To establish a development plan and objectives for the following year whereby the manager can maintain or increase effectiveness. To create and encourage continuous two-way communication. 1. RESPONSIBILITIES: Summarize major duties and responsibilities. 1

2. OBJECTIVES: List specific objectives for this performance appraisal period. The objectives are results which the manager intends to achieve. 3. RESULTS: Describe specific results derived from the objectives which are stated above. 2

For each factor listed below, indicate whether the manager's performance "meets or exceeds expectations", "needs improvement", or is "unacceptable". Write a comment for each rating. A.
