Seasons can be restored for 4th Xrd nd Season-ending injuries: Verified by medical doctor treating the student. Competed in no more than permissible number of Injury cannot occur in last regular contest or post. 1st Restores a of competition but does not restore any terms toward the 10 semester/15 quarter rule. In the NAIA, a hardship waiver permits a student to get a of competition back if the student has suffered a -ending injury that has been verified by a medical doctor. has competed in no more than the maximum amount of permitted for a hardship, and the injury did not occur in the last regular contest or during post. The other thing to remember on hardships is that they only give you s of competition back. The student can only get one of those four s back. It does not restore any of the 10 semesters or 15 quarters. So multiple hardships, while permissible, won t permit a student to compete after his or her 10th semester or 15 th quarter. Coach / AD / FAR must complete Transcript required Completion Signatures There is a NAIA form for hardship requests that must be filled out and sent directly to the NAIA National Office. We will review those and send them back to your institution. Please make sure the entire form is filled out. The top part of the form is filled out by the institution, the bottom section by the doctor. Be sure you send a copy of the student s transcript. It can be an unofficial transcript, but please include a copy of the transcript. Coach, your area of the form involves confirming the number of in which the student played. Obviously everything on the form is important, but this area is where you play a key role. The form requires the signature of the athletics director or faculty athletics representaive and the coach. Doctor must complete Answer and initial all four questions Signature The bottom part if the form is what the doctor fills out. Most delays in reviewing hardships occur because the doctors have not filled out the form properly. Please do not send in a form to the NAIA National Office that is not filled out completely. We are just going to send it back and ask you to fill it out. So please check, that the doctor has answered the four questions and initialed all 4. If any of the four are not answered, we cannot process the hardship and we will send it back to you. It only delays the process and creates more work for everyone. Second, the proper signatures including the doctor s signature are on the form One of the things that we recommend, even though it is not required, is that you hold all of your hardships for your students and your sport until the end of the. That way you can confirm that the injury was ending and the student was not able to compete physically throughout the rest of the and also it gives you and your doctors time to go through and make sure everything is included.
