Medicare Prescription Drug Coverage Deductible Initial Coverage Coverage Gap Catastrophic Coverage If your Rx plan has a deductible, you pay full retail cost until deductible is met 2016 $0 to $360 You pay your copays as set by your Rx plan. When the full cost of medications reaches $3310 you move to the Coverage Gap. You pay 45% of Brand Name and 58% of Generics until $4850 out-of-pocket is reached You pay $2.95 for Generics and $7.40 for Brand Name or 5% -- whichever is greater 20

Donut Hole Payment Medicare Prescription Drug Gap Benefit To be eligible for the Donut Hole special payment you must ben enrolled in a medical plan through OneExchange Each Medicare eligible participant is eligible for this payment Payment is taxable; you will receive a 1099-R Does not apply to Kaiser Permanente Senior Advantage 21

Donut Hole Payment Medicare Prescription Drug Gap Benefit Deductible Initial Coverage Coverage Gap Catastrophic Coverage DONUT HOLE Once you enter the Coverage Gap Stage, your Rx plan will provide you with an Explanation of Benefits. Attach and submit with claim form to OneExchange for a LLNS Special Payment up to a maximum of $2425. The amount you receive depends on the month you enter the Donut Hole. 22
