C6 Recipe information A recipe must be provided that lists all the added ingredients in the Product. Applicants also have a duty to report bases/acids that are added to adjust the ph. If the Product is available in several variants, the different colouring agents, fragrances or biological substances may be reported in the same recipe. In such cases, specify which product variants contain which colouring agents, fragrances or biological substances. For each ingredient and supplier a safety data sheet must be submitted (see REACH regulations for structure). A safety data sheet must also be submitted for all chemical substances not previously approved by the Swedish Society for Nature Conservation, that can be found in the ingredient. Trade name Give the trade name of the ingredients. Supplier Give the name of the supplier. If you use several suppliers for the same ingredient, give each supplier on a separate line. Manufacturer State if the manufacturer of the ingredient is different than the supplier. Function State the function of the ingredient, making your choice from one of the functions below. Surfactants, Complexing agents, Solvents, Preservatives, Thickening agents and dissolving agents, Bleaching agents, Acids, Colouring agents, Fragrance, Biological substances, Enzymes, Fillers, Abrasives and Other additives. State all chemical substances that are included in the trade name. Give each chemical substance on a separate line. Appendix chemical substances Information for assessing a chemical substance that is an ingredient or forms part of an ingredient All chemical substances included in the ingredient, that have not already been approved by the Swedish Society for Nature Conservation for use in products labelled Bra Miljöval, must be reported according to this appendix. Copy this page if you need more than one. The absence of any of the required information in the form means that the ingredient cannot be approved. The Swedish Society for Nature Conservation considers all chemical substances that occur in contents over 0.01% as part of the ingredient, regardless of whether or not the chemical substance was added intentionally. If the chemical substance is a surfactant, complexing agent, solvent or preservative, complete test reports must be submitted regarding degradability, bioaccumulability and ecotoxicology. Section 1 are to be filled in for all types of substances. The remaining sections are to be filled in for the type of substance/ substances stated in the headings. The list must contain all extraction agents and their s. 4 Aerobic degradability Aerobic degradability must be reported for surfactants, complexing agents, solvents, preservatives, thickening agents/dissolving agents, bleaching agents, other additives, acids and colouring agents (only if the colouring agent is not approved as food colouring in accordance with regulation (EC) No 1333/2008). The colouring agent is not classified as H317, May cause an allergic skin reaction. The colouring agent is an azo colouring agent and could potentially be broken down into amines. If the colouring agent is an azo colouring agent and could potentially be broken down into amines, a list of these must be enclosed with the application. If the fragrance has not been previously approved for use in Good Environmental Choice labelled products the following documents should be submitted: 1) Safety data sheet for the fragrance 2) A complete list of all substances present above 1,0% by weight in the fragrance. s should always be stated. If the substance is classified according to CLP, the hazard statements should be stated too. For substances that have been added to the perfume for other reasons than their fragrance properties, a functional category (e.g solvents) in accordance with the criteria document should be stated. However, exact concentrations are not mandatory. 3) A complete list of all substances classified with H317 or included in Annex III (with reference number 67-92) to the cosmetics regulation (EC) No 1223/2009. E4 Company s transport operations Information has been enclosed with another Application and will not be sent again. The Swedish Society for Nature Conservation Application licence Bra Miljöval Chemical products 2006 F1 Confirmation The Swedish Society for Nature Conservation must receive the signed confirmation in order to process the Application/declaration. Company (Licence holder): Product confirmation refers to (name of product): Licence Holder 1) hereby guarentees that all the information submitted to the Swedish Society for Nature Conservation in the Application is accurate, 2) hereby guarentees that the Licence Holder has full and unrestricted right to administer the Product as required to fulfill the undertaking under the License Terms and Conditions. 3) undertakes to abide with Licence Terms and Conditions and pay the Application Fee. 4) accepts that the Application Fee is not refundable. 5) undertakes not to market or sell the Product with Bra Miljöval before Licence is granted, and 6) accept that the prevailing practice documents issued by the Swedish Society for Nature Conservation should guide the interpretation of the Criteria and the License Terms and Conditions.
