What does the program cover? The program provides General Liability coverage for YOU as a coach or game official of amateur youth football (and directly related) activities. General Liability coverage provides protection for covered third party claims of bodily injury and property damage arising from your youth football coaching or officiating activities. This coverage is in excess of any valid and collectible insurance (such as your team s or league s coverage). If there is no other insurance available, this insurance will apply on a primary basis without a deductible.

Yes, your coverage extends to liability you may incur resulting from your instructing other coaches in the USA Football Heads Up curriculum. 4. What are some of the exclusions? Exclusions include, but are not limited to: Medical payments to any participant Actual or threatened abuse or molestation Employment-related practices Corporal punishment Pyrotechnics/fireworks Activities other than youth football coaching or officiating (ages 18 and under), although coverage is provided for coaching high school students who may be age 19 as long as they are still enrolled in high school Overnight activities youth and/or adult Coaching or Officiating adult, collegiate, semi-professional or professional teams Player vs. Player suits 5. The online certificate request form allows you to download a copy of any contract, form or other specifics that may be required to be included on your

certificate. These will be reviewed by our insurance agent to determine if they may be included on the certificate. 8. When is my insurance effective? When does my coverage expire? Coaches Once you qualify, your coverage is in force for as long as you continue your coach membership and complete one of the course requirements annually. Officials Once you qualify, you will remain covered as long as you continue your game official membership. USA Football maintains the right to amend the coverage and insurance provider at its sole discretion. 10. How do I renew my insurance? Coaches insurance renews automatically when you renew your USA Football coach membership and complete your annual re-certification requirement. Officials insurance renews automatically when you renew your USA Football game official membership. 11. If I get hurt, what do I do? This policy does not replace personal medical or accident policies and does not provide medical coverage for your own injuries or illness, even if sustained while engaged in coaching or officiating activities. Your own personal, league, team or employer-sponsored medical policy should be reviewed for terms relating to your coaching or officiating activities.

12. Does the insurance cover practices? Yes. Your organized practices, scrimmages and games during, and in preparation for, an amateur youth football season are included. 13. Does the insurance cover day camps or clinics? Yes, the insurance program covers your participation and work at day camps or clinics. This insurance coverage does NOT cover your participation at overnight camps or clinics. 14. Does the insurance cover my team or my league? Your team or league can be included as an Additional Insured, but only with regard to your coaching or officiating activities for them. This insurance does not protect the team or league from any liability resulting from their activities or actions. It is strongly recommended that every team or league, at minimum, carry their own general liability policy including coverage for the league or team, the coaches, assistants, trainers, players, cheer squads and volunteers. 15. Does the coverage include coaching or officiating outside of the United States? The Coverage Territory is defined as: 1. The U.S. (including territories and possessions), Puerto Rico and Canada, and military bases outside of the U.S., 2. International waters or airspace, but only if claim arises from travel between any places in #1. 3. Everywhere else in the world, but only from activities of a person whose home is in the territory described in #1, and they are away for a short time on coaching or officiating business; or who are stationed at a United States military base located outside places in #1. The suit must be brought in the U.S., (including its territories and possessions, Puerto Rico or Canada). 16. Does the insurance cover travel? No. Coverage is not provided for injury to anyone (including yourself) being transported to or from activities related to your coaching or officiating. 17. Does my insurance cover legal fees? The insurance company will provide defense against a lawsuit brought against you seeking damages as a result of your youth coaching or officiating activities, subject to the terms, limits and conditions of the policy. Please see Question #4 above for some of the major coverage exclusions.

18. How do I file a claim? Emergency claims service is available 24 hours a day, 7 days a week by calling 800-566-7941. 19. Who do I contact with questions?
