4 Feeling the Burden? Many people experience the adversity of less-than-perfect eyesight. Glasses and contact lenses correct these imperfections, but for some they introduce restrictions, limitations or inconveniences. The difficulty keeping glasses clean can be frustrating; glasses that won t stay in position can be irritating; opening your eyes in the morning to a blurred alarm clock may be annoying; fumbling for your glasses when you hear a sound in the night may be worrisome; and pleasure activities and career choices may be restricted by the need to wear glasses or contact lenses. Perhaps you play hockey or football, or like to swim, ski, water-ski or participate in other activities where wearing glasses or contacts creates annoyances you wish you could avoid. Perhaps you would like the freedom to consider occupations, such as military or law enforcement, which may impose vision standards that preclude the use of glasses or contacts. Maybe you are suffering from the frustratingly normal loss of near vision and are having trouble reading small print, numbers on cell phones or in the phonebook. Each person has his or her unique reasons for disliking glasses or contacts. 5 Horizon Laser Vision Center offers different types of vision corrective procedures to help reduce your dependence on glasses or contacts: ilasik (IntraLase laser assisted intrastromal keratomileusis), custom PRK (photorefractive keratectomy), and KAMRA Vision. These procedures are suitable for people who have nearsightedness, farsightedness, astigmatism, and/or presbyopia. Horizon Laser Vision Center is pleased to offer its patients the latest technology. Our IntraLase Method is a 100% bladefree approach to creating your ilasik flap, or the pocket for your KAMRA Vision. Our VISX Star S4 Excimer Laser System, equipped with idesign, 3D Active Tracking and Iris Registration, has been used successfully to treat more than five million eyes globally. These technologies, described on the following pages, allow your surgeon to tailor your treatment to your visual needs. It is through these technologies that we are able to be the ONLY ilasik clinic in Saskatchewan, and leaders in treatment for near vision loss, KAMRA Vision. These procedures are safe, proven effective, painless and affordable.

7 The IntraLase Method A Blade-free Breakthrough The IntraLase Method is a 100% blade-free approach to creating the thin flap of tissue that the doctor folds back in order to perform your ilasik procedure. It can only be performed using the IntraLase procedure. IntraLase is also used to create a small pocket for your KAMRA Vision procedure. Traditionally, doctors have used an instrument called a microkeratome a hand-held blade that moves across the eye for the creation of flaps. Regular LASIK is very safe, but if complications do occur, the microkeratome is frequently the cause. Our blade-free ilasik approach removes this possible source of complication. With the IntraLase Method, tiny pulses of laser light, a quadrillionth of a second each, pass harmlessly through the outer portion of your cornea and form a uniform layer of microscopic bubbles just beneath the surface of your eye.

9 The exact dimensions of this layer of bubbles are determined by your doctor based on what s best for your eye, and are computer controlled for maximum precision things that are not possible with a hand-held blade. The IntraLase flap creation process takes about 12 seconds per eye. In fact, patients are usually surprised how quick and easy the entire procedure is. When it s time for your ilasik treatment to be performed, your doctor easily lifts the flap. When ilasik is complete, a flap created using the IntraLase Method is uniquely able to lock into place. Your eye begins to rapidly heal. With the IntraLase Method, patients may experience fewer flaprelated complications and fewer induced higher order aberrations that contribute to visual disturbances, such as glare and halos. It s the most advanced treatment available and millions of procedures have been performed safely and effectively. It also brings some results better than 20/20 vision. If you are having the KAMRA Vision procedure, a treament for near vision loss, a small pocket is created with the IntraLase. This allows the surgeon to insert the KAMRA inlay over the pupil in one eye.

11 Our Laser Equipment The VISX Star S4 Excimer Laser System is a computer-assisted, surgeon-controlled device that includes idesign, ActiveTrak and Iris Registration technology. The idesign technology uses light waves to identify and measure imperfections in the eye 100 times more accurately than standard methods used for glasses and contact lenses. These measurements are used to individually customize vision correction. The idesign produces a WavePrint map, which is a visual representation of how the patient s entire optical system processes light. This map is much like a fingerprint; no two are alike. It allows for a more precise and detailed analysis of a patient s vision and is used by the ophthalmologist to ensure optimal treatment results. ActiveTrak 3D Eye Tracker follows the tiny motions of your eye instantaneously and automatically, in all three dimensions, continually repositioning the laser to ensure accuracy. Without a tracking device, laser vision correction requires patients to hold their eyes as still as possible during the procedure. However, even a still eye makes minute, involuntary movements, including moving slightly with each breath. With 3D ActiveTrak, you can relax during your laser vision correction, knowing the procedure will be delivered with precision and accuracy. Iris Registration (IR) is an enhancement that works by recognizing the unique pattern of each eye and precisely aligning the laser so that the surgery is even more accurate. The end result is sharper, better quality vision.
12 Cost and Payment The refractive surgery fee, paid at the time of surgery, covers your pre-operative assessment with the Horizon doctors, all of your post-operative visits with your optometrist and ophthalmologist for one year, all of your prescription medications after surgery, and all of your artificial tear preparations for the first month after surgery. The fee also covers an enhancement or re-treatment for two years following your initial treatment, if clinically advisable. With technology advancements, fewer patients are now in need of an enhancement. After your pre-operative assessment with the Horizon doctors, if you are a candidate for surgery and want to book for surgery, we require a $1000 deposit to hold the surgery slot for you and then the remaining balance is due on the day of your surgery. You may pay by certified cheque, VISA or Mastercard. We also offer financing options. Remember to retain all surgery-related receipts for tax purposes. 13 Risks As with any surgery, there are risks and these are detailed in the consent form you will receive prior to surgery. The major risk is not obtaining the desired result and continuing to need glasses or contact lenses. As in any procedure, there is also a small risk of infection, but this can be controlled in the majority of patients by proper compliance with the immediate post-surgical instructions. Currently there is no evidence of long-term effects on the cornea, and the long-term effects of refractive surgery appear to be minimal. Patients must have realistic expectations of how refractive surgery can help them. While the surgery is ideally designed to eliminate the need for glasses or contact lenses, there is no guarantee of the level of vision patients will achieve. Patients will be provided with a thorough explanation of the expected results, but the individual eye and how it heals after the procedure largely determine the final outcome. Refractive surgery has been performed successfully at Horizon Laser Vision Center for over 17 years with excellent results. More than 23,000 patients have had successful surgery in that time.

15 How to Proceed To determine if you are a candidate for refractive surgery, call your optometrist or Horizon Laser Vision Center for direction. If your last eye exam has been within the last 6 months, your optometrist can send information about your eyes to Horizon, including a full history. Any pertinent information about your eyes will be sent to Horizon. If it has been longer than that, an updated full eye exam may be required. You will be contacted to book pre-testing at Horizon and a consultation with the Horizon doctors, who will evaluate your eyes to determine if you are a suitable candidate. The doctor will assess your eyes from a medical perspective and discuss the benefits and the risks of surgery with you. The doctor will also discuss with you which type of surgery is recommended for your particular case. To prepare for surgery, plan to follow the recommended schedule for discontinuing contact lens wear. On the day of surgery, you will need someone to drive you home after surgery. If you are from out of town, please plan to stay overnight following your surgery. Depending on the surgery performed, you will need to see the Horizon doctor or surgeon for a post-op check 1 to 5 days following your surgery. You will need to take 3 7 days off work to rest the eyes after surgery.
16 On the day of surgery, allow yourself 1 2 hours at Horizon Laser Vision Center, although you are often there for less time. You are in the laser room for minutes, but the actual laser time is less than a minute. You should wear comfortable clothing, which does not pull over your head after surgery. Bring along a friend or family member to drive you home after the procedure. All instructions for caring for your eyes will be given to you on the day of surgery. You will receive all the medication you need, as well as sunglasses and eye shields to protect your eyes during sleep. You will be awake during the procedure, but will be given numbing eye drops. You may be offered a relaxant prior to surgery.
17 After the surgery, you will experience some discomfort. Although the procedure is painless, the eye takes time to heal. The amount of discomfort varies from person to person, and also depends if you have ilasik, PRK or KAMRA. You will be given pain medication to relieve the discomfort. Rest your eyes after the surgery. The less you use your eyes, the more comfortable you will be. Be careful driving or operating equipment for 2 7 days following surgery. Your night vision may take longer to get back to normal than day time vision. The restriction on your license, if applicable, will be removed by your optometrist, when your vision meets the visual requirements. Avoid swimming pools, tanning beds, saunas and steam rooms for a minimum 2 weeks following surgery. Use good sunglasses at all times following surgery.
