Coverage is on a guaranteed issue basis during the enrollment period. Insured Employee Coverage Face Life Amount $50,000 Accidental Death and $50,000 Dismemberment VOLUNTARY BENEFITS Supplemental Life Insurance - Symetra Life Underwritten by The Symetra Insurance Company, this program offers up to $500,000 of additional life and AD&D coverage for employees and spouses, as well as, up to $10,000 for dependent children (subject to medical underwriting acceptance). New Hires/ Guarantee Issue: You are eligible to receive $100,000 of life and AD&D coverage, plus $10,000 of coverage on your spouse and up to $10,000 for each dependent child, without evidence of insurability. Applications must be completed in the first 30 days of employment. Long Term Care - UNUM If paying for long-term care benefits for a loved one would create a financial burden, this program is for you. Underwritten by UNUM, this program can provide a benefit of up to $6,000 a month of LTC care for you, your spouse, parent or grandparent with very affordable rates (subject to medical underwriting acceptance).
