Rehab Assist is a unique consulting company founded in 1994. Rehab Assist specializes in providing disability-related management services including case management, geriatric care management, guardianship and life care planning services. What is the mission of Rehab Assist? Our mission is to improve the quality of life for those disabled individuals we serve. Our services can be provided on a one-time or on-going basis. We dedicate ourselves to identifying our client s needs and then orchestrating a plan to address and meet those needs through assertive and timely interventions. Who Uses Rehab Assist s Services? We believe that Rehab Assist provides the highest quality of care management and life care planning services in the field. Our hands-on, intervention-based approach improves the quality of our client s lives. We are proud of the years of service to several hundreds of disabled individuals and their families. We believe that the vast numbers of court appointments speak for themselves. We are NOT one person, but a group of caring people committed to enhancing the life of an individual who has a disability. Rehab Assist is co-owned and operated by licensed clinicians. Henry Brennan is a licensed speech pathologist, certified case manager and certified life care planner. Thomas Kleinhenz is a licensed registered nurse and a certified case manager. Rehab Assist provides services to clients on a local, national and international level.

What Services Does Rehab Assist Provide? Life Care Planning Lifelong medical needs and costs are detailed and projected in a report called a Life Care Plan. These documents are helpful in setting long-term financial reserves. Rehab Assist professionals have vast experience in deposition and trial testimony. Comprehensive Disability Assessments These documents provide a thorough review of an individual s status and the future services they will require. They are most beneficial to families, payers and the court in planning for future needs. Annual Medical Budgets These documents assist the estate-planning team to budget and plan for necessary medical and disability-related expenditures. Probate courts often require this type of projection when analyzing the financial needs of a disabled individual. Special situations such as high tech medical care, special needs trusts or nursing waiver programs can be identified and budgeted for by the case manager. Case / Care Management Highly trained professionals evaluate the needs of the individual and coordinate the necessary services required to meet those needs. These services are provided to pediatric, adult, and geriatric clients on a long term on-going basis or for a shorter duration. Guardianship Under the jurisdiction of the probate court, Rehab Assist serves as guardian of the person for persons who have been adjudicated disabled. These services include plenary guardian, limited guardian and co-guardian.
