Why do we need a PHP? A Population Health Plan (PHP) is an instrument used by the health sector, together with all other sectors and the whole of society, in order to achieve better health and well being. Health Level 2: better health Health Level 1 slide 03 of18

For a certain time frame (usually, 3 to 5 years), a PHP: defines the main population health needs (in terms of mortality, morbidity and health determinants) identifies effective strategies to meet those needs and who will implement them defines population health goals makes recommendations for its implementation slide 04 of 18
