Summary of Services NSL n Emergency Patient Transport Service As from 1 st July 2012 for a period of 5 years NSL Care Services one of the UK s largest providers of patient transport services is to run the n Emergency Patient Transport Services in partnership with NHS rthamptonshire (also for NHS Derbyshire & NHS Lincolnshire). NEPTS is not a right and patients must fit clearly into the eligibility criteria outlined in the criteria following this document. Summary service details noted below NSL Booking Office Telephone number 0843 208 6888 1 Transportation hours of operation Monday to Friday 8.00 am 21.00 pm and Saturday 9.00 am to 17.00 pm The majority of patients should arrive 10 30 minutes prior to their appointment time The majority of patients shall be collected within 30 minutes of request or agreed transport/and or zone time. 2 Out of hours service Outside of core hours sufficient resource will be available to provide skeleton cover 24/7 to NGH, KGH & NHFT The provider must ensure that a timely weekend/bank holiday discharge service is available. 3 NHS rthamptonshire Booking office cover Monday to Friday 8.00 am to 8.00 pm (online booking 24hrs/7 days) the same eligibility criteria is applied by telephone and the on-line system) 4 Renal Transportation Monday to Saturday 6.00 am to 23.30pm for haemodialysis sessions Patients should arrive at the site of their appointment no more than 30 minutes before their appointment time. The exception would be for first appointments where patients must arrive no more than 15 minutes before the clinic start time Patients should leave the dialysis unit no later than 30 minutes after their booked ready time. The exception would be for last appointments where patients must leave no later than 15 minutes after the (last) booked ready time Transportation is usually required 3 times per week for 4-5 hours, where Xmas Day falls on a Saturday dialysis is undertaken on a Sunday, NSL will be required to fulfil these requirements In severe weather conditions or other situations which may impede transportation for haemodialysis, NSL will have a Business Contingency Plan that ensure continuance of service. 5 Extra Contractual Journey (ECJ) Outside these hours transport will be classed as an Extra Contractual Journey (ECJ) (Hours as noted in paragraph 1 & 4) Schedule 2 of contract section 1.4.15 notes the following activities will be classified as an ECJ a) between 2101 and 0759 on a Monday, Tuesday, Wednesday, Thursday, Friday for acute, community and mental health patients and between 1701 on Saturday to 0759 on Monday. b) Any travel into or out of Points of Care outside of the rthamptonshire boundaries not included in the contract specification without prior agreement between both parties. c) Where the patient is transported and is not registered to a General Practice or resident within rthamptonshire PCT. d) Home to home addresses or residential home to residential home. c:\documents and settings\albarrington\desktop\summary of services nsl nepts.doc

e) Renal Patients from 23.31 Saturday to 05.59 Monday. f) Between 17.01 Saturday and 07.59 Monday for all transport (see additional requirements 3.5.2) The patients responsible PCT is expected to bear the costs associated with any NEPTS journey undertaken in accordance with the NEPTS specification. It is the contractor s responsibility to recover the costs from these organisations and to contact the home county PCT for authorisation prior to transporting. Patients that have ongoing long term treatment will only require assessment every 4 weeks. All patients will be transported in vehicles no more than 7 years old. A replacement vehicle will be available within 1 hour of any vehicle breakdown, or from the time a defect is noted, necessitating its temporary removal from use and should remain available for the duration that the original vehicle is out of use. NSL will contact patients 1-2 days prior to the journey to confirm transport is still required and is restricted to patients registered to a GP within the geographical boundary of NHS rthamptonshire NSL staff will ensure patient safety and dignity at all times, helping with outdoor clothing, ensuring seat belts worn. NSL will ensure that the patients have their appointment card/doctors letter with them and this is double checked before the commencement of the journey. (Patients who have mislaid their appointment card should not be refused transportation and this should not delay the journey.) Patients will be collected from their door or from inside their home if appropriate and will be transported to designated waiting areas. Where no designated area exists, the patients will be conveyed to the relevant clinic or ward. NSL will ensure when a two-person crew (driver & attendant) is requested the attendant remains seated in the rear of the vehicle travelling with the patient(s) for the whole of the journey. 6 Clarification of Terms Aborted Journey - a journey is cancelled once the vehicle has commenced the journey route for which the patients is included or upon arrival for collection of the patient. This is chargeable at the same rate as the original booking (return journey planned will be classed as a cancellation and not chargeable. To minimise etc. aborted journey and cancellations NSL will endeavour to confirm all known bookings by contact patients 1-2 days prior to travel a minimum of 2 separate occasions by a variety of means. Essential Services n emergency patient transport services for renal & oncology patients with an acuity of medical need that requires ongoing treatment Emergency Preparedness Services in accordance with Part 2A of this Schedule 2 (The Services) Full monitoring of the journeys taken will be provided on a monthly basis. 110616-NEPTS Eligibility Criteria- V3 Guidance te n Emergency Patient Transport Eligibility (NEPTS) Criteria 1. Document Purpose This document sets out the eligibility criteria for the East Midlands n-emergency Patient Transport Services (NEPTS), to take effect on all NEPTS journeys to be provided and will ensure a standard approach to eligibility so that patients receive a consistent response to requests for this form of transport regardless of where they receive their healthcare. This eligibility criteria was endorsed by PCT Chief executives in May, 2011. 2. Scope To standardise access to NEPTS across the 5 East Midlands healthcare economies that is classified as non urgent, planned and scheduled transportation of patients with a medical need for transport to and from a premises providing NHS healthcare and between NHS healthcare providers. This will cover all NHS and independent service providers contracted to the NHS. 3. Introduction Ambulance and other n-emergency Patient Transport Services: Operation, Use and Performance Standards [HSG 1991(29)] was published in 1991. This set out guidance for the NHS on the operation, use and performance standards for emergency and urgent Ambulances, superseded by the Eligibility Criteria for Patient Transport Services (PTS) document issued in August 2007. The proposals in this document comply with this guidance East Midland NHS Chief Executives agreed at their meeting in February, 2011 to adopt an East Midland wide NEPTS criteria and this paper is derived from: - Reviewing current EM eligibility criteria - NEPTS assessment examples from other NHS Trusts - Reviewing DH NEPTS and associated material. A recent review of NEPTS has revealed that 66% of all PTS journeys are for the lowest category of need and that significant inefficiencies are present within the current arrangements due to NEPTS journeys being aborted, with almost half of aborts due to hospitals not cancelling NEPTS journeys when there was no longer a need. The intention is that these eligibility criteria will commence from 1 st April, 2011

Eligibility Criteria The NHS has limited resources and providing non emergency patient transport must be reserved only for those whose medical conditions warrants it and the expectation is that patients are expected to make their own transport arrangements. NEPTS must only be targeted at those patients who have a clinically stated medical need that prevents them using private or public transport. Prior to booking transport, the protocol will demand that enquiries have been made to determine whether the patient is able to make their own way or alternative arrangements via friends or family. It should also be made clear that NEPTS services will not be provided for social or financial reasons. It should be noted that: Patients attending NHS treatment do not have an automatic right to NEPTS; if they do not fully meet the agreed eligibility criteria, then transport will not be awarded. In general, patients are expected to make their own arrangements for travelling to and from healthcare premises, this ensures that Limited resources are available for those who really need them. If a patient has a medical or mobility condition where they need NEPTS staff to support them to and from a hospital or clinic for treatment, they may be eligible for patient transport. NEPTS caters for those patients where the medical condition will require the skills of NEPTS staff and/or it would be detrimental to their diagnosed condition or recovery if they were to travel by any other means. Patients in receipt of mobility allowance are not usually eligible for NEPTS as their mobility allowance should pay for their transport to and from hospitals. They may be entitled to NEPTS if there is a medical need i.e. if they have leg in plaster and cannot get to hospital by any other route 5. Qualifying Criteria The following sets out the eligibility criteria to determine if NHS funded n-emergency Patient Transport Services will be provided for patients in the East Midlands who: Require to be moved on a stretcher. Defined as, the patient needs to be lying down on a stretcher within the ambulance or have a physical reason for using the stretcher facility e.g. leg in a cast Are in a wheelchair and cannot transfer in and out of a vehicle without assistance and who have no other way of getting to hospital

110616-NEPTS Eligibility Criteria- V3 The patient attending for treatment is likely to have diagnosed severe physical side effects following treatment that would impede driving e.g. for renal dialysis or oncology treatment, judgement must be made whether transport is required for both parts of the journey Have a diagnosed disability (physical and or psychological) which makes them medically unfit to travel by any other means, and who have no alternative method of travelling to or from the hospital A recognised parent/guardian where children with a medical condition, as outlined above, are being conveyed For medical reasons, the patient requires a trained escort. rmally designated ambulance transport staff will escort the patient to and from hospital. In most cases relatives and friends are welcome to meet a patient at their point of destination, although they will be unable to accompany the patient on their journey unless the patient satisfies one of the following criteria defined in Table 1 below. Discharges A patients journey home should be discussed with the patient upon admission. It is expected patients will make their own arrangements unless they are judged as eligible for hospital transport using the established criteria 7. Booking NEPTS The current procedures for booking NEPTS will remain but it is expected that the eligibility criteria will be adhered to consistently throughout the East Midlands and every effort must be used to book NEPTS in advance as on-day demand is limited.

Appeals Any appeal against the refusal to award NEPTS should initially be dealt with informally by the County PTS Lead and if a satisfactory agreement cannot be reached it should be brought to PCT s individual case review panel. NB It would be expected that there will be no exceptions to the eligibility criteria; if the patient fails to meet the criteria the request for NEPTS must be declined and the patient sign posted to known services, including; Local authority provided transportation Public transport, including bus routes Voluntary sector transport provision Private hire/taxi services For those patients who do not have a medical need for ambulance transport, but may require help in meeting the cost of travel to and from their care may be entitled to financial assistance under the Hospital Travel Costs Scheme (HTCS). To be eligible for assistance under the HTCS, a patient must be in receipt of at least one of the following: Income Support Income Based Jobseekers Allowance Income Related Employment and Support Allowance Guarantee Pension Credit Child Tax Credit Working Tax Credit Patients must be assessed every time they request NEPTS as their condition may have changed. Patients will only be able to claim back the full travel costs by using the cheapest form of public transport available. If travelling by private car patients may be able to claim for the fuel used and unavoidable car parking costs, up to the cost of the same journey by public transport. Is the patient in a wheelchair and cannot transfer in and out of a vehicle without skilled assistance and has no other way of getting to a hospital? Is the patient attending hospital likely to have diagnosed severe physical side effects following treatment. Do they need transport both ways? Does the patient have a diagnosed disability that may result in their condition or behaviour deteriorating during transport that could put them at risk and does the patient have no other means of transport? Does the patient believe there is a medical reason for booking hospital transport? CLAIMING TRAVEL EXPENSES The patient may obtain a cash refund for travel to the hospital when travelling by public transport OR private car IF they are in receipt of one of the following;- Income Support Working families tax credit Income based Job Seekers Allowance If they arrive by public transport the ticket must be produced along with proof of benefit received. The claim may then be made at Hospital Administration. If they arrive by private car they may apply for expenses. Does the patient claim mobility allowance? Does the patient have their own transport or a friend/relative that could help out? Is own/friend/relative transport available if we change the appointment time?
