Transition Point 3-5 day service offering stabilization, brief intervention and transition services. Transition Point Facility is always open - seven days per week, 24 hours per day. At a minimum, TP is staffed with a Behavioral Health Technician (BHT) or Licensed Practical Nurse (LPN) and a Peer Support Specialist. Additionally during the day is staffed with a Transition coordinator to assist in coordination of care and discharge planning. Group treatment is facilitated daily by the Peer Support Specialist, Transition coordinator, RN and Behavioral health Clinicians from our Access Points.
