GROUP PERSONAL ACCIDENT CERTIFICATE OF CURRENCY This letter is to certify that our client s policy is current as detailed below and cover has been granted in terms of the Insurer s Standard Policy. The policy referred to is current as at the date of issue of this certificate and whilst, a due date has been indicated it should be noted that the policy may be cancelled in the future. Accordingly, reliance should not be placed on the expiry date. This certificate is not a substitute for the Policy of Insurance. The policy, not this certificate, details the rights and obligations of the Insured and the extent of the insurance cover. Insured Person(s)/ Categories: 1) All Students and Non-Students, Active Life and Active Associate Members, Members appointed by the Governor In Council, Honorary employees and visiting lecturers, Staff of the University, Union, Guild or College who elect to be covered including Wollongong University College & UOW Enterprises 2) Governing Body, Chancellor, Deputy Or Vice Chancellor, Council, Director General Of Education, Members Appointed By The Governor-In-Council, Committee Members, Board Members Of The Guild, Members Of Foundations 3) All Students involved in biomedical sciences Scope of Cover: All Categories (1, 2 & 3) Whilst on or off Campus and engaged in authorised University activities (including but not limited to University Course or Sport related activities) and/or practical placement or community placement activities and, except for Non-Students, includes Travel Directly to and from such activities. Travel Directly means travel to and from the Covered Person's normal place of residence or normal place of employment and shall include any minor deviations or interruptions which in no way increase the risk of Bodily Injury that would have normally arisen had the person travelled directly without deviation or interruption. Endorsements: Aids/Hiv/Hep C Extension- General Exclusion 4 is amended and the Policy is extended to include AIDS/HIV or Hepatitis C (HEP C) acquired through medical treatment or through assault by a third party for Students enrolled in Health Sciences as declared. We will pay a lump sum benefit of $30,000 following diagnosis of AIDS/HIV or HEP C as a result of an Insured Event (as defined below) to the Covered Person or if deceased, the Covered Person's estate. Insured Event shall mean a Bodily Injury suffered by a Covered Person during the Period of Insurance and whilst the person is a Covered Person and acquired by accident or violence whilst the Covered Person is involved in research activities and or University approved activities directly related to their coursework with the Policyholder that causes and results in the Covered Person being positively diagnosed as suffering from Acquired Immune Deficiency Syndrome (AIDS) or Hepititis C (HEP C) within ninety days (90) from the date of Bodily Injury. Any Bodily Injury that may lead to the diagnosis of Acquired Immune Deficiency Syndrome (AIDS) or Hepititis C (HEP C) must be reported as soon as practical to Us and medical tests carried out by a Doctor on a blood sample taken within forty-eight (48) hours from the date the Bodily Injury occurred. The diagnosis of such medical tests must prove positively that the Covered Person was not HIV positive or carry HEP C at the time and date the Bodily Injury occurred. Diagnosis must be made positively by laboratory and/or clinical tests within six (6) months of the accident. Infection transmitted by any other means, including but not limited to sexual activity or non-medical intravenous drug use, is not considered a Bodily Injury under this insurance. Age Limit Extension- The maximum age limit of Covered Persons under the Policy is extended to seventy-nine (79) years of age. Notwithstanding anything contained in the Policy to the contrary the following conditions shall apply to this extension: There is no cover under the Policy with respect to any Covered Person who is aged eighty (80) years or more and all cover with respect to a Covered Person shall cease upon their attaining that age. This will not prejudice any entitlement to claim benefits which have arisen before a Covered Person has attained the age of eighty (80) years. The maximum amount We will pay under Section 1 - Part A - Lump Sum Benefits - Event 1 - Accidental Death with respect to any Covered Person aged between sxity-five (65) and eighty (80) years of age is $20,000. There is no cover under the Policy for Part A - Lump Sum Benefits - Event 2 - Permanent Total Disablement with respect to any Covered Person aged between sixty-five (65) and eighty (80) years of age. Bed Care/Extra Cash- If during the Period of Insurance and whilst the person is a Covered Person, the Covered Person is confined to bed (other than in a hospital or other medical facility) as a result of a Bodily Injury, for a period in excess of forty-eight (48) hours, and the Covered Person presents Us with a written opinion of a Doctor that verifies that the Bodily Injury caused the Covered Person to be confined to bed, We will pay the Covered Person $35 per day up to a maximum of 365 consecutive days.

Emergency Home Help- If during the Period of Insurance and whilst the person is a Covered Person, a Covered Person who is retired, unemployed or not in receipt of a Salary suffers from Event 25 and/or 26 described in Part B of the Table of Events and is unable to carry out Domestic Duties, We will pay for the cost of reasonably and necessarily incurred Domestic Duties expenses as a result of that Bodily Injury up to $250 per week payable from the 8th day of treatment by a Doctor for a maximum of 52 weeks. Definitions Under Emergency Home Help- Domestic Duties means the usual and ordinary domestic duties undertaken by someone as a homemaker and could include child-minding and home help services. Conditions Applying To Emergency Home Help- 1. Childminding and home help services must be carried out by persons other than the Covered Person's Close Relatives or persons permanently living with the Covered Person. 2. Childminding and home help services must be certified by a Doctor as being necessary for the recovery of the Covered Person. Drug & Alcohol Exclusion- The Policy shall not apply to any Event resulting directly or indirectly from a Covered Person being under the influence of intoxicating liquor, including having a blood alcohol content over the prescribed legal limit whilst driving, or being under the influence of any other drug unless it was prescribed by a Doctor and taken in accordance with a Doctors advice. Non-Medicare Medical Expenses- If during the Period of Insurance and whilst the person is a Covered Person providing services, without payment, to an educational, religious, charitable or benevolent organisation on behalf of the Policyholder, the Covered Person suffers from a Bodily Injury, We will pay the Non-Medicare Medical Expenses incurred up to a maximum of $10,000. Excess $50. Definitions Under Non-Medicare Medical Expenses- Non-Medicare Medical Expenses means expenses: (a) incurred within twelve (12) months of sustaining a Bodily Injury: and (b) paid by a Covered Person or by the Policyholder for Doctor, physician, surgeon, nurse, physiotherapist, chiropractor, osteopath, hospital and/or ambulance services for the following treatments: o Medical o Surgical o X-ray o Chiropractor o Osteopathic o Physiotherapy o Hospital o Nursing Treatment but does not include dental treatment, unless such treatment is necessarily required, to teeth other than dentures and is caused by the Bodily Injury referred to in (a) above.

Conditions Applying To Non-Medicare Medical Expenses- 1. The benefit payable is less any recovery made from any private health insurance fund with respect to the expense. 2. No benefit is payable in respect of the Medicare gap, being the difference between the payment made by Medicare and the Medicare Benefits Schedule fee for the expense. Exclusions Applying To Non-Medicare Medical Expenses- 1. No cover is provided for Covered Persons engaging in voluntary work experience with the Policyholder (except to the extent that they are engaged in providing services, without payment, to an educational, religious, charitable or benevolent organisation on behalf of the Policyholder). Student Tutorial Costs- If during the Period of Insurance and whilst the person is a Covered Person, a Covered Person who is a student, suffers from Event 25 and/or 26 described in Part B of the Table of Events and is unable to attend registered classes, We will pay the cost of reasonably and necessarily incurred home tutorial services as a result of that Bodily Injury up to $250 per week payable from the 8th day of treatment by a Doctor for a maximum of 52 weeks. Conditions Applying To Student Tutorial Costs- 1. The Covered Person must be registered as a full time student. 2. Home tutorial services must be carried out by persons other than the Covered Person's Close Relatives or persons permanently living with the Covered Person. Sports Injury/Sickness Excess Period- Any loss arising from participation in any form of sport is subject to a twenty-eight (28) day Excess Period with respect to Section 1 - Part B - Weekly Benefits - Bodily Injury and/or Part C - Weekly Benefits - Sickness.
