Notes: Your plan requires a selection of a Primary Care Physician. Your plan requires a referral from your Primary Care Physician for select covered services. All medical services subject to a coinsurance are also subject to the annual medical with exception of facility emergency room charge and services in physician's office subject to coinsurance (including dialysis, chemo/radiation therapy and prescription drugs). This plan includes an aggregate embedded accumulation for the family and out-of-pocket maximum. This means that the family amounts can be met by any combination of amounts from any family member, however one member must satisfy their individual and one member (either same or other family member) must satisfy the individual out of pocket amount. If your plan includes a hospital stay copay and you are readmitted within 72 hours of a prior admission for the same diagnosis, your hospital stay copay for your readmission is waived. If your plan includes an emergency room facility copay and you are directly admitted to a hospital, your emergency room facility copay is waived. Human Organ and Tissues Transplants require precertification and are covered as any other service in your summary of benefits. Your copays, coinsurance and count toward your out of pocket amount. For plans with an office visit copay, the copay applies to the actual office visit and additional cost shares may apply for any other service performed in the office (i.e., X-ray, lab, surgery), after any applicable.
