Atrius Health A Leader in Health Care Innovation Formed in 2004, Atrius Health is a non-profit alliance of six community-based medical groups and a home health care, private duty nursing, and hospice agency. The groups include Dedham Medical Associates, Granite Medical Group, Harvard Vanguard Medical Associates, Reliant Medical Group, South Shore Medical Center, and Southboro Medical Group, and VNA Care Network & Hospice. Leading the Way: Improving Quality and Reducing Cost Atrius Health has a history of leadership in many of the concepts that are now gaining traction in statewide and national efforts to improve health care quality and decrease cost: Payment Reform Atrius Health has years of experience successfully managing cost and quality under a global budget or similar payment structure and favors the expansion of this payment model for providers. In recent years, Atrius Health has actively worked with local health plan payers on incentives linked to accepted measures of quality and has demonstrated success in improving performance on screening, vaccination, and health outcomes. BCBSMA Alternative Quality Contract Atrius Health was one of the first organizations to sign on to the BCBSMA Alternative Quality Contract (AQC) which is viewed by many as one of the first models of an Accountable Care Organization. Under the AQC, Atrius Health coordinates care for BCBSMA members using leading edge population management methods to do outreach for preventive services and chronic disease management. Since joining the AQC in 2009, Atrius Health has demonstrated significant improvement in measured clinical quality. Medicare Pioneer Accountable Care Organization (ACO) Atrius Health was chosen as one of 32 leading healthcare organizations nationally to participate in the Pioneer Accountable Care Organization (ACO) model, a transformative new initiative sponsored by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. Through the Pioneer ACO model, Atrius Health works with CMS to provide Medicare beneficiaries with higher quality care, while reducing growth in Medicare expenditures through enhanced care coordination. Prior to participating in the Pioneer ACO model, Atrius Health had been functioning as an ACO for many years and witnessed how a strong base of primary care coordinating care across the continuum of specialists, hospitals, home health and primary care can improve quality while lowering costs. Health Information Technology The Atrius Health medical groups clinicians and their support teams utilize an electronic medical record (Epic) to integrate and coordinate care electronically across specialists and different healthcare sites. As a leading edge user of this Epic system, clinicians across Atrius Health benefit from strong decision support tools, efficient workflow, and access to medical records of many of our closest hospital partners. Atrius Health medical teams utilize an online patient portal which allows patients to access portions of their medical record online, receive lab test results electronically, and communicate over a secure connection with their doctor s office, including through their smartphones. Over the past five years, Atrius Health has built a sophisticated data warehouse that combines the electronic medical records, claims data, and pharmacy data for hundreds of thousands of patients. This data warehouse has become a tremendous data source for leading edge research, practice improvement, and enhanced patient care.

Patient-Centered Medical Home Many in the industry view the Patient-Centered Medical Home, in which a multi-disciplinary primary care team coordinates care broadly for its patients, as the primary care office of the future. Atrius Health practices have embodied this standard for decades. In 2012-13, the 37 Atrius Health internal medicine practices across Dedham Medical Associates, Granite Medical Group, Harvard Vanguard Medical Associates, Reliant Medical Group, South Shore Medical Center, and Southboro Medical Group received the highest possible national accreditation as Level 3 Patient-Centered Medical Homes from The National Committee for Quality Assurance (NCQA). This recognition makes Atrius Health one of the largest practice groups in the country to achieve this NCQA medical home designation. The standards required to be met for recognition are aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association. Linking Primary Care and Home Health Care In 2013, VNA Care Network & Hospice joined Atrius Health after over a year as its preferred home health provider. VNA Care Network & Hospice brings an expertise in advance care planning and end of life decision-making to the strong relationships that our patients enjoy with their primary care providers. Atrius Health sees tremendous opportunity to improve patient care and quality and reduce overall costs by creating stronger connections between the primary care office and care that is provided in the home. Shared Medical Appointments Offering more than 30 group appointments at 9 sites in 10 different specialties, Atrius Health leads the nation in the use of shared medical appointments as an option for routine medical care. Harvard Vanguard Medical Associates and Reliant Medical Group s shared medical appointments allow physicians or other clinicians and patients to interact during a 90-minute visit in the company of other patients (private exam time is available when needed). Patients have more time with their physician (90 minutes), improved access to their doctor and a multidisciplinary care team, greater patient education, and closer follow-up care and the help and support of other patients dealing with similar issues. Leadership Academy The boards and management of Atrius Health believe that the task of addressing difficult national healthcare issues must be led by physicians who have the clinical knowledge and the relationship with the patients. With this principle in mind, Atrius Health s Leadership Academy for Chiefs and Clinical Leaders was designed to advance the leadership and management skills of our current clinical leaders. This 10- month program is now being taught to a tenth group of chiefs and clinical leaders. Leadership Academy draws from leaders across our Atrius Health system of care and was designed in collaboration with our specialty directors and chiefs to strengthen skills in four areas: Leadership, Management, Financial Stewardship, and Clinical Operations. Already, 375 physician leaders have been through courses offered the academy. Lean Lean methodology enables organizations to identify wasteful processes and eliminate or improve those. Atrius Health has adopted Lean as an operating system over the past few years to enable us to continue to deliver the higher quality, lower cost, patient-centric care needed in the changing healthcare environment. Through Lean methods, Atrius Health is eliminating tasks that do not add value from our patients perspective, empowering all levels of staff to solve problems and improve job satisfaction, and creating a culture of continuous improvement and innovation.
