Health Care & Benefits Overview CCR provides comprehensive health, dental, life, AD & D and disability benefits to all eligible Ottawa employees through a Traditional benefits plan Ottawa s benefit provisions are more generous to those provided to nonbargaining CCR employees Short and Long-term Disability benefits provide income security for employees, in the event of a non-work related illness or disability Life insurance provides the employee s family with financial protection should they need it In addition, the company offers a comprehensive, 100% company-paid, Employee Assistance Program to employees and their families, to assist with a variety of personal challenges
Introduction to BenefitsPlus ADVANTAGES Over Traditional Choice of coverage based on personal and family needs Prescription Drug Cards to be provided for ALL levels no need to pay out at first instance Certain maximums on Paramedical services for Option 3 are greater than the current plan. No annual maximum for all health expenses combined No annual deductibles per person or per family Ability to purchase additional insurance protection All eligible employees will have some level of coverage as waiver of benefits is no longer an option Changes to coverage can be made annually

Life and AD&D Insurance CORE BENEFITS Basic Employee Life Insurance Coverage is 1 times the employee s annual base salary OPTIONAL BENEFITS Optional Employee and/or Spousal Life Insurance Can be purchased in units of $10,000 to a max of 50 units or $500,000 Coverage in excess of $50,000 (for employee only) must be approved by insurance carrier All coverage purchased for spousal must be approved by insurance carrier Optional Child(ren) Life Insurance Can be purchased for eligible children in units of $5,000 to a maximum of $20,000 per child Optional AD&D Insurance Can be purchased in units of $10,000, to a maximum of $200,000 Choose Single or Family coverage 5
Disability Maximum 48 hours per calendar year Short -Term Disability Benefits Up to a maximum of $600 per week for a max. 26 weeks Long-Term Disability Benefits 66.67% of monthly income up to maximum of $2,400 6

Dental Benefits Option 1 Option 2 Option 3-20% Basic (Preventative) - 20% Major Restorative - 50% Orthodontia - Annual Max - $1,000-100% Basic (Preventative) - 50 % Major Restorative - Annual Max - $1,500-100% Basic (Preventative) - 80% Major Restorative - 50% Orthodontia - Annual Max - $2,500 Orthodontia coverage is 50% up to a maximum of $3,000 Some Dental expenses that are covered (depending on option level selected) include: * Polishing/Scaling * Endodontics * Periodontics * Dentures/Crowns * Extractions * Orthodontia 9
Option 1 - Coordination of Benefits Option ( COB ) Allows employees to optimize the amount of coverage (up to 100%) through BenefitsPlus and their spouse s benefits coverage By selecting the COB option, employees will have basic coverage through CCR and have money that will be allocated towards their Healthcare Spending Account Healthcare Spending Account Monies allocated in the HSA can be used towards reimbursement for eligible out-ofpocket medical/dental expenses not covered by the BenefitsPlus plan An HSA can pay for any eligible expenses as per the medical tax credit rules under the Income Tax Act. Unused HSA credits may be carried forward 1 calendar year The HSA is a non-taxable benefit 10
Example of How the HSA works $300 in eligible expenses $150 reimbursed through the BenefitsPlus Plan and through Coordination of Benefits (if applicable) Apply remaining $150 out of pocket towards 2013 HSA allotment - $150 is reimbursed until there is no money left in HSA
Life Event Changes A Life Event Change is considered to be: Marriage Cohabitation (for a minimum period of 12 months) Separation, divorce or termination of cohabitation with a common-law spouse Birth, adoption or change in custody/dependency of a child Death of a spouse or dependent child Loss or gain of other group health coverage If you experience a life event change, you must notify the HeRe team within 60 days of the life event change. 12
