RIVERSIDE TRANSIT AGENCY FULL-TIME ADMINISTRATIVE EMPLOYEES NEW HIRE ENROLLMENT OVERVIEW 2015 Riverside Transit Agency (RTA) is extremely proud of the package of benefits available to you. The benefits package provided to you is an extremely valuable part of the total compensation you receive as an RTA employee, and thoughtful consideration should be given to your choices in order to achieve the greatest return from this opportunity. BENEFITS ELIGIBILITY EMPLOYEES All active full-time or part-time employees qualify for coverage under the benefit plans describe in this summary. Part-time employees should consult the Human Resources Department to determine the level of coverage for which they qualify and the amount of required contributions. Interns, temporary employees, and on-call employees are not eligible. DEPENDENTS Dependents eligible for coverage are defined as follows: Your legal spouse Your registered domestic partner with the California Secretary of State Medical Plans Married / unmarried children from birth to age 26 if they are natural born, adopted, stepchildren, or children for whom you are a court appointed legal guardian. Supporting documentation will be required. Dental and Vision Plans Unmarried children from birth to age 19 (up to age 23 for full-time students) if they are natural born, adopted, stepchildren, or children for whom you are a court appointed legal guardian. Supporting documentation will be required. Your eligible dependent children over age 19 (dental and/or vision) or over age 26 (medical), who are incapable of self-support due to a physical or mental disability. Documentation of dependent status may be requested. Failure to provide documentation may result in cancellation of coverage. COVERAGE EFFECTIVE DATE The effective date of coverage will be the 1 st of the month following 30 days of employment. To enroll in the benefits most appropriate for you, you will be required to complete the enrollment forms provided to you during your new hire benefits orientation. New employees do not need to wait for an Open Enrollment period; they are eligible for enrollment regardless of the date they begin employment, provided that they enroll within 30 days from their benefits effective date. Once enrolled, however, new employees become subject to the rules governing all employees, and no benefit changes can be made until the next Open Enrollment Period without a Qualifying Status Change event. It is extremely important; therefore, that new employees carefully consider the wide array of options available before making final coverage choices. ENROLLMENT DEADLINE If you are a newly-hired full-time or part-time employee, you have 30 days from your benefits effective date to enroll in the benefits plan in which you are eligible.

MEDICAL INSURANCE The Riverside Transit Agency pays for the full-time employee health premium up to the lowest HMO medical premium in CalPERS based on employee-only rate in Riverside County and contributes $420 towards the dependents monthly premiums. PPO coverage is offered; however, RTA will pay up to the lowest HMO as specified above. Please refer to the attached rate sheet for a list of health plans and rates currently being offered. IN-LIEU-OF MEDICAL CONTRIBUTION The Riverside Transit Agency gives you the option to waive your medical insurance coverage and elect an RTA contribution of $110 per month to your 457 deferred compensation account in lieu of medical coverage for yourself. You will need to provide proof of other coverage. You may also be eligible for an additional $110 per month if you have eligible dependents that you do not wish to cover on the medical insurance. Proof of dependent status will be required (i.e. Marriage certificate and/or birth certificate). You may elect to enroll in our medical plan and waive medical coverage for your eligible dependents. The maximum monthly amount that you can receive is $220 ($110 for yourself and $110 for dependents). RETIREE MEDICAL The Riverside Transit Agency provides eligible retirees and their dependents medical benefits as follows: Employees hired on or before June 22, 2006, are age 50 and have five (5) years of service credit with CalPERS, are eligible for: o Contribution up to lowest HMO retiree medical coverage in CalPERS based on employee only rate in Riverside County. PPO available at additional cost. o Contribution toward dependent medical coverage. Employees hired after June 22, 2006, are age 50 and have ten (10) years of active full-time RTA service are eligible for: o Contribution up to lowest HMO retiree medical coverage in CalPERS based on employee only rate in Riverside County. PPO available at additional cost. DENTAL INSURANCE The Riverside Transit Agency pays for the full-time employee only HMO dental premium. Metlife Dental Plan is the current carrier. PPO coverage is offered; however, RTA will pay up to the HMO employee only premium. Please refer to the attached rate sheet for rates. VISION INSURANCE The Riverside Transit Agency does not contribute toward the premium. Blue View Vision Plan is the current carrier in which you can treat with their providers or a provider outside of their network. Please refer to the attached rate sheet for rates. LIFE INSURANCE The Riverside Transit Agency provides company paid life insurance to you in the amount of two times (2x) annual basic earnings up to the maximum of $350,000. SUPPLEMENTAL LIFE INSURANCE Supplemental life insurance is available from Standard Life at your own expense for yourself, spouse, and children. Employee optional life - coverage can be purchased up to 5 times your covered annual earnings, not to exceed $250,000. New hires may elect up to $200,000 without medical underwriting. Spouse optional life - coverage can be purchased up to $250,000, not to exceed 100% of the employee optional term coverage amount. New hires can elect up to $20,000 spouse life without medical underwriting.

Children optional life- coverage can be purchased up to $10,000, not to exceed 100% of the employee optional term coverage amount. New hires can elect child life without medical underwriting. AD&D LIFE INSURANCE Accidental death and dismemberment life insurance is available from Standard Life at your own expense for yourself, spouse, and children. Coverage can be purchased up to $350,000 without medical underwriting. AFLAC INSURANCE You may elect two voluntary plans from Aflac at your own expense for yourself, spouse and children. The Group Accident Plan- pays a benefit for the treatment of injuries suffered as the result of a covered accident outside of work. The Group Critical Illness Plan- provides a lump-sum benefit following the diagnosis of specific covered critical illnesses. New hires may elect up to $20,000 without medical underwriting. Pre-existing condition provisions still apply. LONG TERM DISABILITY The Riverside Transit Agency provides long term disability coverage at no cost to you through Standard Life Insurance. The policy pays 66.67% of your monthly earnings in the event that you are disabled more than 90 days. PAID TIME OFF You are entitled to the following paid time off: Holidays eight (8) fixed holidays and five (5) floating holidays. There is no waiting period to be eligible for the fixed holiday pay. Vacation 10 days of paid vacation in the first two years of employment up to a maximum of 30 days at 20 years and after. Sick leave You will earn eight (8) hours of sick leave for each month s continuous employment. You will have the option twice a year to cash out your unused sick time by putting the money into your deferred compensation plan or receiving the money in cash. Sick time can also be converted to service credit for retirement purposes. Otherwise, your sick balance can be carried over year to year up to a maximum of 1040 hours. Leave of Absence o Court Service (including jury duty) will be paid up to maximum of 10 workdays per calendar year. o Bereavement Leave Up to four (4) days will be paid. o the state and five (5) when death occurs out of the state. RETIREMENT The Riverside Transit Agency contracts with the California Public Employees Retirement System (CalPERS) to administer the retirement benefits. The retirement benefits provide for: 2% at 62 this is the current benefit factor formula, in addition to your age at the time of retirement, used in determining the percent of pay to which you are entitled to for each year of service. The employee contribution rate is 6.25%. New hires with previous CalPERS membership or membership with another employer who has established reciprocity with CalPERS should consult the Human Resources Department to determine if they are eligible to be in the 2% at 55 benefit factor formulas. Service retirement eligible at age 52 with five (5) years of CalPERS service credit. New hires with previous CalPERS membership or membership with another employer who has established reciprocity with CalPERS should consult the Human Resources Department to determine if they are eligible at age 50 with five (5) years of CalPERS service credit. Disability retirement if you have a disabling injury or illness that prevents you from performing your job duties, you may be eligible for this benefit. You must have a minimum of five (5) years of CalPERS service credit to be considered.

Survivor and Death Benefit may be payable to the named beneficiary upon the death of an active or retired CalPERS member. There is no employee and employer contribution to Social Security due to enrollment in the CalPERS retirement benefits. All employees are responsible for the CalPERS employee contribution rate of 6.25% of base pay. Eligible new hires with previous CalPERS membership or membership with another employer who has established reciprocity with CalPERS are responsible for the CalPERS employee contribution rate of 7% of base pay. 457 DEFERRED COMPENSATION This is a tax-savings retirement account offered through Great West in which you can make pre-tax contributions up to $18,000 (IRS limit for 2015) if under age 50; and, up to $24,000 per year if over age 50. 401A PLAN A third retirement plan is available through the Great West in which the Riverside Transit Agency will contribute $25 per month to this account. Depending on the administrative group in which you belong to, your contribution will be either 5% or $25 per month. These are pre-tax contributions. The election to participate in this retirement plan is irrevocable. Once enrolled, you cannot waive at a future date. OTHER BENEFITS Employee Assistance Program (EAP) confidential counseling available for all employees and their immediate family members. Section 125- All deductions for the medical, dental and vision insurance are taken pre-taxed.
