3 Patient Portal 2.0 for meridianemr New Demographics and Patient History Values To meet Meaningful Use 2014 Edition requirements, the Demographic and Patient History values include updates to language, race, ethnicity, smoking status, and family conditions. Appointments: Message about Early Arrival On the Patient Portal Welcome page, the Appointments section displays a new standard advisory message if an appointment is scheduled: Appointments for surgery may require you to arrive early. Check your pre-op instructions. Patient Portal Password Generation To generate a Portal password for a patient, add the patient s address in the Demographics window > Edit Patient. 3
4 In the Demographics Chart Window, click Generate Password (Reset Password is the button label after you ve generated a password the first time, and the chart has been refreshed). A popup displays the generated password. Be sure to write down the new password accurately. Note: To activate the new password, you must click OK to close this window. If you leave this window open, you may receive errors when logging into portal with the newly created password. 4
5 Authorized Patient Representative (APR) Add/Edit Contact In order to allow parents and legal guardians to have access to a patient s portal account, the practice can indicate in Add/Edit Contacts which contacts may have access to the patient s portal account (Authorized Patient Representative). 5
6 To add an Authorized Patient Representative, go to the Add Contact or Edit Contact window. Check the Authorized Patient Representative (Patient Portal) check box. When you select this check box, the Address and the From Date are required fields. The From date defaults to the current date. The To date is not required but if used, it determines when the APR s access to that patient s portal account expires. If no To date is selected, the APR will have no time limit for accessing the portal account. o When the date range expires, or if the practice unchecks the Authorized Patient representative (Patient Portal) check box, that person will no longer have access to the patient s Portal account. Passwords for APR: To generate a password for a new Authorized Patient Representative, click Save and Generate Password. To reset a password while editing an existing Authorized Patient Representative, click Save and Reset Password. A popup displays the generated password. Be sure to write down the new password accurately. Note: To activate the new password, you must click OK to close this window. If you leave this window open, you may receive errors when logging into portal with the newly created password. After you set up the Authorized Patient Representative and provide a password, the APR can log into Patient Portal to view and/or change information on the patient s behalf. 6
7 Contacts Chart Window We renamed the Emergency Contacts window to Contacts, and added the new fields from Add/Edit Contacts to the Contacts grid. To generate a password for an Authorized Patient Representative from this window, right-click the desired contact and select Generate Password (or Reset Password to reset it) from the popup menu. 7
8 Selecting a Patient Portal Account to View A single individual may be an Authorized Representative for multiple patients. For example, a parent may have two children who both see the same physician. To access Patient Portal as an authorized representative with multiple accounts: 1. Log into Patient Portal by entering your username and password. The Select a Patient dialogue box displays. 2. Click to select the desired account name from the list. That patient s Portal Home page opens. The page header displays the name of the person logged into the Portal. 8
9 Note: Each patient s authorization is independent. If the same person is an authorized representative of two patients, and authorization for one patient s account expires or is revoked, the authorized representative can still log in and access the Patient Portal account for the other patient. View, Download, Transmit CCDA-formatted Documents As part of the Meaningful Use 2014 Edition requirements, patients must be able to view, download, and transmit a Consolidated Clinical Document Architecture (CCDA) formatted document sent by the practice from the EMR to the Patient Portal. The CCDA format is required to transmit records via DIRECT messaging, a service provided by a third party that allows providers to participate in a network to securely send and receive medical records. These records can be automatically imported into different EHRs. The Practice creates the CCDA-formatted document in meridianemr and sends it to the patient s portal account. On the portal, the document displays in the Documents and Lab Results view. To meet HIPAA privacy regulations, the system encrypts the document when transmitting it, but converts it to a humanreadable format when the patient views it. To learn more about creating CCDA-formatted documents and sending them to the Portal, please refer to your EMR release notes. Viewing and Downloading CCDA Documents in Patient Portal 1. With the desired document displayed, select Download from the Documents and Lab Results window. 9
10 2. Double-click the document name and select the file format. The file goes to the computer s Download folder for you to save in the desired location. 3. Click Save and navigate to the location where you want to save the file. 10
11 Note: If the file is not a CCDA-formatted document, only the Print button displays (as shown in the following example). Download and Transmit buttons are not available. DIRECT Transmission from Patient Portal The Transmit part of the MU2014 Edition requirement for View, Download, and Transmit includes providing the ability for the practice and the patient to transmit CCDA-formatted documents to other providers via DIRECT messaging. If your practice has not purchased the third party service necessary for transmitting via DIRECT, this functionality will not be available to your patients. If you have purchased the DIRECT messaging service, you can contact your meridianemr representative to have the patient transmit feature enabled or disabled on your patient portal. Note: Transmit functionality requires the use of a third party service and is available for purchase. Please contact sales@healthronicsit.com for more information. 11
12 Transmitting a CCDA-formatted Document via DIRECT Messaging from Patient Portal When your practice enables DIRECT transmissions from the portal, portal users may transmit CCDAformatted documents via DIRECT messaging. Note: Only CCDA-formatted documents may be transmitted via DIRECT. To transmit the document, the portal user selects a recipient from the practice s DIRECT network. The system transmits the document to the selected recipient via DIRECT messaging, and then stores that action as part of the Portal Activity History log. To transmit CCDA documents on the Patient Portal: 1. Log in to Patient Portal. 2. Click to view the desired document, either from the Home page or from the Documents and Lab Results window. 12
13 If the document is a CCDA-formatted document, and your practice enabled transmission from the portal in the meridianemr Admin settings, Download and Transmit buttons are available. 4. Click the Transmit button. A list of providers that are part of your practice s DIRECT messaging network is displayed. 5. Click the radio button next to the desired recipient. 6. Click Send. The system sends the CCDA-formatted document to the recipient and logs the action in the Portal Activity History log. 7. Click Cancel to close the Provider list. 8. Click Done to exit. 13
14 Portal Activity History Log Viewing an Activity History Log on Patient Portal Portal users have the ability to view a Portal Activity History Log of actions taken on the Patient Portal. From the Home page, click the View Portal Activity History button. The Activity History window opens. The log tracks the activity by date, event (which shows the item names and what action was taken), and the username of the person who was logged in when the action was taken. Click Done when you finish reviewing the information. 14
15 Read Receipt Confirming a Portal User Accessed an Image or Lab on Patient Portal After an image or lab report is in the Patient Portal, a meridianemr user may verify whether the portal user accessed the item. A new Portal Status field provides the following indicators for an item selected in the Images grid or Labs grid: If Portal Status check box is not checked, the item has never been on Patient Portal. If Portal Status check box is checked but white, the item has been marked for portal but it has not been accessed by a portal user. If Portal Status check box is checked and green, the item is on Patient Portal and has been viewed. Clicking on Accessed on Portal next to the check box will display information about when the item was accessed by the portal user. If your practice has removed the item from the portal, the check box will be red, and clicking on Accessed on Portal next to the check box will still display information about when the item was accessed by the portal user. Messages Between memr and Patient Portal You can exchange messages with portal users on Patient Portal, using the meridianemr Portal Messaging chart window. The system sorts messages into three categories, each on a separate tab: 15
16 Generic Messages, Appointment Messages, and Medication Refills Messages. Each item represents a message thread consisting of an original message plus all subsequent replies. To view a message thread: 1. Click the on the desired message. 2. Select Display Message Thread from the popup menu. Initiate Messages from meridianemr to Patient Portal In meridianemr s Portal Messaging chart window, you can initiate secure messages to the Patient Portal. To compose a new message: 1. Click the grid. 2. Select Compose Message from the popup menu. 3. In the Compose Message window, select a message category in the Type field, 4. Enter your message in the text area. 5. Click Send. The message displays on the patient s messages list in Patient Portal. 16
17 Confirming a Portal User Accessed a Message on Patient Portal After a message is sent to the Patient Portal, a meridianemr user may verify whether the portal user accessed the message. The Portal Messaging window contains a column titled Accessed on Portal to indicate if the message was accessed (even if the portal user has not sent a response). If the message has not been accessed a white box with not checkmark will display with the Not Accessed text If the message was viewed by the portal user, there will be a green box with a checkmark and the text will indicate Accessed on portal. 17
