Challene yourself and earn up to $75: $50 for completin a Total Health Assessment This online questionnaire ives you a picture of your overall health, plus an action plan that s based on your answers. $25 for completin an online healthy lifestyle proram Get help to reduce stress, quit smokin, lose weiht, and more. You can take a proram when it s convenient for you, at your own pace. Startin January 1, 2015, visit kp.or/feds to et started. There you ll find information and links to the Total Health Assessment and healthy lifestyle prorams. The rewards proram runs from January 1, 2015, throuh December 31, 2015, and is open to all Kaiser Permanente FEHB subscribers and their enrolled spouses who are 18 and older. You can take the Total Health Assessment as often as you d like and as many healthy lifestyle prorams as you d like, but you can only earn rewards for them once durin the reward period. You must complete the activities durin the calendar year. Rewards will be issued 4 to 6 weeks after you complete your activity. 6

DeltaCare USA With this dental HMO plan, you ll select a dentist from the DeltaCare USA network. There are no claim forms to fill out and no deductibles. Most preventive and dianostic services are available at little or no cost to you.
