5 Your CHS LiveWELL Health Plan [2/3] Through the CHS LiveWELL Health Plan, your medical and prescription drug coverage consist of a single plan for 2015: a consumer-directed health plan with a health savings account (HSA). Key features of the CHS LiveWELL Health Plan: Preventive care is covered at 100% Pays benefits in the form of coinsurance once you have met the plan s annual deductible Out-of-pocket limits protect you from significant out-of-pocket costs You may see any provider you want, however, the plan pays more when Carolinas HealthCare System and network providers are used You have six tiers of coverage to choose from: Teammate Only Family Plans: Teammate + Spouse Teammate + Working Spouse Teammate + Child(ren) Teammate, Spouse and Child(ren) Teammate, Working Spouse and Child(ren) We offer a consumer-directed health plan with a health savings account (HSA). Health Plan Coverage Your Enrollment Checklist Review all benefits enrollment information Enroll in your benefits. Remember your voluntary benefits are available to complement your core benefits. Consider increasing your CHS Retirement Plan 401(k) contributions at retirement or call Retirement Plan Services Who Is Eligible? Teammates working 24 or more standard hours per week. Dependent Eligibility Eligible dependents include your: Spouse Children up to age 26 Unmarried children of any age who are incapable of self-support due to a mental or physical disability, which began prior to age 26, and who are primarily dependent upon you (contact Carolinas HealthCare System Benefits Administration for required disability certification forms) Dependent Verification Documentation is required to verify eligibility of all dependents being enrolled in the medical, dental and vision plans. It is considered fraudulent to cover non-eligible individuals on your plans. Making Changes to Your Benefits You can only make changes to your benefit elections upon new eligibility or when you have a qualifying family change or during Open Enrollment. A qualifying family change includes: Marriage, annulment, divorce or death of a spouse Birth, adoption or death of a child Recognition of marriage Employment change for you, your spouse or a dependent child that results in a loss or gain of healthcare coverage or other loss of health plan coverage Child loss of eligibility Enrollment or removal from daycare, which allows changes to your Dependent Daycare Flexible Spending Account If you experience a qualifying family change during the year, be sure to contact Benefits Administration within 31 days of the change. The benefits changes you make must be consistent with your family change.

6 CHS LiveWELL Health Plan CHS LiveWELL Health Plan offers you control! When you enroll, you take responsibility for your healthcare costs. As a consumer-directed (high-deductible) plan, it allows you to explore your options of care select what is best for you while at the same time investing in your personal health savings account (HSA). Health Savings Account (HSA) An HSA is like a personal savings account for your healthcare expenses that you own and it s all taxfree. You own the account and decide how much to contribute up to the IRS limits. To help your savings grow even faster, Carolinas HealthCare System makes an annual contribution and matches your contribution (based on your level of coverage and specific dollar maximums determined each year). Here is a detailed look at how the CHS LiveWELL Health Plan option with an HSA can benefit you and your family: Enroll in CHS LiveWELL Health Plan Eligibility to Participate To participate in an HSA, you must be enrolled in a high-deductible consumerdirected health plan not covered by other health insurance, Medicare or TRICARE, and you cannot be claimed as a dependent on someone else s tax return. Preventive services: You receive care Plan pays 100% Teammate pays 0% AND Invest in your HSA: 1. Teammate Pre-tax premiums Pre-tax HSA contributions 2. CHS LiveWELL incentive next year Annual contribution This HSA money is yours and rolls over from year to year. Teammates working 30 or more standard hours and earning less than $30,000 in annual base salary pay lower premiums and receive an additional $100 HSA contribution. Matching contribution Rx* and healthcare services before deductible is reached: Teammate pays 100% of billed rate Plan pays 0% Rx* and healthcare services after deductible is reached: Teammate pays 20%-30% Plan pays 70%-80% If you have an outpatient or inpatient event, you will likely reach your out-of-pocket maximum. Even if you have a catastrophic event, you only pay until you reach your out-of-pocket maximum, then the plan pays 100%. 8 Get Informed with Your Benefits Tools and Resources Carolinas HealthCare System is providing you with a number of tools and resources to help you learn about the CHS LiveWELL Health Plan including: Special Opportunity for Teammates Earning Less Than $30,000 Teammates working 30 or more standard hours a week and earning less than $30,000 will receive additional HSA contributions of $100. The premiums for those teammates will also be reduced. Reaching Your Deductible and Out-of-Pocket Maximum The family deductible must be met by one or more enrolled family members before any individual is considered to have met their deductible. Your deductible, copays and coinsurance, including prescription medications, all apply to your out-ofpocket maximum. 9 Your Health Savings Account The health savings account (HSA) is designed to help you save for current and future healthcare-related expenses, such as your deductible, coinsurance, dental and vision expenses or prescription drugs. The HSA is a savings account set up in your name at Branch Bank & Trust (BB&T) that is funded by Carolinas HealthCare System and you, if you choose. Your Contributions You can contribute to your HSA on a pre-tax basis through convenient payroll contributions or on an after tax basis if you choose. You can make contributions to your account as long as the total contributions to your account for 2015 do not exceed $3,350 for Teammate Only coverage and $6,650 for all other coverage levels. These limits include contributions from you and Carolinas HealthCare System, so be sure to consider all contributions made by the System (annual contribution, matching contributions and CHS LiveWELL Incentive) before you elect how much you want to contribute. Unlike a traditional flexible spending account (FSA), you may change your HSA elections at any time. Federal rules also allow catch-up contributions to an HSA if you are age 55 or older, or you will turn 55 any time during For 2015, you can contribute an additional $1,000 to your account if you are eligible for the catch-up contribution. Contributions from Carolinas HealthCare System to Your Account To help you pay for your eligible healthcare expenses and save for future expenses, Carolinas HealthCare System makes annual and matching contributions to your HSA. In addition, any CHS LiveWELL Incentives you earn in 2015 will be deposited as a lump sum to the HSA in Annual Contribution To help fund your account, you will receive an annual contribution of: $100 for Teammate Only coverage $350 for all other Family coverage levels This annual contribution is deposited into your account the pay period following the pay period in which your coverage becomes effective, so you have dollars right away to spend on your eligible healthcare expenses. Matching Contribution If you choose to contribute to your HSA, Carolinas HealthCare System will make matching contributions up to: $250 for Teammate Only coverage $750 for all other Family coverage levels These matching contributions are made in installments throughout the year based on the contributions you make. CHS LiveWELL Incentive HSA Contribution CHS LiveWELL is one of the many investments Carolinas HealthCare System makes in your health by providing programs and resources that help make living well a reality. The maximum incentive for 2015 will be $750, which is consistent with national averages for programs of this type. This will allow us to continue supporting your good health while being mindful of healthcare costs for the System. If you earn the CHS LiveWELL incentive in 2015, up to $750 will be deposited into your HSA no later than the second regular paycheck of Your HSA Dollars Can Grow If you go several years without a significant inpatient or outpatient event, you can accumulate thousands of dollars in your HSA and pay less each year in out-of-pocket costs. Electing Voluntary Accident and Critical Illness insurance plan coverage can also help protect you from unexpected expenses. [6/7] Health Plan Coverage

10 Your Cost for Healthcare Coverage Working Spouse Rate If your covered spouse is eligible for medical coverage through his/her employer (not applicable if your spouse works for Carolinas HealthCare System), but you choose to enroll him/her in the CHS LiveWELL Health Plan, you must choose the Teammate + Working Spouse or Teammate, Working Spouse and Child(ren) coverage option. The rates for these options are higher, in order to offset the increased cost of covering spouses on the CHS LiveWELL Health Plan. Before you enroll in healthcare benefits, be sure to review all of the coverage your spouse has access to through his/ her employer. The decisions you make each day to address your own health needs enable you to provide the best care to our patients. Smoker/Tobacco User Premium Surcharge The smoker/tobacco user health plan premium surcharge encourages you to make healthy decisions regarding tobacco use. This premium rate increase will only affect tobacco users (teammates and any covered dependents). If it is determined that you are not paying the appropriate health plan premium, you will be required to retroactively pay the smoker/tobacco user premium surcharge or the working spouse premium. Further disciplinary action may be taken as well. Resources for Smokers/ Tobacco Users Through CHS LiveWELL, you have access to resources and support that are designed to help you kick the tobacco habit for good. You also have access to prescription drug coverage through the CHS LiveWELL Health Plan to help you quit. Non-Smoker/Non-Tobacco User Certification By choosing the non-smoker/non-tobacco user coverage option, you certify that you nor your covered dependents smoke or use tobacco products, or are currently enrolled in a tobacco cessation program. If it is unreasonably difficult due to a medical condition, or if it is medically inadvisable, for you or anyone you cover to stop tobacco usage, please contact Benefits Administration at for assistance in developing another way to receive the non-smoker/non-tobacco user health plan rate.

By accessing the appropriate care resource, you can more effectively manage your healthcare costs without sacrificing quality. Use any of the following On-Site Care locations for convenient and cost-effective care for the rapid onset of illness and injuries: Airport Center CMC-Pineville Carolinas Medical Center/ Levine Children s Hospital Campus CMC-NorthEast CMC-University CMC-Union CHS Virtual Visit CHS Virtual Visit gives teammates, spouses and their dependents located in North Carolina LIVE access to a Carolinas HealthCare System medical provider 24 hours a day, 7 days a week via mobile device, tablet, smart phone or computer. Go to PeopleConnect to learn more. Focus on Preventive Care Regular preventive care can help you stay well and catch problems early when they are most treatable. This is why the CHS LiveWELL Health Plan covers preventive care at 100%. More information about preventive care and a full preventive care schedule is available on PeopleConnect. Time Out for Prevention To ensure you have the time you need to take advantage of your preventive care benefits, we continue to offer Time Out for Prevention. Teammates have two hours of paid time off to use at any time during the year for completing routine, covered preventive care. Before using Time Out for Prevention be sure to discuss it with your manager and provide enough notice to ensure the appropriate time keeping code is recorded during your absence.

12 Prescription Drug Benefits When you enroll in the CHS LiveWELL Health Plan, you automatically receive comprehensive, flexible and valuable prescription drug coverage, which includes a Preventive Prescription Drug list that is covered at 100% by the plan. Retail Prescriptions When a physician prescribes a medication to treat a shortterm illness, you can get it filled at any pharmacy in the Catamaran Network but using a Carolinas HealthCare System pharmacy will provide the most savings. A full listing of network pharmacies is available online at CMC Rx Pharmacy The System has its own group of pharmacies, saving you money and time when you have prescriptions to fill. You will pay reduced copays when you use one of the following CMC Rx Pharmacy locations: CMC Rx Pharmacy Medical Center Plaza CMC Rx Pharmacy Morrocroft Medical Plaza CMC Rx Pharmacy NorthEast Gateway CMC Rx Pharmacy NorthEast Pavilion CMC Rx Pharmacy NorthCross CMC Rx Pharmacy Matthews Medical Plaza CMC Rx Pharmacy Steele Creek You will pay 100% of your prescription drug costs until your deductible is met. These costs can be paid from your HSA. If you use certain Preventive Prescription Drugs, the deductible may be waived. CarolinaCARE Prescription Mail Service The mail service pharmacy reduces your out-of-pocket expenses and the time you spend ordering and picking up prescriptions by allowing you to receive up to a 90- day supply of your medication. If you or your dependents take ongoing monthly maintenance medications, all of your prescriptions must be filled through the mail service at CarolinaCARE, or you will pay 100% of the cost. All prescription mail delivery services will be administered by CarolinaCARE. Ongoing maintenance medications may be filled at any retail pharmacy three times only (including CMC Rx Pharmacy). After your third fill at a retail pharmacy, you must pay the full cost of the medication if you are not using CarolinaCARE.

14 Managing Your Costs using flexible spending accounts No one wants to get sick or hurt. Being prepared can ensure you and your family are better able to manage the financial challenges brought on by medical bills. To help you prepare, Carolinas HealthCare System provides several options designed to help you save for future healthcare expenses through a health savings account (HSA) and flexible spending accounts (FSAs). Electing Voluntary Accident and Critical Illness insurance plan coverage can also help protect you from unexpected expenses. We also have flexible spending account options to help you manage costs with a Dependent Daycare FSA. If you are enrolled in CHS LiveWELL HEALTH PLAN, you may enroll in the LFSA in addition to the HSA. The LFSA can be used for dental and vision expenses only. Even if you choose not to participate in the CHS LiveWELL Health Plan, you can still take advantage of a healthcare flexible spending account. Limited Purpose FSA (LFSA) If you enroll in CHS LiveWELL Health Plan, you have access to a health savings account (HSA) to help you save for current and future healthcarerelated expenses. Additionally, you can participate in the Limited Purpose FSA (LFSA), to set aside additional pre-tax dollars (up to $2,500) to cover eligible dental and vision expenses. Healthcare FSA (HCFSA) If you or your eligible dependents are not covered under the CHS LiveWELL Health Plan you are eligible to make contributions to the HCFSA. Use the money in your HCFSA to pay for eligible expenses for yourself or anyone you claim as a dependent on your income tax return Eligible expenses are medical, prescriptions, dental and vision charges not fully covered by other benefits or insurance Your annual election is deducted from each pay in equal amounts The entire amount of your annual election is available, whether or not the funds have fully accumulated in your account HCFSA will not be offered in 2016; we will not offer a grace period in 2015 for remaining funds Expenses must be incurred on or after your effective date of coverage during the plan year You decide how much you want to contribute to your HCFSA up to the maximum contribution for 2015, $2,500 For more details about the HCFSA and eligible expenses, visit p502.pdf. Dependent Daycare Flexible Spending Account (DCFSA) You can use the DCFSA to pay expenses for any eligible, work-related dependent daycare expenses you incur such as licensed daycare centers for children or dependent parents or spouses, summer day camps, nursery schools or afterschool care. Unlike the HCFSA, your funds must be in your DCFSA before they can be reimbursed. Eligible dependents include: Children under age 13 Disabled dependents of any age who live with you more than eight hours per day, whom you claim on your tax return and who are enrolled in an eligible daycare Controlling Costs by Being a Good Healthcare Consumer We all play a role in our healthcare costs. Being an active healthcare consumer means knowing the cost of your care and making informed benefit decisions. There are many tools and resources available to help you achieve this, such as: CHS On-Site Care CHS Virtual Visit Nurse Advice Line CHS Back Pain Clinic Teammate Injury Helpline Employee Assistance Program (EAP) Online enrollment support and education tools Financial Health Support budgeting and retirement planning sessions

15 [12/13] Annual Election You can contribute up to $5,000 ($2,500 if married, filing separately) to your DCFSA for This limit may change if: You are considered a highly compensated teammate, the limit is subject to reductions based on IRS discrimination testing results Both you and your spouse contribute to a DCFSA, only $5,000 combined is allowed, based on IRS guidelines Using Your HSA and FSAs Carolinas HealthCare System partners with Stanley Hunt Dupree & Rhine (SHDR), a division of Branch Bank & Trust (BB&T), to administer all HSAs and FSAs, making it easy to access your account information in one place. If you enroll in an FSA or an HSA for 2015, you will receive a Benefit Access Card, which is loaded with the funds from all of your accounts (or if already enrolled, your current card will automatically be loaded). You can easily track, manage and pay for eligible expenses by accessing your account online at carolinashealthcare. 18 LIFE AND DISABILITY BENEFITS Benefits for Teammates Working 30 or more Standard Hours Per Week Group Term Life Insurance Carolinas HealthCare System provides Group Term Life to each eligible teammate at no cost after completing 90 days of continuous service. Teammates whose standard hours are 30 or more per week receive group life insurance coverage at 1-1/2 times their annual base salary (based on rate of pay and standard hours of work), rounded up to the next $1,000, up to a maximum of $300,000. Disability Short Term Disability Carolinas HealthCare System provides eligible teammates a Short Term Disability (STD) plan that pays benefits in the event a teammate s illness extends beyond 14 calendar days. Benefits are paid until the earlier of the teammate s recovery from disability or through the 180th calendar day of disability. This coverage is provided at no cost and you are automatically enrolled after completing 90 days of continuous service. Short Term Disability Buy-Up Option The STD program has a 14-day waiting period before the plan pays benefits. Teammates can use Paid Time Off (PTO) during the 14-day waiting period when away from work due to an illness or injury. To shorten the waiting period, teammates may elect to purchase the STD Buy-Up policy that shortens the 14-day waiting period to seven days. Teammates can enroll and purchase the STD Buy-Up policy during their initial benefits enrollment. If a teammate chooses not to enroll in the STD Buy-Up policy during their initial benefits enrollment, future enrollment may require a health questionnaire to qualify. Long Term Disability If an illness or injury prevents a teammate from returning to work beyond the 180-day elimination period, he/she may be eligible for Long Term Disability benefits. Teammates who complete 90 days of continuous service are automatically enrolled in this plan. This coverage is provided at no cost to eligible teammates. Disability benefits are subject to pre-existing condition clauses. Please see the Disability Plans Booklet located on PeopleConnect for plan details. Supplemental Life and Accidental Death & Dismemberment (AD&D) Coverage Teammates whose standard hours are 16 or more can purchase Supplemental Life and AD&D Insurance in increments of 1 times your annual base earnings up to 5 times your annual base earnings. As a newly eligible teammate, you will not have to answer any health questions to qualify for this coverage. The maximum amount you can purchase cannot be more than the lesser of 5 times your annual base earnings or $1,000,000. 20 Accident And Critical Illness Insurance Complements to Your Health Plan Accident and Critical Illness Insurance may help absorb the deductibles and other out-of-pocket costs that arise when a severe illness or injury strikes. You can use this benefit any way you choose to pay deductibles and coinsurance, expenses your family incurs to be by your side or simply to replace lost earnings from being out of work. 22 LONG-TERM CARE INSURANCE Long-term Care can place an enormous emotional and physical strain on loved ones and family members. Long-term Care Insurance offsets expenses such as home health care, assisted living and nursing home costs. Coverage for this type of care is typically not covered by health insurance, Medicare or disability programs. It is important to note that the need for Long-term Care services can occur at any time, due to illness or accident. Long-term Care is personal care or supervision needed by people of all ages for an extended period of time.
