Meaningful Use Registration Overview: Medicare Program Gather Information National Provider Identifier (NPI) National Plan and Provider Enumeration System (NPPES) User ID and Password Payee Tax Identification Number (if you are reassigning your benefits) Payee National Provider Identifier (NPI)(if you are reassigning your benefits) Verify Eligibility / Select Program Physicians can only register for Medicare or Medicaid incentives Before 2015, an eligible professional may switch programs only once after the first incentive payment is initiated Physicians may not be hospital based - seeing 90% or more of their Medicare covered services in: Hospital Inpatient setting (POS 21) or Emergency Department (POS 23) Register Register for the Medicare and/or Medicaid EHR Incentive Programs NOTE New York Medicaid Registration will not be open until 4 th Quarter 2011 NOTE Each physician will have to register separately and is legally responsible for information provided. NOTE Third party registration will become available NOTE You are only required to register once. NOTE- You do not need certified EHR technology requested.

To Register for the EHR Incentive Program Access CMS Registration/Attestation web portal https://ehrincentives.cms.gov Click Continue On Next Screen Click Continue again for login page to appear Click On Registration Link Click Continue

Proxy Reg Enter the NPPES User ID & Password User ID & Password are Case Sensitive Or Proxy User ID & Password 1) Contact the PECOS or NPPES Help desk if you cannot remember you password or login 1866-484-8049 (PECOS) Or 1800-465-3203 (NPPES) 2) Eligible Professionals (EPs) can authorize personnel as proxy s to register & attest on their behalf an I&A web account must be created for the user Click Log In

Jane Doe Welcome Screen Consist of five tabs to navigate the portal. Home Registration Attestation Status Account Management Select to begin registration process

Once you enter the registration you can select from five options Register Modify Cancel Reactivate Resubmit Select Register to begin process

Data required for this registration is grouped into three topics. All three must be completed. EHR Incentive Program Personal Information Business Address & Phone Click on Start Registration to continue

(Medicare) Select Incentive Program & Click Apply Select EP Type A certified EHR system is not required to register but will be required for attestation. Click Yes/No for EHR Click Save & Continue

(Medicaid) IMPORTANT: You cannot register for the Medicaid EHR Incentive Program Until Your State has Launched it s site. If your State does not appear in the Medicaid State/Territory drop down menu your state site has not launched. Click Save & Continue

SSN Payee TIN indicates that the provider receives the payment. EIN Payee TIN indicates the group receives the payment. Choose Group Name if selecting EIN (click apply) Enter Group NPI Select where payment will go in the Payee TIN Type & Click Apply Click Save& Continue IMPORTANT Medicaid EPs can elect to have payment sent to another qualified entity by selecting Payee TIN Type of EIN. This information will be sent to the State. There are rules around reassignments governing this program. Please see the CMS website for more information http://www.cms.gov/ehrincenti veprograms/

Review Business Address & Phone: Data is pulled from NPPES Address can be revised but is not sent to NPPES Business address cannot be a P.O. Box Information will be posted on the EHR website once you receive payment Click Save & Continue

Check marks indicate registration topics have been completed Click Begin Submission

1) Review and verify registration for accuracy 2) To make changes click on Previous Page to revise information. Topics for registration appear Click Modify to change your registration Click to Make Changes Click Submit

EHR Incentive Program If Disagree is selected the user is directed to the Registration Instructions Page. Select Modify to restart process Review Disclaimer select Agree Or Disagree

SUCCESSFUL SUBMISSION Important: Select Print. Print page for your records. Assigned Registration ID You will not receive an email confirmation Click Print

FAILED SUBMISSION Important: Select Print. Print page for your records. Assigned Registration ID You will not receive an email confirmation For failed submission, review Failed Submission Reasons and contact appropriate departments to correct. Click Print

Click Status Tab to Review Registration Status Click Select to view Details

Registration Status will appear as Active when all validations pass. Click Additional Information for registration status reason.

(Medicaid) Registration status will read PENDING STATE VALIDATION until the registration process is completed by the State. Other Registration Statuses: Pending State Approval In Progress Rejected Status reason is listed on the Additional Information page

EHR Incentive Program Help link is located on every screen and can provide additional assistance and vital contact information.

IMPORTANT: Eligible Professionals can switch between the Medicare and Medicaid EHR Incentives programs only once after receiving an incentive payment. The switch has to occur before 2015.

Questions-Registration

Medicare eligible professionals attest using the CMS' web-based Medicare and Medicaid EHR Incentive Program Registration and Attestation System. In the system, providers will fill in numerators and denominators for the meaningful use objectives and clinical quality measures, indicate if they qualify for exclusions to specific objectives, and legally attest that they have successfully demonstrated meaningful use. Medicaid EPs must prove only that they have adopted, implemented or upgraded certified EHR technology in the first year of participation they do not need to prove that they are meaningfully using certified technology until the second year of Meaningful Use. Medicaid Incentive Programs participants must attest using their states EHR Incentive Program site. The New York Medicaid EHR Incentive Program website will open Dec. 2011. Medicaid Incentive Program participants cannot use the CMS web-base portal to attest. EHR certification number is required to attest. Certifications numbers can be obtained from Certified Health IT Product List (CHPL). To obtain a CMS EHR Certification ID, follow the steps outlined below: 1 Select your practice type 2 Search for EHR product(s) 3 Add product(s) to cart and 4 (IP or Ambulatory) by Name determine if meet criteria Request CMS EHR Certification ID

Select Attest to begin process

When all topics are checked completed or N/A user can select PROCEED WITH ATTESTATION All areas must be complete to complete attestation Click Start Attestation to being process

EHR Certificate Number is Required for Attestation. This information must be entered. IMPORTANT: YOU MUST ENTER YOUR EHR CERTIFICATION NUMBER TO ATTEST. The EHR Certification Number is 15 characters long Reporting period must be 90 consecutive days Click Save & Continue

Process: 1) Select Appropriate option for Patient Records 2) Answer Yes or No to the Exclusion Question. If No is selected the screen will expand to enter the numerator & denominator for the measure (see below) 3) Click Save & Continue

You can log out at any time and continue your attestation later. All of the information entered up to this point will be saved within the attestation module.

Process: IMPORTANT: YOU MUST SELECT FIVE MENU MEASURES AT LEAST ONE MUST BE A PUBLIC HEALTH MEASURE 1) Select five by clicking the box immediately following the measure Menu: Public Health Measure Up to two public health measures can be chosen Menu: Non Public Health Measures Click Continue

Process: IMPORTANT: YOU MUST ATTEST TO ONE PUBLIC HEALTH MEASURE EVEN IF AN EXCLUSION APPLIES TO BOTH Click Save & Continue

Process: IMPORTANT: YOU MUST REPORT ON A MINIMUM OF 6 CQMs or a MAXIMUM OF 9 CQMs CHOOSE THREE FROM THE CORE OR ALTERNATE CORE. (If you report zeros on any of the core you must report on an alternate core even if that alternate core is zero. It is possible to report on all core & alternate core if all are zeros). & CHOOSE THREE FROM THE ADDITIONAL SET OF THIRTY- EIGHT Click Save & Continue

When all topics are marked as completed or N/A you may proceed with attestation. The next screen will allow you to review entries before final submission. Click Proceed with Attestation

Check the box next to each statement Click Agree to complete attestation If you click Disagree you will be moved back to the home page & your attestation will not be submitted Click Agree or Disagree Click Yes if you are sure you want to attest

If you click Disagree you will be moved back to the home page & your attestation will not be submitted If Agree is chosen and you have met all measures, you will receive the Accept Attestation submission receipt Click Agree or Disagree

Print receipt you will not receive an email confirmation

Rejected: You did not meet one or more of the measures. Check for errors & if a error was made modify and resubmit for same 90 day reporting period or submit for different 90 reporting period. The 90 day reporting period can be a day later (For Example 3/1-5/31 vs. 3/2-6/1) Please note: If different 90day is chosen EP may have to recalculate numerator/denominator Print receipt you will not receive an email confirmation

Process : Click Resubmit Click Proceed with Attestation New submission receipt will appear & disclaimers. 2) Click Proceed with Attestation 1) Click Resubmit

***THIRD PARTY PROXY REGISTRATION*** Third Party Proxy Process : Click Create a login Click Yes to access the I&A system

***THIRD PARTY PROXY REGISTRATION*** Third Party Proxy Process: The application and security check will prompt the requestor to answer security questions, create a profile, and user name and password. Please note: User IDs cannot be changed. Any secret questions you create will be permanently linked to your account. User IDs are case sensitive.

***THIRD PARTY PROXY REGISTRATION*** Third Party Proxy Process: Enter NPI for provider Click Search Click Save to got to My Access Request Page or Click Save & Add Another to multiple providers A proxy can register and attest for a maximum of 300 providers. Click Save or Save & Add Another

***THIRD PARTY PROXY REGISTRATION*** Third Party Proxy Process: Click each box for the NPI use wish to access. To select all, click Select All. Click Submit

***THIRD PARTY PROXY REGISTRATION*** Third Party Proxy Process: You will receive an email notification that the External User Services (EUS) Help Desk has approved your request Record your tracking number for use on any correspondence The EP must log into the I&A system and approve your request Notify the EP that you have requested access At this time there is not an automated email notification of the I&A system
