These services are generally covered by Medicare, Medicaid or other third party insurance providers. VNA Extended Home Care services are primarily provided to clients who do not meet strict medical necessity criteria. Payment is self pay, or third party long-term care insurance. Through contractual arrangement with Aging Services, Inc., VNA Extended Home Care services are also offered to those who meet non-medicare eligibility requirements.

VNA Hospice & Palliative Care Caring for a terminally ill loved one can be a difficult burden to shoulder alone. VNA s Hospice and Palliative Care services provide support, comfort and relief so patients and their loved ones can maximize their time together. Hospice originally defined a place where one could rest between travels. At VNA of Indiana County, hospice still defines a place of rest one where terminally ill patients can find help to maintain or enhance the quality of their remaining lives. While hospice exists neither to hasten nor to postpone death, it does work to affirm life by providing support for those in the final phase of an incurable disease or prolonged illness. Like hospice, palliative care is directed at providing relief to a terminally ill person but uses symptom management and pain management not to cure, but to provide comfort and to maintain the highest possible quality of life for as long as life remains. VNA Extended Home Care VNA Extended Home Care is designed for patients who need long-term care when a chronic condition, trauma, or illness limits their ability to carry out their own basic care needs. For many of these patients, care can be safely provided in the comfort and security of their own homes. Helping patients of all ages from infants to seniors maintain an independent lifestyle, VNA Extended Home Care provides skilled, private duty nursing in the home, hospital, or in a long-term care facility. VNA Extended Home Care services include: Nursing services Personal care Companion Homemaker/housekeepers In-house respite care Infant/new mother care Care management All of the above services are supervised and coordinated by a registered nurse, and clients/ patients choose whatever hours of services they need. VNA takes all of the worry out of finding the right kind of help because we screen and obtain references for all prospective caregivers, assuring our patients that they re in the good hands of qualified, compassionate caregivers. VNA Mom and Baby Club The VNA Mom and Baby Club was created to celebrate mom and her baby. The arrival of a newborn is a wonderful, magical time, but it can also be a stressful time when both mom and baby need extra time to adjust to their surroundings. A service of VNA Extended Home Care, the Mom and Baby Club provides home assistance to each mom and her baby (or babies!) and is flexible enough to meet the needs of the whole family. Some of the services provided by the Mom and Baby Club include: Basic Baby Care 101: including formula prep, feeding, burping, bathing and diapering Night care so moms can sleep RN checks for Mom and baby Mean planning and cooking Cleaning Laundry Shopping/errand services Child care for older children Breast feeding assistance The VNA Mom and Baby Club offers three levels of service so you can build your own package to meet your needs. You can even pre-arrange services several weeks prior to your due date. Patients may also purchase additional hourly visits and packages. VNA Geriatric Care Management Our population is aging at an exponential rate. In seven years, the first members of the baby boom generation will be eligible for Medicare. And of those surveyed by the American Association of Retired Persons (AARP), more than 80 percent of those age 50 or older want to stay in their own homes and never move. Drawing on our experience as Indiana County s largest nonprofit home care provider, the VNA has taken the lead to create a system to identify and refer older clients to a variety of resources they require, including long-term health care and residential, financial, emotional, and social services. Through innovative and cost effective programs, the VNA provides Geriatric Care Management services that encompass the entire continuum of care acute, chronic, long-term, and hospice and palliative care. With VNA as a partner, we can help our older adults limit and eliminate the discouragement and defeat they ve faced in the past when it comes to managing their on-going care. We re committed to caring for and enhancing the lives of our older adult population. The VNA Geriatric Care Management program was designed to meet the needs of today s emerging older population by providing quality, affordable home care that helps foster a healthy lifestyle and continued independence.
VNA Funds Development VNA is the largest nonprofit provider of long-term care in the communities we serve. In 2004, VNA conducted over 44,000 home visits to 3,000 patients. On any given day, more than 700 patients are on our caseload. Many are underinsured or have no insurance and no way to pay for care. The Funds Development division seeks to underwrite VNA s family of services when resources are limited or unavailable. The financial partnership of generous individuals, corporations, foundations, and government agencies allows VNA to ensure that thousands of critically ill and disabled patients of all ages receive care, regardless of ability to pay. How You Can Help Us Care Contributions are a way to honor someone, or to mark a special occasion. Donors may indicate preference for general support or support to a specific service or fund. Designating VNA in an estate plan is another important way to make a difference, with many advantages for the donor. Together, we can make a difference in our community by providing the financial resources that allow for compassionate care right at home. Giving something back to the VNA felt more than right, especially after what they did for our family. We ll never forget and no one else should either, because someday, we ll all need their care.
