Introduction For more than 65 years, Kaiser Permanente's social mission has been the foundation of its community service programs. Through partnerships with community organizations and government entities, Kaiser Permanente strives to benefit the community by addressing issues and concerns that affect overall community health. Kaiser Permanente engages the community and brings a variety of resources to these partnerships including, cash grants to support community activities, volunteers, and in-kind donations. Community engagement is how we strengthen our relationships in the community to better understand and support community needs in a meaningful way, and improve the health of the communities we serve. Each year through our grant programs, we support local organizations that are working to improve the health of the communities we serve. The East Bay Area Community Benefit grants program provides funding and support to local nonprofit organizations addressing the following priority health needs identified in the 2013 Community Health Needs Assessment (CHNA). Health Priority Needs Access to Care - Increase the number of low income people who have access to appropriate health care services in our East Bay area Increase access to, enrollment in, and maintenance of health care coverage Increase access to specialized, culturally appropriate care Healthy Eating, Active Living - Reduce the number of overweight and obese children, adolescents, and adults in low income our East Bay communities Increase healthy eating among children and families Increase physical activity in schools, community and institutional settings Violence Prevention - Reduce the number of adolescents, young adults, and people of color exposed to violence, including witnesses, survivors and perpetrators Create and maintain safe environments in schools, residential neighborhoods, and workplace settings Increase skills building and employment opportunities for high risk youth Increase access to services that identify, address and prevent domestic violence Increase access to trauma informed, mental health services and training Asthma Management and Prevention - Reduce asthma episodes among high risk children and adolescents residing in West Contra Costa County (West Contra Costa area focus) Improve asthma management among high risk asthma sufferers emphasizing environmental impacts and policies Improve asthma management among high risk children and adolescents through behavioral and clinical strategies www.kp.org/communitybenefit/ncal
Grant Programs Overview Initiative-Based Grants Program Our initiative-based grants are an opportunity to support existing community-based programs that are aligned with the priority health needs and implementation strategies identified through our 2013 Community Health Needs Assessment, with emphasis placed on efforts that reflect collaboration and demonstrate program evaluation capacity. Organizations will be invited by Community Benefit staff to submit a grant application by invitation only for this program. Grants will range from $20,000 to $30,000 and will be awarded for a 12-month program period. Only previously funded organizations will be considered for this program. 2015 Initiative Based Grants Timeline Information on this program Full grant applications will and application materials be due by April 20 th at 5pm will be released by Wednesday, March 11 th Funding decisions will be made and awarded by June 30 th Grant Period: July 1 st, 2015 through June 30 th, 2016 Mini-Grants Program Our mini-grants create an opportunity to establish new community partnerships and support innovative projects that are aligned with select priority health needs and implementation strategies from our 2013 Community Health Needs Assessment. Eligible organizations can submit a two page Letter of Inquiry (LOI) for $5,000 to $10,000 awards for the strategies outlined below. Please note that only one LOI can be submitted per organization (please choose which need your program is aligned with). Access to Care: Increase access to specialized, culturally appropriate care by Supporting Federally Qualified Health Centers, community health centers, and free clinics to provide culturally responsive health care to vulnerable populations. Training and supporting patient navigators in the provision of culturally sensitive assistance, care-coordination, and guiding patients through available medical, insurance, and social support systems. Healthy Eating/ Active Living: Increase healthy eating among children and families by Ensuring strong nutritional foods and beverages standards and policy implementation, food literacy skills development, and nutrition education in schools, preschools, and childcare settings. (West Contra Costa) Promoting adoption of health-promoting food and beverage retailing and distribution policies and programs. (Alameda)
Violence Prevention: Reduce the number of adolescents, young adults, and people of color exposed to violence including witnesses, survivors and perpetrators. Create and maintain safe environments in schools, residential neighborhoods, and workplace settings by Supporting comprehensive child development programs that enhance the cognitive and social development of pre-kindergarten, low-income children. (Alameda) Increase skills building and employment opportunities for high risk youth Supporting youth leadership development, entrepreneurship, and skills-building programs. Asthma Management (Specific to West Contra Costa): Improve asthma management among high risk asthma sufferers emphasizing environmental impacts and policies by Reducing exposures to home-based environmental triggers (i.e. allergens, irritants) by implementing multicomponent interventions with an environmental focus. Establishing policies and procedures to support children with asthma in schools (including pre-school), including maintaining asthma action plans at school and making medications available. 2015 Mini-Grants Timeline Information on this program and LOI materials will be released by Monday, March 23 rd Letters of Inquiry will be accepted from through Monday, March 23 rd through Friday, April 3 rd If your LOI is approved for consideration full grant applications will be due by June 1st Funding decisions will be made and awarded by June 30 th Grant Period: July 1 st, 2015 through June 30 th, 2016 Note: Organizations can not apply for both a mini-grant and an initiative-based grant.
Application Process for Mini-Grants To be considered for mini-grant funding, please answer the following questions and submit your responses on your official letterhead. Do not exceed 2 pages. Submit your Letter of Inquiry (LOI) via email to Public-Affairs-Community-Benefits@kp.org by April 3 rd, 2015, 5pm. 1. Describe the community health issue that your organization is addressing including root causes or underlying factors you are trying to address. 2. Brief description of your organization s background, including relevant experience and collaborating partners. 3. Brief description of the range of programs/services offered by your organization 4. Brief description of the proposed program, including key objectives, activities, and outcomes. 5. Describe your target population in detail (age, race, ethnicity, gender and group characteristics such as, teen parents, homeless, low income, etc.) 6. Describe how your program aligns with our funding strategies. 7. Describe your major community partners and how they will contribute to the success of your program. 8. Describe your evaluation methods as they relate to the program objectives/outcomes. Guiding Principles Priority will be given to proposals that do one or more of the following: Alignment with Kaiser Permanente s mission of prevention and community health priorities Use of evidence-based prevention strategies that: 1. Address the reduction of health disparities 2. Address root causes of the identified health disparities 3. Impact change at multiple levels, including individual behavior, public policy, and the physical and social environment Strength of intervention or program and demonstrated efficacy on the population being served Organizational capacity to implement and/or sustain the program over time Evidence of sustained collaboration with community partners doing similar work Evidence of strong programmatic evaluation capacity In-kind Contributions To the extent that we can, we are looking for additional partnership/engagement opportunities with our community partners. Please include in your Letter of Inquiry information on additional nonmonetary resources (in-kind contributions) that would be beneficial to the program which you are seeking funding for including technical assistance, clinical expertise, furniture or medical equipment, and/or health education resources.

Eligibility Guidelines When submitting a Letter of Inquiry/Grant request to the East Bay Area Public Affairs Department, please ensure that following criteria is met: 1. Kaiser Permanente will consider requests from qualified organizations that serve our East Bay Area only 2. Only nonprofit or public benefit organizations are eligible to receive funding. Unincorporated groups or agencies can make arrangements to utilize an eligible nonprofit organization as a fiscal sponsor 3. Funds must be used to serve vulnerable populations within the East Bay Area: Oakland, Alameda, Albany, Berkeley, Emeryville, Piedmont, El Cerrito, El Sobrante, Hercules, Pinole, Richmond, and San Pablo 4. Strategies must be consistent with the values and mission of Kaiser Permanente, and address the priority needs and strategies identified through the 2013 Community Health Needs Assessment 5. Only one Letter of Inquiry for funding will be considered per nonprofit for either program * For school-based programs, preference will be given to programs located within public schools/districts. ** For faith-based organizations; we will only accept applications for funding from inter-faith collaboratives of three or more faith-based organizations that are providing services to the general public. A community nonprofit partner with an eligible 501c3 IRS status can apply for funding as a fiscal sponsor on behalf of the inter-faith collaborative.
Required Documents If invited to submit a grant proposal the following additional attachments are required. 1. Tax exempt status letter 2. Request/cover letter on letterhead stating organizations legal name, address, requested amount, project summary and project title 3. Current list of Board of Directors and their affiliations 4. Completed Project Budget Template 5. Completed Workplan Template 6. Most recent Form 990 (Requests over $20,000) 7. Most recent audited financial reports (Requests over $20,000) 8. Current itemized organization operating budget 9. Key staff/volunteer list - names, titles, and qualifications of key staff/volunteers for the project If applicable 10. Memorandum of Agreement with fiscal agent 11. Letter of support for school-based projects 12. Formal attestation for faith-based organizations Fiscal Agent If a fiscal agent is being used, then the request and required information must also be submitted on the fiscal agent s letterhead along with the copy of the Memorandum of Agreement. In addition, all required documentation must be submitted for the fiscal agent organization. Funding Limitations Kaiser Permanente s East Bay Area Public Affairs Department will not consider funding requests for: political campaigns*, contributions to endowments or memorials, emergency loans, athletic programs and teams, field trips, religious purposes**, or personal requests such as scholarships, individual tuition, educational purposes, conferences, etc. *As a nonprofit organization, Kaiser Permanente is legally prohibited from funding political campaigns. ** Kaiser Permanente may fund faith organizations providing secular programs that serve the greater community and are consistent with our Community Health Priorities. A community nonprofit partner with an eligible 501(c)3 IRS status can apply for funding as a fiscal sponsor on behalf of an inter-faith collaborative.
