In addition to the core benefits Carolinas HealthCare System offers, such as medical, dental and vision coverage, you also have access to voluntary benefits. These voluntary benefits are designed to help protect your lifestyle and your family s future. Our voluntary benefit programs provide you and your family: A broad choice of plans that can be tailored to meet your needs The benefit of group rates and discounts Convenient access to Carolinas HealthCare System approved vendors, products and services This brochure provides a high-level overview of the voluntary benefits available to you and your family. HOW TO ENROLL After you have completed your core benefits enrollment on PeopleConnect > YourHRLink, you will see a link to enroll in the additional voluntary benefits described in this brochure. For detailed instructions on how to enroll, refer to the enclosed 2014 Benefits Enrollment Instructions sheet. ELIGIBILITY Voluntary benefits are available to teammates with standard hours of 16 or more per week. You can enroll in these plans: If you are a new teammate, or if you become benefits eligible due to a change in standard hours. You must enroll within 31 days of the first of the month coinciding with or following 90 days of employment or change in standard hours If you experience a change in family status (for example, marriage, birth of a child, divorce). You must make changes within 31 days of the family event During Open Enrollment IMPORTANT INFORMATION ABOUT OUR NEW VOLUNTARY BENEFITS PLATFORM With our new Voluntary Benefits platform, many current plans are being replaced with new products and will no longer be available through payroll deduction. You will need to decide whether to enroll in the new plans or continue your current coverage through self-bill. 1

DEPENDENT BACK-UP CARE PROGRAM The Dependent Back-Up Care Program helps you balance the demands of life and work while caring for a dependent. Do you have other care available if an emergency arises or regular care arrangements are not available? You can use this Back-Up Care Program to arrange care for a child or dependent adult. Here are just a few examples of times when the program could help you: Evening and weekend in-home care School holidays or weather emergencies Caregiver vacations or illnesses Business travel In-home care after minor surgery The program provides access to: Up to 15 days of care Savings on care expenses Access to trained care consultants LOW ANNUAL FEE Get the help and peace of mind you are looking for with the Dependent Back-Up Care Program. Teammates only pay a low-cost premium, $5 per pay biweekly/$10.83 per pay monthly, and a discounted rate on in-home care based on the number of dependents who require care. I would not have been able to come to work that day without the help of the Dependent Back-Up Care Program! - Ronesia Jefferies Ronesia Jefferies, Patient Care Coordinator Women s Institute, Reproductive Endocrinology and Infertility Ronesia s Success Story! When Ronesia s regular child care provider was short-staffed, they asked parents to try to make other arrangements for their children in order to keep the teacher/child ratio within the guidelines required by the state. Ronesia did not have any other options, except to call off of work and stay at home with her daughter. Ronesia realized she could call the Dependent Back-Up Care Program for assistance, and that is exactly what she did. Ronesia described the service as, very easy, fast, convenient, stress-free and inexpensive. It allowed Ronesia to continue her normal work schedule. Ronesia s Recommendations If you still have questions or need further assistance after reading the information that is available online, Ronesia suggests speaking with someone in customer service. The person that helped me was very kind and patient. She answered all of my questions without acting as if I was bothering her...and I sure had a lot of questions! 2

OPTIONAL GROUP TERM LIFE INSURANCE / ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) Life insurance offers financial protection to your loved ones if something happens to you. It pays a benefit that can allow your family to continue their way of life. In addition to any group life insurance that Carolinas HealthCare System may provide to eligible teammates, we offer you the opportunity to purchase Group Term Life Insurance so that you may feel confident in knowing that your family is protected against an unforeseen death. Also available is Accidental Death and Dismemberment (AD&D) Insurance. AD&D pays a benefit to your beneficiary upon your death, or a benefit to you in the event of a loss due to certain accidental injuries. Term Life Insurance and AD&D benefits are also available for your spouse and children. Optional Group Term Life Insurance Teammate Spouse Child(ren) During the Open Enrollment period, you may enroll in or increase your coverage by one increment (with no health questions asked, subject to plan guidelines up to $400,000; for spouse, up to $50,000) 1x to 5x your base annual earnings up to $1,000,000 Up to $100,000 (in $10,000 increments) Up to $10,000 (in $2,000 increments) Group Accidental Death & Dismemberment (AD&D) Teammate Spouse Child(ren) During the Open Enrollment period, you may enroll in or increase coverage with no health questions asked 1x to 5x your base annual earnings up to $1,000,000 Up to $100,000 (in $10,000 increments) Up to $10,000 (in $2,000 increments) WHOLE LIFE INSURANCE Can your family maintain their lifestyle without you? Whole Life Insurance helps ensure they can. Whole Life Insurance is an individual insurance policy designed to provide a death benefit to your beneficiaries if something should happen to you. It can also build cash value that you can utilize while you are still living. At an affordable premium, you can have the added financial protection you and your family may need during times of uncertainty. If you already have life insurance, you may want to consider whether your current coverage will meet your family s needs. Plan features: You can purchase coverage for yourself, your spouse, your children and/or your grandchildren Whole Life Insurance is voluntary, which means you purchase the precise amount of coverage that is right for your needs The benefit is Guaranteed Issue, so no physical exams are required to apply for coverage up to a certain amount As the policy builds cash value, you can eventually use it to make premium payments or to pay urgent expenses while you are still living Coverage is portable you can take your policy with you if you change jobs or retire Choose a Plan for a Lifetime Whole Life Insurance is known as permanent insurance, because it never expires. You keep the policy as long as you make the payments, which means the premiums will not go up. ACCIDENT AND CRITICAL ILLNESS INSURANCE Complements to Your Health Plan Accident and Critical Illness Insurance may help absorb the deductibles and other out-of-pocket costs that arise when a severe illness or injury strikes. NEW! ACCIDENT INSURANCE Accidents happen. You cannot always prevent them, but you can take steps to reduce the financial impact. Accident Insurance is designed to help cover the out-of-pocket medical expenses and extra bills that can follow an accident. The plan pays benefits for injuries and accident-related expenses, such as fractures and dislocations, hospitalization, physical therapy, emergency room treatment, transportation, lodging and more. The policy pays a benefit directly to you not a doctor or hospital and you can use the money however you choose. Benefit amounts are based on the type of injury and treatment needed. Why do I need accident insurance? Nearly 40% of self-reported episodes of injury, leading to hospitalization occurred during sports or leisure activities, and 44% occurred in or around the home* Falls are the leading cause of inpatient and outpatient care in emergency rooms, outpatient clinics and doctors offices* Injuries due to traffic accidents, overexertion and strenuous movements and striking against or being struck accidentally by objects, also make up a big percentage of injuries* Premiums set-up based on your pay cycle. * National Center for Health Statistics NEW! CRITICAL ILLNESS INSURANCE The out-of-pocket costs of a serious illness can be catastrophic, even with medical insurance. Critical Illness Insurance helps provide financial protection in the event of a covered serious illness. The policy pays a lump sum benefit directly to you if you are diagnosed with a covered condition. You can use this benefit any way you choose deductibles and coinsurance, expenses your family incurs to be by your side or simply to replace lost earnings from being out of work. You choose the benefit amount when you enroll. Children are covered at NO COST for 25% of the benefit amount you select A Wellness Benefit is included for you and your spouse, which pays an annual benefit if a covered health screening test is performed (blood tests, stress tests, colonoscopies, chest X-rays, mammograms, etc.) Coverage is portable you can take your policy with you if you change jobs or retire The policies and their provisions may vary or be unavailable in some states. SHORT TERM DISABILITY (STD) If a disability kept you out of work and prevented you from earning a paycheck, how would you cover your bills and other household expenses? Short Term Disability helps replace a portion of your income if a covered off-the-job injury or illness keeps you out of work. The benefit can help cover out-of-pocket expenses and make up for lost wages during a time of need. After you are declared disabled, STD can provide you with a benefit from 30% to 60% of your salary, depending on the state in which you live, up to a maximum of $3,000 per month. You select the coverage level that best fits your needs. The voluntary Short Term Disability plan is available to any teammate with standard hours of 16-29 (and to physicians with standard hours of 30 or more), who are not eligible for the Short Term Disability plan provided by Carolinas HealthCare System. LONG TERM CARE Long Term Care Insurance is something to consider as you prepare for the future, and is specifically intended to protect your income and your assets in the event that you need long term care. It provides care when, because of a severe injury or chronic illness, you cannot perform basic activities of daily living without help. Long Term Care offers benefits for services including: Home health care Assisted living Nursing home care Adult day care You can purchase a policy for yourself and your spouse. It is more important now, than ever before, to protect your identity. LEGAL PLAN Affordable legal assistance can sometimes be difficult to find. With the Legal Plan, you have access to comprehensive legal assistance, advice and discounted representation on many different legal services, including divorce, child custody, traffic tickets, wills and much more! The cost is only $7.36 biweekly (or $15.95 monthly), which covers you and your eligible dependents. AUTO/HOME INSURANCE You have access to policies for your auto, home and other personal insurance needs. Additional discounts, such as tenure, multi-car, multi-policy, garaging and more, may also be available. You have the added convenience of paying your premiums through payroll deduction.
