New For 2012 The Short Term Disability Insurance buy-up plan now has a 7 day elimination period for sickness or accident. The previous plan had a 14 day elimination period.

Voluntary Benefits Program Introduction Voluntary Benefits Solutions (VBS), a division of Gallagher Benefit Services, Inc., offers Voluntary Benefits to eligible full-time and part-time Office Depot associates. VBS specializes in the design, implementation and administration of voluntary benefit programs. All of the voluntary benefits are offered through VBS and their benefit and insurance providers, not Office Depot. The Voluntary Benefits Program includes discounted rates and favorable underwriting program features not usually available to associates or their family members individually. Benefits offered through the program do not replace any Office Depot-sponsored benefit plans and are 100% associate-paid. The benefits available offer additional financial protection with premiums paid through convenient after-tax payroll deductions for full-time associates. Full-time Associate Eligibility There is no waiting period for Auto and Home Insurance and Pet Insurance; application for coverage can be made anytime throughout the year. Enrollment in the Legal Plan must take place within the first 60 days of employment or during the Voluntary Benefits annual enrollment period. Short-Term Disability Buy-Up Insurance, Critical Illness Insurance and Hospital Confinement Indemnity Insurance have a 30-day waiting period and then coverage can only be applied for during the Voluntary Benefits annual enrollment period, which is generally held each year in October or November. 1

With the Voluntary Benefits Program, you have the option to choose the insurance coverage and services that work best for you. Full-time associates pay for Voluntary Benefits with after-tax payroll deductions and part-time associates pay by direct payments to the insurance companies. You can enroll by calling the Voluntary Benefits Call Center at 877-337-6807. On-site Meetings Corporate, and certain Supply Chain or BSD locations, depending on number of associates, will have VBS Benefits Representatives onsite to meet individually with associates. Watch for posters in your work location listing the dates, and check with your Manager to schedule an appointment with a VBS Benefits Representative to learn more about the Voluntary Benefits Program and enroll on the spot if you decide to purchase certain benefits. Employer-Paid Disability Program When you meet with a Benefits Representative one-on-one or you access the Voluntary Benefits Call Center, you will be able to verify your employer-paid Short-Term Disability coverage and learn how to increase your coverage through Trustmark Insurance Company for added protection. Automatic Renewal If you do not make any election changes during the Voluntary Benefits annual enrollment period, your current elections will continue for the following plan year. Short-Term Disability Buy-up Coverage that provides benefits in the event of a covered accident or sickness. What would happen if you couldn t work? Your Base Short-Term Disability coverage provided by Office Depot will help. If you re married, could your spouse pay all the bills with just one paycheck? What would happen to your family s future plans? If you are a single parent, how would you provide for your children? Even if you are single and have no children, how would you support yourself? If a disabling injury or illness stops you in your tracks, one of your most valuable assets, your income, could stop but your bills will go on. Base Coverage Provided by Office Depot Office Depot provides eligible full-time associates with Base Short-Term Disability coverage equal to 50% of your gross base monthly earnings (excluding overtime) from your primary occupation. Benefits begin after 7 days of total disability and are payable for up to 24 weeks. Buy-up Coverage for Additional Protection For additional protection, you may apply for the Short-Term Disability Insurance Buy-up Program. You may choose coverage up to the amount you re eligible for. Monthly benefits are paid directly to you, and you can use them however you wish. And after 90 days of total disability and while you continue to be disabled, you do not pay premiums for continued coverage. Benefits begin after 7 days of total disability and are payable for up to 6 months. Please read the policy to ensure you understand the program design. Important Notice for current Short-Term Disability Buy-up Policy Holders: The Short-Term Disability Insurance buy-up plan is changing for 2012. The new plan will have a 7 day elimination period for sickness or accident. The previous plan had a 14 day elimination period. You will have the option to keep your existing policy and/or increase the benefit OR replace your current policy with a 7 day elimination period policy. A benefit representative is available to discuss these options. 3

Short-Term Disability Buy-up Coverage that provides benefits in the event of a covered accident or sickness. Short-Term Disability Plan Feature Base Coverage Buy-up Coverage When Benefits Begin When Benefits End Portability How Disability is Defined Pre-existing Condition Limitation Benefit to You Office Depot provides base coverage equal to 50% of your gross base monthly earnings at no cost to you. For additional protection through Trustmark Insurance Company, not Office Depot, apply for voluntary coverage from $300 to $3,000 per month in $100 increments. Your buy-up coverage may not exceed 20% of your gross base monthly earnings and your total coverage (base plus buy-up coverage) may not exceed 70% of your gross base monthly earnings.* Benefits begin on the 8th day of disability. Benefits are payable for up to 6 months. Once you have applied, been approved for coverage for the Buy-up Program and payroll deductions have started, the Buy-up Program is yours to keep even if you change jobs or retire from Office Depot. Premiums and coverage remain the same unless you choose to adjust them. To be considered a covered disability under the Buy-up Program, an illness or injury (including pregnancy or childbirth after coverage has been in effect for 10 months) must prevent you from working at your job and must not be work-related (in New York, must not be covered under any state or federal worker s compensation, employer s liability, or occupational disease law). You must also be under a doctor s care for the condition and not be working at your place of employment for pay or benefits. After the Buy-up Program s effective date, in most states if you become disabled because of a pre-existing condition, the disability is not covered if it begins during the first 12 months after the Program s effective date. Pre-existing condition means a sickness or physical condition for which you were treated, received medical advice or had taken medicine within 12 months before the effective date, even if this plan replaces current disability coverage. * If you live in California, Hawaii, New Jersey, New York or Rhode Island, your short-term disability benefits are provided through mandatory state programs. Any benefit payable from your Office Depot base coverage may be offset by the benefit amount paid through your state s program. Your buy-up coverage amount can bring your total benefit up to 70% of your base monthly earnings. Short-Term Disability benefits are underwritten by Trustmark Insurance Company, Lake Forest, Illinois. Refer to Plan Series DI-902 and other optional riders for your state for exact terms. 4

Critical Illness Insurance Coverage that provides a lump sum benefit in the event of a covered illness. Critical Illness Insurance provides a lump-sum cash benefit upon the first diagnosis of a covered critical illness after the plan effective date. Covered critical illnesses are limited to the specific definitions found in the policy. Coverage is offered through Trustmark Insurance Company. Is critical illness coverage right for you? The 5-year survival rate for cancer has increased to more than 3 in 5, or 65%.* The survival rate of a heart attack is 67%, and a stroke is 87%** On average, the costs surrounding a cancer diagnosis are $146,000 per patient.*** Travel expenses for treatments away from your hometown Medical expenses not covered by insurance Adapting your home for special needs Household expenses if you couldn t work * American Cancer Society, 2009 ** Heart Disease and Stroke Statistics, 2005 Update, American Heart Association *** American Cancer Society, 2005 Critical Illness Insurance does not require a specific expense for payout. It is intended to help cover some of the expenses not covered by medical insurance, such as out-of-pocket deductibles and copays, child care, travel expenses and more. Pre-existing condition limitations and exclusions may apply and in some states, you must have comprehensive medical coverage in place before you purchase this coverage. Optional Features Along with the plan features shown on the facing page, you can also elect the following options: EZ Value Option for Critical Illness As a hedge against inflation, qualifying associates and family members can sign up for this option, which automatically increases coverage on each of the first five policy anniversaries. The example below shows a non-tobacco user with associate only coverage, age 30, $5 per week premium and initial lump sum benefit of $33,766. Covered tests are defined in the insurance policy. THIS IS A LIMITED, SPECIFIED DISEASE POLICY. Critical Illness Insurance including Cancer is underwritten by Trustmark Insurance Company, Lake Forest, Illinois. For complete details of coverage including exclusions and limitations, refer to Plan Form CACI-82001 and Rider Form HS-12000/R for your state. Please note that coverage in Connecticut may vary. Critical Illness is not available in Connecticut. 5

Critical Illness Insurance Coverage that provides a lump sum benefit in the event of a covered illness. Critical Illness Insurance Feature Coverage Options Benefit Amount Covered Critical Illnesses Benefit to You Associate only, associate and spouse, associate and child(ren), associate, spouse and child(ren). Benefit amounts from $5,000 to $100,000 (maximum amount not to exceed two times gross base salary) are available. One benefit amount applies to each family member. Heart attack, stroke, invasive cancer, renal (kidney) failure, blindness, ALS (Lou Gehrig s Disease), transplant of a major organ, paralysis of at least two limbs, carcinoma in situ (25% benefit*), coronary artery by-pass surgery (25% benefit*). Note: Most skin cancer is excluded. New York Heart attack, stroke, invasive cancer, renal (kidney) failure, ALS (Lou Gehrig s Disease), transplant of a major organ, carcinoma in situ (25% benefit), coronary artery obstruction (25% benefit), skin cancer benefit ($250), Reversible Ischemic Neurologic Deficit (RIND) ($250 benefit), Transient Ischemic Attack (TIA) $250 benefit). Pays for Expenses Not Paid by Medical Insurance Portability Pre-existing Condition Limitation Critical Illness insurance is intended to provide security for many expenses not covered by basic medical insurance such as: Family income replacement during recuperation Out-of-network treatment Medical insurance co-pays and deductibles Alterations to home or car Child care and housekeeping expenses Special transportation or housing expenses Nursing care and experimental treatment or drugs Once you have applied, been approved for coverage and payroll deductions have started, the plan is yours to keep even if you change jobs or retire from Office Depot. Premiums and coverage remain the same unless you choose to adjust them. No benefit will be paid for any condition caused by or resulting from a pre-existing condition which begins in the first 12 months after the coverage effective date. A pre-existing condition is a sickness or injury for which medical care, diagnosis or advice was received or recommended during the 12 months immediately prior to the coverage effective date. Consult your policy for exact terms. * If the insured receives the one time 25% benefit for carcinoma in situ or coronary artery by-pass surgery (New York: skin cancer, RIND, TIA, carcinoma in situ or coronary artery obstruction), the remaining 75% benefit will be available for a first diagnosis of another covered illness or condition. Coverage for an insured terminates once a full benefit has been paid. If multiple family members are covered and one receives the full benefit, premiums for the remaining insureds will be reduced. Children s premium does not reduce until all children have received the full benefit amount. 6

Hospital Confinement Indemnity Insurance What Is Hospital Confinement Indemnity Insurance? 1 Why Hospital Confinement Indemnity Insurance? A hospital stay even for a few days can be quite expensive. According to the American Hospital Association, in 2008, the average hospital expense, adjusted per inpatient stay, was $9,788.25 2. Even if you already have major medical health insurance, your entire hospital bill may not be covered. Do you have a deductible that must be met first? Coinsurance requirements? What about other out-ofpocket expenses major medical insurance was never intended to cover like transportation, child care, or household bills? 2 Hospital Statistics 2010 Edition, Health Forum LLC, an affiliate of the American Hospital Association. Benefits can be used to help with hospital stays and related expenses. Because major medical health insurance usually carries a separate hospital deductible, Aflac s insurance policy provides a hospital confinement benefit.
