MANAGING GROUP REQUIREMENTS: PMO GROUPS 2.1 SAFETY Note: the Contracting Party is to review the following documents as applicable to the work and in conjunction with the associated contract documents: General Requirements for Ontario Operations Managing Group Requirements Contract-Specific Requirements Plant-Specific Requirements Note: all items below are in addition to the items specified in the other documents listed above. Also: for PMO Projects, the Vale Representative is the PMO Construction Coordinator in all cases Applicable Standards Training Requirements Safety Plan Compliance Audits by Vale The Vale Representative will complete a weekly audit of the compliance to the Contract-Specific Safety Plan (NAV-TP-0095). The audit report (NAV-TP-0023) will be forwarded to the Contracting Party, complete with all the related documentation. All noted deficiencies must be addressed immediately First Aid All Construction work sites shall include a First Aid kit and the poster entitled In case of Injury at work (form 82) all as described in the First Aid Requirements regulation Contract-Specific Safety Plan A Contract-Specific Safety Plan (NAV-TP-0095) must be submitted, including a schedule listing all of the planned safety activities such as of Construction PHR Reviews, Toolbox Meetings, Safety Meetings, Site Inspections & Audits, Mobile Equipment Pre-operational Inspections, and JHSC Joint Inspections etc. This is to be submitted in the standard format for Vale s Surface / Mines PMO Group projects. This must be posted onsite. DM # v6 Page 3 of 6
4 2.1.6 Safety Plan Documentation 2.2 HEALTH 2.3 ENVIRONMENT The Contracting Party must record all safety activities listed in the Contract-Specific Safety Plan (NAV-TP-0095), and maintain copies of these documents at the site office. During the execution of the Work, the Contracting Party shall submit Weekly Safety Compliance Reports (NAV-TP-0023) to the Vale Representative. These reports shall include the associated documentation to confirm the Contracting Party s compliance to the Contract-Specific Safety Plan activities. The report is to be submitted in the standard format for Vale s Surface / Mines PMO Group projects. No additional requirements see Section 1 General Requirements or Section 3 Contract Specific Requirements. 2.4 QUALITY ASSURANCE 2.5 SCOPE 2.6 SCHEDULE 2.7 CONTRACT MANAGEMENT Daily Reporting The Contracting Party shall contact the Vale Representative at least one working shift prior to the start of the shift (ideally, the previous calendar day) with the following information: Description of the daily activities Number of workers Duration of shift Description of the daily safety information provided to the workers Any potential issues, delays, safety concerns that have been identified Coordination requirements with Vale or other parties DM # v6 Page 4 of 6
5 Where required by Plant / Mine protocols, the Contracting Party s Representative is to attend the pre-shift line up meeting with Vale Operations. The Contracting Party shall submit the following documents to the Vale Representative, to the agreed-upon time & format: a) Any schedule revisions. b) Time and Material sheets for the previous shift, where appropriate. Time and Material sheets must be signed and a copy provided to the Vale Representative within 24 hours of the work being performed. The original Time and Material sheet must be attached to the associated invoice as confirmation. c) Written notification of any extra work identified during previous shift, complete with the estimated cost & schedule impacts. See Field Work Instruction in the Section 1 General Requirements document. d) Note: if there is a change in the manpower requirements and / or duration of the scheduled work for that day, the Vale Representative must be notified immediately Weekly Reporting for On Site-Work One or more representatives of the Contracting Party shall attend the Weekly Construction Review Meeting arranged by the Vale Representative. The Contracting Party shall submit the following documents to the Vale Representative weekly, by the agreed-upon day & time, and in the agreed-upon format: a) Weekly Safety Plan Compliance Audit (NAV-TP-0023) b) Follow-up from any related Incident / Injury Investigation Reports, if applicable c) New or Revised PHR s and Job Procedures, if applicable d) Total number of man-hours worked during the past week broken as per the Weekly Safety Plan Compliance Audit (NAV-TP-0023) e) Total number of man-hours worked to date on the contract f) Updated Schedule, showing work completed to date, details of work scheduled for the upcoming week, and milestone activities to end of the contract g) Description of the work completed in the previous week and planned work for the upcoming week. h) All required QA/QC reports, such as NDE reports, outstanding work inspections, and related project data. i) Estimated contract cost for the past week, and the total contract cost to date j) Forecasted costs for the next week and forecasted cost to completion of the contract DM # v6 Page 5 of 6
6 2.7.3 Post-Contract Documents The Contracting Party shall submit the following documents to the Vale Representative upon completion of the Work: a) Finalized Contract Specific Safety Plan Compliance Audit (NAV-TP-0023) b) Finalized Quality Assurance Checklist / Plan (NAV-TP-0168), with all back-up documents c) Total Man-hours worked on the contract on the Compliance Audit (NAV-TP-0023) d) As-built drawings e) Commissioning documents, where required f) All equipment manuals, reference documents, and technical reports associated with the Scope of Work g) Final Invoice, with a final Outstanding Claims Notice / Claims Waiver h) Contract Completion Certificate (NAV-TP-0195) i) Where applicable, Statutory Declaration, WSIB Clearance Certificate, Statement of Invoices (NAV-TP-0199), Deficiency List(NAV-TP-0185) Pre-Job Conferences For contracts involving greater than 5,000 total man-hours of labour, the Contracting Party shall hold a Pre-Job Conference ( Mark-up Meeting ) with representatives from the local Trades Unions applicable to the Work, to review the scope of work and the assignment by Trade. The Pre-Job Conference is to be held after award of contract, and prior to mobilizing to site. Representatives of all subcontractors must attend. The Vale Representative is to be invited. The Contracting Party shall document the outcomes of the Pre-Job Conference as per PMO standards, and distribute to all participants. In the event that non-unionized workers will be employed onsite for the contracted work review any potential conflicts between unionized and non-unionized workers with the Contracting Party. 2.8 SITE OFFICES & TEMPORARY FACILITIES 2.9 PERMITS AND PASSES 2.10 DEFINITIONS DM # v6 Page 6 of 6
