accurate and more comfortable LASIK experience, the answer is yes. But to get this procedure, you have to ask your doctor for the femto second lasik. Other Applications of Femtosecond Laser Restore Vision Centre is priveledged to be one of the few centers around the world, in which an all in one femtosecond laser Refractive Lenticule Extraction (ReLex) is performed.
