Nurseline Crisis to Wellness Warmline CRN staffs LPN s available 24/7/365 for medical and health and wellness concerns. CRN completes outbound calls to recipient and clinical team, participates in staffings, and captures data/generates reports. Intended outcome is to reduce frequent utilization of the crisis services, connect recipients to services and community/natural supports and improve overall wellness. Duration of services is no more than 60 days. Recipients can repeat the program. CRN provides telephone support by certified peer support specialists with intended outcome of promoting recovery. Transition Point 3-5 day service offering stabilization, brief intervention and transition services. Transition Point Facility is always open - seven days per week, 24 hours per day. At a minimum, TP is staffed with a Behavioral Health Technician (BHT) or Licensed Practical Nurse (LPN) and a Peer Support Specialist. Additionally during the day is staffed with a Transition coordinator to assist in coordination of care and discharge planning. Group treatment is facilitated daily by the Peer Support Specialist, Transition coordinator, RN and Behavioral health Clinicians from our Access Points.
