VOLUNTARY COUNSELLING AND TESTING {VCT} AND COMPREHENSIVE CARE CENTRE {CCC} VCT for HIV at Jocham hospital involves two counselling sessions: * Prior to taking the test known as pre-test counselling and following the HIV test - post-test counselling * Counselling focuses on the infection {HIV}, the disease {AIDS}, the test, and positive behaviour change. We have partnered with prominent non governmental organisations and the public sector in educating the immediate community about HIV/AIDS.
PHYSICAL MEDICINE DEPARTMENT This is the assessment, treatment and prevention of illness, injuries, and disorders by use of physical means such as exercises, electrical agents, short tissue manipulation (manual therapy) rather than the use of drugs. It compliments other therapies and treatments. Jocham hospital physical medicine department comprises of the physiotherapy and occupational therapy. RADIOLOGY DEPARTMENT Jocham hospital radiology department is modern, dynamic and a professional diagnostic centre. Equipments available are the X-ray, Ultra sound machine and CT scanner among others.
INPATIENT DEPARTMENTS The inpatient department is coordinated at the customer care desk located at the reception area of the hospital. The inpatient division consists of: - Wards - Operating theatres and - Maternity wing Jocham hospital has a 43 bed capacity allocated as follows: - Paediatric ward has 8 beds. - Female ward has 13 beds. - Male ward has 12 beds and, - Maternity wing 10 beds. The hospital is NHIF accredited with a rebate rate of Ksh. 1,500 per day that was effected from 1 st may 2008 up to date.
Cont All wards are headed by a Head of department and supported by senior nurses and cleaning staff with a one to one patient nurse allocation. Each bed is fitted with an oxygen supply and a bell to summon a nurse in case of an emergency. The beds have adjustable inclination knobs to suit individual and medically recommended resting position. Access to kitchen services, washrooms, and a relaxation garden is a necessary facility that the hospital has assured its patients. Accommodation facilities range from self contained private rooms, semi private {double bed} and the general ward bed {four beds cubicles} to enhance patients choice.
MATERNITY WING The maternity wing takes care of expectant mothers and their newborn babies. The ward comprises of maternity, labour room and nursery. We take good care of expectant mothers during pregnancy, labour and after delivery. Child welfare clinic and childbirth education classes are offered. Nursery is where the new born babies are admitted for close monitoring. The well babies room in with their mothers. We have education sessions for the mothers on baby bathing, breast feeding techniques, cord care and general baby care on daily basis. The unwell and premature babies requiring close monitoring and stabilisation from birth complications are admitted in the premature unit. Here we have specialised teams of neonatal nurses who monitor the baby either by nursing them in the incubator or cot. JOCHAM FUNERAL HOME The management of jocham hospital felt it prudential to include this service due to high level charges associated with similar private set up. To bridge this big gap, we came up with the idea of having a cost effective mortuary set up. The mortuary is well equipped with modern equipment for preservation, and has qualified personnel to take care of vital procedures. A body viewing and prayer shed is available. We arrange funeral services in accordance with the wish of families and the deceased. When the body is brought to the funeral home, they are embalmed to delay decomposition. Embalming procedure involves replacement of the blood of the decedent with a mixture of preservative chemicals and dyes, aspiration of the internal organs, and the setting of the persons features. If the deceased was disfigured in an accident or illness, our embalmer utilises retrospective technique to make the body presentable for an open casket service. There is a large area set aside for families to gather at a visitation. This area contains a space to display the deceased in the casket for their families to pay their respects. This structure of management was adopted to make sure that all information and directives flow efficiently. The finance department is mandated to handle finances and accounting functions of the hospital. This department oversees all admissions, transfers and discharges, procurement and stores, internal audit and information technology.
Cont The procurement and stores division is an integral part of Jocham hospital with the objective of creating, assuring and protecting value of the hospital while coordinating a steady flow of materials and services to ensure continuity of supply. It is centralised to ensure process optimization, uniformity, professionalism, collection of information regarding supplier performance and attainment of economies of scale associated with consolidated purchases. This and other policies ensure the hospital provides affordable services to all while breaking even. The marketing department coordinates marketing, public relations and customer service. This department makes sure that the existing and prospective clients get the right information by identifying the best communication channels possible and making sure that there is maximum client satisfaction. The security, transport and fire safety department is a well modernised facility that makes use of current innovations and quality equipment to ensure that our patients, staff, establishment and facilities are well protected.
SUPPORT SERVICES This carries with it the Internal Audit, Engineering and Maintenance, Laundry and House keeping, and the catering department. The Internal audit department provides quality auditing services that result in reduced costs, gains in operational efficiencies and strengthened internal controls to assist the hospital in the achievement of her goals and objectives. The medical engineering and maintenance department is responsible for coordination of projects for execution, calibration, of equipment, maintenance and repair. The laundry and house keeping department is charged with the responsibility of cleaning all linen and wears as a process of patient care, in readiness for re-use and carrying out repair work as may be necessary. The catering department provides patient meals every day; and very so often for staff and seminar/conference attendees and visitors. HOW NHIF CLAIMS PROCESSING WORKS AT JOCHAM HOSPITAL 1. ADMISSIONS Client requested whether he/she is a contributor to NHIF If yes, give membership number or national ID number 2. CLIENT REFFERED TO NHIF/BILLING OFFICE The computer is linked to NHIF office. Billing clerk accesses the client s details If the is registered notify NHIF through the system. 3. AFTER NOTIFICATION The client is informed to bring a copy of national ID card and NHIF card before discharge.

4. ON DISCHARGE The bill is prepared. Cont NHIF rebate is deducted from the bill per the hospitalised days. The client is requested to sign the claim form 5. REQUIREMENTS ARE ATTACHED TO THE CLAIM FORM Hospital bill Discharge summary of the patient. Copy of national Identity card Copy of NHIF card.
Cont 6. The attachments are forwarded to medical records officer for disease coding on the claim form and the NHIF data base (system). 7. The hospital launches the claim for payment to NHIF. This is done on weekly basis (after every 7 days). 8. The hospital waits for payment.
Challenges encountered During emergency situations, there is no immediate information. This delays notification to NHIF that is supposed to be done within 24 hours of admission. Majority don not have NHIF cards. Many contributors do not up date the list of their dependants immediately. The current rebate of ksh. 1,500 is below the average consumption consideration the daily increase of many consumer goods. Consider increasing to Ksh. 2,500.
QIT Quality Improvement Team NHIF invited all hospitals in the region for a joint initial training. The attendance was 4 staff members per hospital and Jocham sent the required representation. All were advised to form a team with a representative from each department (JH complied and the team is currently in place). QIT meets on monthly basis in hospital board room(a programme incorporated by the office of the Chief Administrator). Representatives from NHIF have visits the hospital periodically to advice the QIT and the administration on lines of weakness and how to maintain the lead position in areas of strength.
The impact of QIT to the hospital Quality of services provided has improved over time. The hospital environment is kept tidy to the benefit of all stakeholders. Staff motivation levels have increased over time. Customer expectations are being met and exceeded as a result.
