Potential Causes of Sudden Cardiac Arrest in Children Project S.A.V.E. When sudden death occurs in children, adolescents and younger adults, heart abnormalities are likely causes. These conditions are often undiagnosed and fall into three main categories. Possible diagnoses include: Structural/Functional Hypertrophic Cardiomyopathy (HCM) Coronary Artery Anomalies Aortic Rupture/ Marfan Syndrome Dilated Cardiomyopathy Myocarditis Left Ventricular Outflow Tract Obstruction Mitral Valve Prolapse (MVP) Coronary Artery Atherosclerotic Heart Disease Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) Post-Operative Congenital Heart Disease Electrical Long QT Syndrome (LQTS) Wolff-Parkinson-White Syndrome (WPW) Brugada Syndrome Catecholaminergic Polymorphic Ventricular Tachycardia Short QT Syndrome Complete Heart Block (CHB) Other Primary Pulmonary Hypertension (PPH) Drugs/Stimulants Commotio Cordis indicates familial or genetic causes

Definitions Any of these conditions can result in an unstable and ineffective heart rhythm, ventricular fibrillation, which leads to sudden cardiac arrest. Structural/Functional 1. Hypertrophic cardiomypathy (HCM) is a primary and usually familial cardiac disorder with unique pathophysiology and a diverse clinical course which can affect the structure of the heart muscle, normal outflow of blood from the left ventricle and the electrical function of the heart. Its main feature, seen on echocardiogram, is an excessive thickening of the heart muscle in the ventricular septal wall measurement. The normal alignment of muscle cells is absent (myocardial disarray). Many of those with HCM lead normal lives without any symptoms. HCM which is symptomatic is thought to be the leading cause of sudden cardiac arrest in young athletes. (http://www.4hcm.org) 2. Coronary artery anomalies are congenital or acquired (e.g. Kawasaki s disease), and can put patients at risk of arterial compression leading to myocardial ischemia and possible sudden death with the increased oxygen demands of exercise. 3. Marfan syndrome (MFS) is a genetic disorder that affects the body s connective tissue, especially the eyes, skeleton, blood vessels and heart. In the cardiovascular system, the wall of the aorta can be weakened, allowing it to dilate and potentially rupture. Also separation of the layers of the aorta can occur, causing tearing or dissection. (www.marfan.org ) 4. Dilated cardiomyopathy is the most common of the heart muscle diseases, and can be found at any age, including childhood. The heart becomes enlarged and less efficient and arrhythmias can occur. Poor exercise tolerance and shortness of breath with exercise can be signs indicating a need for further evaluation. 5. Myocarditis is an inflammation of the heart muscle and can be caused by viral, bacterial or fungal infection; rheumatic fever; tuberculosis or toxic drug poisoning. 6. Left ventricular outflow tract obstruction can occur with aortic valve stenosis at any level and HCM. It usually causes changes in the pressure gradients and progressive symptoms of dyspnea, chest pain and syncope. 7. Mitral valve prolapse (MVP) is a disorder in which the mitral valve doesn t close properly allowing the potential leakage of blood from the left ventricle back into the left atrium. Often asymptomatic, it also can feature mild intermittent symptoms of a racing heartbeat, dizziness or lightheadedness, shortness of breath and fatigue. Less commonly symptoms include severe regurgitation of blood into the atrium, putting the patient at risk of bacterial endocarditis or congestive heart failure. Irregular heart rhythms are also rare complications, and sudden death has been associated rarely. 2

Coronary artery atherosclerotic disease which blocks and stiffens the blood vessels of the heart itself is seen more commonly in adults and can lead to areas of heart damage and irregular heart rhythms, including ventricular fibrillation. 9. Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a genetic, progressive heart disease, causing the right ventricular muscle to be gradually replaced with fatty and fibrotic tissue. The disease can cause abnormal heart rhythms, and often presents with palpitations, chest pain, exercise-associated syncope or, in rare instances, sudden cardiac arrest. (www.arvd.com) Electrical 10. Long QT syndrome (LQTS) is most often a genetic condition affecting the electrical system of the heart. LQTS is three times more common in the US than childhood leukemia. It can also be acquired after use of certain medications. Individuals with this condition may have no symptoms or may have symptoms of fainting with exercise, emotion or startle, unexplained seizure activity or a fast, chaotic heart arrhythmia that can lead to loss of consciousness and cardiac arrest. It is usually recognized by an abnormality on the electrocardiogram (ECG) known as prolonged QT interval and usually can be treated medically after diagnosis. (www.sads.org) 11. Wolff-Parkinson-White syndrome (WPW) is a type of supraventricular tachycardia in which an extra electrical pathway exists between the atria and the ventricles, leading to arrhythmias. Some people with WPW never have tachycardia or other symptoms. People with this syndrome who have symptoms or episodes of tachycardia (rapid heart rhythm) may have dizziness, chest palpitations, fainting or, rarely, cardiac arrest. About 80 percent of people with symptoms first have them between the ages of 11 and 50. 12. Brugada syndrome is a genetic condition that affects the electrical system of the heart due to an abnormality in the cardiac ion channel. It may be evident on ECG (elevated ST segment, ventricular tachycardia). These EKG changes occur more commonly with a fever. Patients, most often young adults in the third and fourth decades, may have symptoms of syncope, unexplained seizures and are at risk for sudden cardiac death. 13. Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a genetic condition affecting the electrical system of the heart. There are usually no specific resting ECG changes, but affected patients are prone to very fast heart rhythms during exercise and emotion. 14. Short QT syndrome is a familial disease causing a shorter than normal QT interval that can lead to palpitations, syncope and increased vulnerability to ventricular fibrillation and sudden cardiac arrest. 3

15. Complete heart block is a failure of electrical conduction from atria to ventricles, disrupting the normal electrical pathways in the heart and can lead to dysrhythmias and sudden death. 16. Postoperative congenital heart disease in the older child or young adult can result in sudden death due to coronary ischemia, dysrhythmias, sepsis, thrombotic events or pulmonary hypertensive crisis. Other 17. Primary Pulmonary Hypertension (PPH) can be a primary condition or associated secondarily with congenital heart defects. One of the complications can be an irregular heart rhythm, including ventricular fibrillation, leading to sudden death. 18. Drugs and stimulants such as cocaine, ephedra, heroin, marijuana, amphetamines, ecstasy, inhalants, gasoline, glue and emetine can cause sudden death in users. 19. Commotio Cordis is a life-threatening dysrhythmia caused by a direct, nonpenetrating, often low-impact, blow to the chest at a critical time in the heart beat, most often during participation in sports such as baseball, hockey, lacrosse and softball. Approximately one third of the reported cases occurred outside of sporting activities, including intentional chest blows during fighting or child abuse, or even impact by snowballs or hollow plastic toys. The impact causes a sudden disturbance of cardiac rhythm, most commonly ventricular fibrillation. Early resuscitation and defibrillation are critical to survival. 4
