2 September 11, 2013 Kickoff Dinner Dr. Kent Knoernschild Occlusal Loading & Prosthetic Complications Erie Café 536 W. Erie St Chicago. IL Dr. Kent L. Knoernschild is Associate Professor, Program Director of the Advanced Education Program in Prosthodontics, and Co-Director of the Implant and Innovations Center at the University of Illinois at Chicago. Dr. Knoernschild is a Diplomate of the American Board of Prosthodontics and holds a private practice limited to prosthodontics. He has lectured internationally on the comprehensive treatment of patients with complex restorative needs based on best evidence. He is the ADA Commission on Dental Accreditation Commissioner for Prosthodontics, and has served as President of the American Academy of Fixed Prosthodontics and the International Association of Dental Research Prosthodontics Research Group. Dr. Knoernschild has conducted clinical and laboratory research and has mentored residents who received internationally recognized awards. He has given over 100 presentations nationally and internationally, authored more than 50 publications, and received more than 20 research grants. Occlusal Loading & Prosthetic Complications: Incidence, Rationale and Resolution. Complications in implant dentistry, particularly surgical complications, are generally considered to occur at a low level. Prosthodontic complications, on the other hand, can be quite common, problematic to patients, and at best annoying to the clinician. The spectrum of prosthodontic complications will be reviewed with a focus on etiology, frequency, and avoidance. The attendee should obtain valuable information relative to avoidance of prosthodontic complications.

3 October 16, 2013 Full Day Dr. Rick Roblee Interdisciplinary Dentofacial Therapy Dr. Roblee practiced general restorative dentistry in Dallas, Texas before going back to study graduate orthodontics at Baylor College of Dentistry. He is recognized worldwide for his work in esthetic dentistry, orthodontic techniques and interdisciplinary therapy. He has given more than 500 professional presentations in the United States, Canada, Europe, Australia and Japan on these topics. Dr. Roblee is a Diplomate of the American Board of Orthodontics a distinction that less than 20% of practicing orthodontists actually receive. He is the author of numerous publications and videos, including a textbook entitled Interdisciplinary Dentofacial Therapy: A Comprehensive Approach to Optimal Patient Care (Quintessence Publishing Co., Inc.). He is also an associate clinical professor at Baylor College of Dentistry in both the restorative department and the orthodontic department and is an active member in numerous prestigious dental organizations, including the American Academy of Fixed Prosthodontics and the American Academy of Esthetic Dentistry. While maintaining a full-time practice in Fayetteville and Springdale, Dr. Roblee continues to be instrumental in the development of Web-based collaborative software for dentistry. In creating the first interdisciplinary digital network for clinical records through his company, ushealthrecord.com, Dr. Roblee provides a vehicle for exceptional patient care through the collaborative value of shared images, treatment planning and patient services. Truly an information engine for all of dentistry, Dr. Roblee further supports his passion for education by combining the online records component with a continuing education platform for study clubs around the world to utilize.

4 Interdisciplinary Dentofacial Therapy Dr. Roblee with discus interdisciplinary dentistry utilizing orthodontic therapy. Many aspects of othodontics will be included from malocclusion, implant therapy, ridge development & complex restorative dentistry. The goal of the lecture is to incorporate orthodontic therapy in interdisciplinary care. The lecture will focus on some of the most cutting edge aspects of orthodontics. Some of the most challenging esthetic dilemmas are those that fall into The Gray Zone between the normal treatment protocols of two or more disciplines. Severe cases of black triangles, periodontal defects, implant placement problems, gummy smiles and insufficient lip support are examples of this category. The complexities of diagnosing these problems, coupled with the lack of optimal treatment options and inadequate collaboration, have often led to either inappropriate treatment or disappointing compromises. The presentation will focus on an interdisciplinary approach to diagnosing problems in The Gray Zone and managing them by combining traditional therapies with new treatment options that more predictably achieve success. These new options include Temporary Anchorage Devices (TAD s), Periodontal Accelerated Osteogenic Orthodontics (PAOO) and Dentoalveolar Distraction Osteogenesis (DDO). To facilitate interdisciplinary collaboration, the most advanced internet based virtual record platform will also be discussed. Dr Roblee will also review the use of Invisalign in complex restorative cases for the general practitioner who already uses Invisalign or is considering beginning cases. Learning Objectives: Esthetic problems frequently fall into a Gray Zone between the different disciplines. An interdisciplinary approach is needed to properly diagnose and treatment plan these problems. Recent advances in the various disciplines now enable us to more predictably treat these problems. Internet-based virtual records are a powerful collaboration tool for the interdisciplinary team. Discuss the role of Invisalign in interdisciplinary cases.

5 December 13, Half Day Holiday Staff Appreciation Mr. Dave Weber The 12 X s of Success Dave Weber is an internationally recognized speaker, with a track record of success. He is an honors graduate from Mercer University with a BA in communications. He simultaneously captained the university s NCAA Division I Soccer and volleyball teams for three years and received ALL-Conference honors in both sports. As a sales representative for Motorola, Mr. Weber received over 15 outstanding sales awards, including the prestigious Top 1 award each year he was with the corporation. In 1987, he started his own speaking and training firm. Dave s high energy and entertaining style have made him a crowd favorite. He s been nicknamed a Chihuahua on caffeine and while his style might initially get him invited to present, it is his great content and timeless principles that get him invited back time and time again. The 12X s of Success Based on the premise that you don t have to have a title or letters after your name to be a leader Dave shows that you can be a leader because you re a person of influence. Dave will share 12 key principles that all begin with the letter X! 6 January 15, 2014 Full Day Dr. Chandur Wadhwani Restoratively Driven Implant Failure Dr. Chandur Wadhwani received his undergraduate and dental degree from the University College London School of Dentistry in He graduated top of his class and was awarded honors and distinction in Dental Surgery. He lectured at the Dental School as a part time clinical lecturer in the restoration department for many years. The remainder of his time in London was in private practice, working in the prestigious Harley Street. Dr. Wadhwani received his specialty certificate in prosthodontics with a Masters Degree from the University of Washington School of Dentistry in Dr. Wadhwani is an affiliate instructor in the Department of Restorative Dentistry at the University of Washington and is past president of the Washington State Society of Prosthodontists, past President of the Tucker 60th study club, and one of the founding members of the Eastside Dental Specialists group. Dr. Wadhwani has lectured both nationally and internationally on topics related to clinical prosthodontics and implant dentistry. He is the primary author on several publications in peer reviewed dental journals such as the Journal of Prosthetic Dentistry International, Journal of Oral-Facial Implants, Journal of Esthetic and Restorative Dentistry, British Dental Journal and Dentistry Today. Dr. Wadhwani is presently involved in several aspects of dental research in Endodontics, Periodontics and Prosthodontics in conjunction with the University of Washington. Dr. Wadhwani has been awarded the prestigious Top Cosmetic Prosthodontist by his professional peers in Seattle Magazine s Top Dentist issues for 2009, 2010, 2011 and 2012.

7 Restoratively Driven Implant Failure Traditional tooth related dentistry relies on cementation to attach restorations to teeth. Although dentists are relatively proficient in this area, very little is known about using cements with dental implants. To date no protocol exists for cementing implant retained restorations. Several techniques and products have been developed for this purpose, however most are arbitrary and lack science. Many types of cement are problematic; some corrode titanium and most are undetectable on radiographs, few have been tested for microbial activity. This lecture will examine these issues and give up-to-date and as of yet unpublished research data that will be used to determine a suitable protocol for use in clinical practice. Implants are becoming common practice for dentists, but few have a good understanding of how to restore well, and maintain the health of these medical devices. The lecture is designed to give restorative dentists an understanding of the differences between restoring the Natural dentition and Dental implants (Nature verse Manmade). The "pitfalls" of not understanding and using scientifically based restorative materials and techniques may lead to destruction of the implant supporting tissues (soft-tissues and bone). This "wholistic" approach lecture will bring together the methodology, and materials needed, with absolute indications on what to do and what to avoid. Cement flow will be demonstrated, application technologies explored and microbiological aspects described. Implant health measurements will also be evaluated, radiographic techniques, probing issues along with other measurements will be thoroughly explained and clinical relevance given. At the conclusion of this course, attendees will be able to: - Understand the problems associated with restoring dental implants - Understand the biological difference between teeth and implant soft tissue connections - Select appropriate implant materials, cements and implant systems for specific restorations based on research and science - Prevent future failures and understand limitations -Clinically relevant techniques will be discussed that can be implemented easily into every day dental -practice

8 April 16, Full Day Dr. John Burgess Restorative Topics to be Voted. 211 E. Chicago Ave., Chicago Dr. John Burgess is the Assistant Dean for Clinical Research and the Director of The Biomaterials Graduate Program at the University of Alabama in Birmingham. He graduated from Emory University School of Dentistry and completed graduate training at the University of Texas Health Science Center in Houston. After completing a General Practice Residency and a General Dentistry Residency in the Air Force, he served as military consultant to the Surgeon General in General Dentistry. He was Chairman of Dental Research and Dental Materials at Wilford Hall Medical Center before retiring from the USAF. Dr. Burgess is a diplomat of the Federal Services Board in General Dentistry and the American Board of General Dentistry. He is a Fellow of the Academy of Dental Materials and the American College of Dentists, as well as an elected member of The American Academy of Esthetic Dentistry and The American Restorative Academy. He is a member of the Academy of Operative Dentistry, The American and International Associations for Dental Research, the Alabama Dental Association and the ADA. Currently he is a member of the ADA Council on Scientific Affairs. A dedicated researcher he has served as the principal investigator on industrial, foundation, state and national grants. Dr. Burgess reviews for four dental journals, is the author of over 300 journal articles, textbook chapters and abstracts and has presented more than 800 continuing education programs nationally and internationally. Dr. Burgess is an active investigator on clinical trials evaluating posterior composites, adhesives, fluoride releasing materials, impression materials and class 5 restorations. He maintains a part-time practice in general dentistry. TITLES OF LECTURES AVAILABLE AND CONTENT Maintaining the restored dentition (High Caries Risk Patient). (1.5 hr) Ever had a patient whose name on the appointment schedule strikes fear into your heart because your recently placed dentistry is failing. Not only do you do not want to show this case to other dentists as your typical dentistry you would like to hide the patient? Beginning with an easy to use caries risk assessment to quickly identify patients at risk, this lecture discusses factors which increase tooth decay and details in a logical step by step manner how to motivate patients to remove those factors. Useful oral hygiene

9 instruments, modifications and use and effectiveness of remineralizing agents, resin infusing materials like ICON are detailed for at home and office use to treat those patients who are caries prone. But that is just the start as new materials have developed with significantly different mechanisms of action which are extremely useful for high risk patients. Amorphous calcium phosphate, MI Paste Plus, Novamin, ICON, ph modifiers, fluoride supplements and xylitol will be advocated as alternate and supplemental methods for treating high caries risk patients. Useful websites and instrument modifications will be shown to improve the patient s ability to maintain your restorative treatments. Bases and Liners, Vital Pulp Treatment or Should I etch the pulp. (2 hour) The types and materials used in the most current bases including calcium hydroxide, visible-light cured resins, glass ionomers and visible-light cured glass ionomer. Included in this topic is the use of no base or liner, MTA, Biodentine, TheraCal LC as well as vital pulp therapy (Cveck, indirect and direct pulp capping) with adhesives and the results from ongoing clinical trials with no bases under restorative materials. Vital pulp therapy attempts to maintain pulp vitality as a method of preserving tooth strength and integrity. This presentation will classify vital pulp therapy in permanent teeth by describing and comparing the success rates of direct and indirect pulp capping techniques and the Cveck pulpotomy. Beginning with a step by step diagnosis for the vital tooth, predictable protocols including the materials used and the success rates of these materials will be supplied for each procedure. This presentation will address some controversial issues like should I etch the pulp and place an adhesive over the exposure or are calcium hydroxide releasing materials more effective? Understand which dentin stimulating material to use to produce the most effective results and when vital pulp therapy is critical for the long term success of the tooth. Contraindications to vital pulp therapy based on the restorability of the tooth, furcation position and caries index will also be discussed. Ceramics. (2 hours) If you are placing all ceramic restorations you should hear this presentation. Ceramic materials have evolved from weaker feldspathetic porcelain to high strength zirconia cored materials. Are zirconia restorations producing significant enamel wear on the opposing enamel? Are they viable restorative materials and how much reduction is necessary to produce viable restorations with all ceramic materials? E.max and other ceramic materials, mechanical and physical properties will be discussed. Indications, where to use these materials and contraindications will be discussed. All this and nice cases as well. Cements. (1.5 hour) This lecture covers all classes of luting agents from zinc phosphate to glass ionomer and resin modified glass ionomers to adhesive resin cements. Classifying resin cements into total etch, self-etch and self-adhesive divisions simplifies their use. The proper use of these luting agents is demonstrated and their application in specific clinical cases (veneers, all metal, post cementation, ceramic onlay, high strength ceramic materials and conventional metal ceramic restorations) is demonstrated. The proper manipulation and application of the newest materials such as Fuji Plus, Fuji Cem, RelyX, RelyX ARC, Enforce, Panavia 21, UniCem 2, BisCem and MaxCem Elite will be compared to ensure success with non retentive preparations. Provisional cementation will also be discussed. Curing lights. (1 hour) This presentation provides up to date information on different curing units such as the high intensity halogen. PAC light, argon laser curing systems and

10 LED curing lights. Advantages and disadvantages for each unit will be discussed. Different methods of curing composite resin including incremental curing, bulk curing, soft curing, ramp curing modes and pulse phase curing modes will be discussed. Recommendations for maintaining curing units and practical suggestions for their upkeep will be discussed. Bonding agents. (2 hours) Dentin bonding agents are discussed to provide answers as to what agent works best in what situation. Fourth (Scotchbond MP Plus, ProBond, Allbond 2, Optibond) fifth generation materials (One Step, Optibond Solo, and Prime and Bond NT), sixth and seventh generation bonding agents will be compared and current techniques demonstrated to ensure clinical success. The one and two bottle selfetching materials, Clearfil SE Bond, Easy Bond, I Bond, Easy Bond and Prompt L-Pop will be evaluated. A discussion of wet bonding, the hybrid layer and sclerotic dentin management will be shown. The newest and first Universal Bonding agent will be discussed: Scotchbond Universal, it effectiveness in bonding to metal, ceramic, dentin and enamel as well as the results of an ongoing clinical trial will be shown. Effective management of hypersensitive teeth is discussed. The bonded amalgam restoration will be described and the results of current clinical trials evaluating bonded amalgam restorations will be revealed. Bonding to other intra-oral substrates is also illustrated. Effective porcelain repair using current repair kits and metal bonding techniques is shown. Restoring endodontically treated teeth. (2 hours) When are posts necessary and which post system should be used for anterior and posterior teeth? Advantages and disadvantages of active and passive posts are discussed. Using esthetic ceramic, carbon fiber and fiber containing composite resin posts to their fullest potential, the esthetic treatment of discolored roots will be shown. Advantages and specific limitations of post materials are emphasized. Core and foundation materials are compared and advantages of each are provided. Cement selection for the final restoration is discussed. Successful Techniques for Whitening Discolored Teeth. (2 hours) From a little whiter to extreme white, this presentation covers a wide range of clinical cases and includes techniques that produce whiter teeth for most people. Successful techniques for bleaching vital and non-vital discolored teeth are demonstrated in a detailed step by step series of techniques. For example chair side, abrasive, and home bleaching techniques will be discussed as well as success experienced in combination applications. Bleaching difficult cases will be shown (tetracycline and other) and methods to obtain whiter teeth for these patients will be revealed. Chairside high intensity halogen light bleaching success with the Zoom and Rembrandt Lighten will be shown and discussed. Effective methods to decrease cold sensitivity will be demonstrated.
