1 Kasik Tax & Accounting 2014 Tax Service Engagement Letter What you can expect from us: One completed copy of the 2014 income tax return, with the preparer s name listed in the paid preparer section of the signature box (page 2 of the Form 1040). *For information on additional copies see Notes, 1.* A copy of all information provided to us by you for preparing the taxes. Please keep for your records, we do not keep all tax information you provide us! An opportunity to take the return home to review it after it has been prepared and to ask questions about its contents or the lack thereof and receive an answer to these questions prior to the filing of the return. *For more information, including potential additional fees for this service, see Notes 2 * If we are unable to keep an appointment with you we will contact you as soon as possible at the numbers you ve given us. If we close due to bad weather we will report the details to KCRG as soon as the decision is made. On days where the weather is bad, please check KCRG cancellations online or call just before coming out. What we expect from you: A legible, completed pro-forma (available online at kasiktax.com or in our office), and all important tax information needed to prepare the tax return. o If you have any doubts or don t know what is needed please schedule a phone appointment or in-office appointment so we can help you get organized. * For more information, including additional fees for this service, see Notes 3 * A signed engagement letter (this form), that you have read, understand, and agree to comply with fully. A signed statement that you have read and understand our privacy policy. That you tell us if you do not understand something, particularly if that something is something you are signing. Complete honesty in regard to your tax situation, that you will answer any questions that we have to our satisfaction and that you will not withhold any information that we might need to prepare the returns. If you are unsure if something has a tax consequence you will notify someone in our office of that possibility so we can discuss it with you. A call to let us know if you will be unable to make an appointment you ve scheduled with us. We reserve the right to bill you $25.00 for any appointments that you do not call to cancel at least 4 hours ahead of time.

2 Kasik Tax & Accounting 2014 Tax Service Engagement Letter (p2) Limits on Liability: We are not omniscient! We do not know what happens in your daily lives and we rely on you to keep us in the know. Even things that aren t necessarily financial in nature can have an impact on your return. The list of things that might apply are endless or might as well be, so in the interest of keeping things short just go with the accountants need to know everything rule of thumb. In the event that you forget to give us something, or otherwise give us bad information you agree that you will not hold us liable for any damages that result from that omission or error. Again, if you aren t sure, let us know, say something, or schedule an appointment. We can t help if we aren t aware that there s an issue! We reserve the right to refuse service: In the event that you fail to answer questions we have for you, or get us information we need in a timely manner you understand that we have the option to terminate our agreement to provide services at that point and we will not be responsible for any damages that result from that action. (example of this : you dropped off most of your information in February and the questions we have called you about several times are still not answered on April 14 th, ) IF we decide to terminate this agreement, we will contact you at the phone number(s) you left with us to make arrangements for you to pick up anything in our office at that point. If we are unable to reach you and you haven t contacted us about picking up your information by May 1 we reserve the right to mail it out to you at the address you gave us along with an invoice for $10.00 to cover the costs of shipping. We will use USPS with delivery confirmation as the carrier. You have the right to terminate our agreement: If you change your mind and want to terminate our agreement you may do so by calling us at our office or by coming in and telling us in person. We will give all your information back to you. If you terminate the agreement before we call to let you know the return is finished, we will not bill you for any time we have spent on the return up to that point. If you terminate the agreement after we have prepared the return, you will not be billed as long as all copies of our work are returned to us and you sign a statement agreeing that in the event that any copies of our work are in your possession you will not use it for the purpose of doing your taxes or for any other purpose. If you fail to sign the statement or fail to return all copies to us you agree to be responsible for paying the full tax preparation fee. After the return has been electronically filed neither party may terminate this agreement.

3 Kasik Tax & Accounting 2014 Tax Service Engagement Letter (p3) Payment: Payment is required at the time of service for appointments, and at the time of pickup for tax preparation service. Returns will not be filed until all fees have been paid, UNLESS you have made alternate arrangements with us. IMPORTANT PLEASE NOTE: At 12:00 noon on April 15, 2015 we will file an automatic extension for anyone who s return is not complete, or who has not picked up their finished returns, or who has not returned the signature forms or other needed documents (we will tell you if you have others that need signatures) for electronic filing. This extension will provide you with additional time to turn in your information (if applicable), review your return, and get those signature forms to us. **The additional charge for filing an automatic extension is $20.00 and will be added to the cost of your return at the time the service is rendered** ***********************************Notes************************************** 1. Additional copies after the first one provided will cost $10 per copy, if you wish us to mail, fax, or your return to you- you must first sign a release form that we will provide to you at the time of the request. 2. We recommend contacting us via telephone or . We will try to return calls made during office hours the same day if possible. In office appointments to review returns will be billed separately and based on the amount of time taken at a rate of $100/hr. the minimum fee for this service is $ Changes made to returns after review will result in additional costs, depending on the complexity of the changes made consult your preparer for an estimate if needed. 3.
