3 Vaccinations for Infants Inquiries: 3 rd floor, counter3-, Kenko-Zukuri Kakari (Health Maintenance Section), tel Vaccinations are available for groups on the 4 th floor of the Public Health and Welfare Center (see p. 6), and for individuals at hospitals and clinics in your neighborhood (p. 8). 2 Whether coming as an individual or in a group, bring along your boshi techo (maternity record book) and yoshin-hyo (vaccination coupons or questionnaire). If you do not have yoshin-hyo ( questionnaire), come to the Public Health and Welfare Center (ward office, 4 th floor) and receive one at Counter 4-1 by showing your boshi techo (maternity record book). Yoshin-hyo (questionnaire) is available in English at the Public Health and Welfare Center. 3 If your child is sick, vaccination should be put off until a later time. Come to the 4 th floor with your maternity record book (boshi techo) and questionnaire (yoshin-hyo). An appointment is not necessary. Polio live vaccine Age group for free vaccination: 3 months to 7 years and 5 months. (It is best to receive the vaccination twice between the ages of 3-18 months.) Date and time (1:00 1:50 p.m.) are assigned, but if you cannot come on the designated day, you may come on the next checkup day. 8 Welfare Services Medical expense subsidies for children to age 5 For children less than 1 year old, medical treatment is free if the parent has joined the national health insurance plan. In case of other health insurance, you need to apply for a free treatment certificate.
