SCHOOL OF NURSING UNDERGRADUATE PROGRAMS OFFERED Second Degree Accelerated Program Registered Nurse to Bachelor of Science in Nursing (RN-BSN) APPLICATION DEADLINES Second Degree Accelerated Program November 1 Registered Nurse to Bachelor of Science in Nursing Rolling Admission NON-MATRICULATED STATUS Second Degree Accelerated students are eligible for matriculation upon successful completion of 12 credits in nursing Registered Nurse students are eligible for matriculation upon successful completion of NS 250 Professional Nursing RN LICENSE Registered nurse applicants must submit a copy of their current RN license. INTERVIEWS Applicants for all undergraduate nursing programs are required to participate in an interview with the Adult Program Director as part of the admission process. Candidates can contact the School of Nursing office (203) 254-4150 to schedule an appointment. FINANCIAL AID Information concerning Financial Aid can be obtained by contacting the Financial Aid Office at (203) 254-4125. PERSONAL STATEMENT/ESSAY AND RÉSUMÉ Please describe your purpose in undertaking a baccalaureate nursing education. Include the factors or experience that led to your decision to pursue a BSN at Fairfield University. Please submit any additional information that you feel will help the committee in making a decision. Applicants are required to submit a current résumé in addition to the personal statement. 1

UNDERGRADUATE APPLICATION GENERAL INFORMATION AND INSTRUCTION All required materials must be completed and mailed to: Office of Graduate & Continuing Studies Admission, Kelley Center, Fairfield University, 1073 North Benson Road, Fairfield, CT 06824-5195. APPLICATION FEE A non-refundable application fee of $60 must accompany your completed application. Please make your check or money order payable to Fairfield University. TRANSCRIPTS Official transcript(s) of your academic work must be sent directly from each college or university you have attended to Fairfield University s Office of Graduate & Continuing Studies Admission. Your application will not be considered until all official transcripts are received. PERSONAL STATEMENT/ESSAY AND RÉSUMÉ A personal statement and résumé specified by the program to which you are applying must be submitted. RECOMMENDATIONS Two recommendations are required for admission to all undergraduate programs at Fairfield University. Applicants must use the recommendation forms provided, in addition to a separate written evaluation. MEASLES / RUBELLA IMMUNITY PROOF Connecticut Law requires that students born after December 31, 1956 provide proof of Measles and Rubella Immunization. Although this is not required to complete an application, you must provide proof of immunization prior to course registration. Please keep in mind that this process can take some time, and that you MUST be in compliance before registration. Immunization verification information should be submitted directly to the University s Health Center. Please send the form on page 9 of this application along with your verification. Any questions regarding this policy should be directed to the University Health Center by calling (203) 254-4000, ext 2241. INTERNATIONAL STUDENTS International students must provide a certificate of finances (evidence of adequate financial resources in U.S. dollars) and should apply well in advance of the beginning of the term in which they intend to begin nursing studies. Applicants must submit certified English translations and course-by-course evaluations from approved evaluators of all academic records. All international students whose native language is not English must demonstrate proficiency in the English language. A TOEFL composite score of 550 for the paper test, 213 for the computer-based test, or 80 on the Internet based test is strongly recommended for admission to the school. Scores must be sent directly from the Educational Testing Service (Fairfield s ETS code is 3390). TOEFL may be waived for those international students who have earned an undergraduate or graduate degree from a regionally accredited U.S. college or university. If you require a student visa (for full-time study) to attend Fairfield University remember that: international applications and supporting credentials must be submitted to the Office of Graduate & Continuing Studies Admission at least three months prior to your intended start date. FINANCIAL AID Federal Stafford Loans Under this program, undergraduate students may apply for up to $18,500 per academic year, depending on their educational costs. Students demonstrating need (based on federal guidelines) may receive up to $8,500 of their annual Stafford Loan on a subsidized basis. Any amount of the first $8,500 for which the student has not demonstrated need (as well as the remaining $10,000 should they borrow the maximum loan), would be borrowed on an unsubsidized basis. When a loan is subsidized, the federal government pays the interest for the borrower as long as he or she remains enrolled on at least a half-time basis, and for a six-month grace period following graduation or withdrawal. When a loan is unsubsidized, the student is responsible for the interest and may pay the interest on a monthly basis or opt to have the interest capitalized and added to the principal. Alternative Loans In addition to the Federal Stafford Loans, a variety of low-interest, alternative loans are available for students pursuing an undergraduate education. For additional information on these loans, contact the Office of Financial Aid at (203) 254-4125. FAIRFIELD UNIVERSITY Immunization Information On May 16, 1989, Public Act 89-90: An Act Concerning Proof of Immunization Against Measles and Rubella for Certain Persons at Institutions of Higher Education was signed into law. This act requires that any student enrolled in a course of study leading to a degree or graduate certificate who was born after Dec. 31, 1956, must provide proof of immunization for measles and rubella. The act further requires that the University exclude any student from enrollment or attendance without a certificate of immunization or other acceptable evidence of immunity to each disease. For you to be enrolled or take classes after Jan. 1, 1990, it will be necessary for you to provide the University Health Center with one of the following: A Certificate of Immunization To demonstrate proper immunization against each disease, a student shall present the school with proof of immunization from a physician, nurse, or health official who has administered the immunizing agents to the student. The certificate shall specify the immunizing agent and the dates on which it was administered. (The Fairfield University Health Center can provide immunizations; call (203) 254-4000, ext. 2241 for information.) OR Proof of Immunity To demonstrate that a student is immune to any of the diseases, the student shall provide the school with laboratory evidence demonstrating immunity. The Act further defines adequate immunization for: Measles: Two doses of measles vaccine administered at least one month apart. The first dose must have been administered at one year of age or older and after Jan. 1, 1969; the second dose must have been given after Jan. 1, 1980. Rubella: One dose of rubella vaccine administered after the student s first birthday. *Exceptions from this requirement are as follows: The student presents to the school a physician s written statement that immunization against one or more of these diseases is medically unadvisable. The physician s statement must specify the reasons the vaccine is contraindicated and that reason should be consistent with the U.S. Public Health Service Advisory Committee s statement regarding contraindications. If the statement does not include all diseases, the student must meet the immunization/immunity requirements for those diseases not covered by the statement. OR The student must present a statement in writing indicating an opposition to immunization because of a sincere religious belief. Any student so exempted from these requirements will be excluded from attending classes for a specified period upon the documentation of a single case of measles or rubella occurring within the student body. *There is NO exception for students applying to Nursing programs.
