Step 1: Go to the online Exemption Request Form Login to Mocomotion On Welcome tab, Main Menu channel, click on Self Service (or from the Student tab, Student Starter Kit channel, click on Self Service) Click on Student Information Click on Student Exemption User Documentation Student 2
Click on Exemption Menu Click on Exemption Request The following screen will display: PLEASE NOTE MESSAGE: POP UPS MUST BE ENABLED User Documentation Student 3
Step 2: Choose the Exemption Type Click on General Education for an explanation of a General Education course. Click on Regular Course for an explanation of a Regular Course. User Documentation Student 4
Step 3: Choose the course for exemption If the course for exemption is a General Education course, the Course for Exemption information will automatically be filled in for you: If the course for exemption is a Regular Course, you must choose the Subject Code and Course Code from the dropdown menus. If you have already passed the Course for Exemption, the following message will display: If you have already passed a course that is equivalent to the Course for Exemption, the following message will display: User Documentation Student 6
If you have already received an exemption for the Course for Exemption, one of the following messages will display: User Documentation Student 7
If the Course for Exemption is a non-credit course, and therefore not eligible for an exemption, the following message will display: If an exemption request has already been submitted for the Course for Exemption, the following message will display: User Documentation Student 8
Step 4: Enter the Transferring Institution / Course information From the dropdown menu, choose the Institution. User Documentation Student 9
SITUATION A If you choose Mohawk from the drop down list, a window will display that shows all your credit courses with a passing grade to Select for your exemption request. Click Select once you find the course you want to use for the exemption. If you need to select more than one course to be considered for the exemption, go to the next line and repeat the process. NOTE: If you have multiple Mohawk College ID numbers, and the courses you are using for the exemption are under a different ID number, you will have to return to the screen and: - choose OTHERS from the Institution dropdown list - type in Mohawk (under ID #00xxxxxxx) in the Institution box - type in the Course Code and Course Name for the course - click Add and this information will appear on the Exemption Request screen - type in the Grade. If you expect to see one of your courses on the Mohawk course screen and it is not displayed, please contact the Registration Centre at 905-575-2000 (or toll-free at 1-866- 410-4795) for assistance. SITUATION B If you choose an Institution other than Mohawk, a screen similar to the one below will appear: An ADD line will be displayed, possibly followed by a list of courses to Select. SITUATION B1 If there is course information filled in on a line to Select, as in the example above, this means: that particular course taken at that institution has been identified as a course that qualifies to be used as an exemption for the Course for Exemption. If you have successfully passed a course from a Select line - click on Select and this information will appear on the Exemption Request screen - type in the Grade. If you need to select more than one course to be considered for the exemption, go to the next line and repeat the process. User Documentation Student 11
SITUATION B2 If there is not a course for you to Select, or if you do not have one of the courses listed, you will have to enter your information on the ADD line (see screen on previous page): - type in the Course Code - type in the Course Name - click ADD and this information will appear on the Exemption Request screen - type in the Grade. SITUATION C If you do not see your institution on the dropdown list, choose OTHERS, and the following screen will display: On the ADD line: - type in the Institution name - type in the Course Code - type in the Course Name - click ADD and this information will appear on the Exemption Request screen - type in the Grade. To get out of the Transferring Institution / Course screen, click on the Cancel button. If you X out of the screen, the Institution Name will remain on the line and you will have to click on the Clear Record button beside the line in order to be able to choose an Institution name again. Step 5: Submit your request and print a copy When you have finished entering your information, click on SUBMIT and the following popup will be displayed, to let you know not to close the browser until you click OK to continue and receive an Exemption Request #: Click OK in the popup box and wait for your exemption request information to display. NOTE: If you click Submit in error before you have entered all your Transferring Institution Courses, you will have to contact the Registration Centre, Room C066, Fennell Campus either in person or by phone 905-575-2000 (or toll-free at 1-866- 410-4795) to have your request Voided. You will then be able to re-apply for the exemption online. PRINT A COPY OF THE EXEMPTION REQUEST PAGE THAT APPEARS User Documentation Student 14
Step 6: Take your copy and supporting documentation to the location indicated If the following screen is displayed, this means your Exemption Request fits SITUATION A, B2 or C, as described in Step 4 above. User Documentation Student 15
Following the instructions on the screen, Print the Exemption Request and bring it, (along with your official transcript and course outline for any courses you are using that are from an institution other than Mohawk College), to the appropriate Academic Office. If you Click for list of Academic Offices, the following information will display: User Documentation Student 16
If the following screen is displayed, this means your Exemption Request fits SITUATION B1 as described in Step 4 above. Following the instructions on the screen, Print the Exemption Request and bring it, along with your official transcript from the transferring institution, to a Registration Centre for confirmation of your passing grade. If you Click for locations, the following information will display: Registration Centre Locations POST SECONDARY OR CONTINUING EDUCATION COURSES Submit your Exemption Request form and official transcript in person to one of the following locations: Registration Centre, Room C066, Fennell Campus Continuing Education Registration, Room F124, Fennell Campus Student Services Office, Room A102, Brantford Campus Student Services Office, Room 121, IAHS Campus Student Services Office, Room A111, Stoney Creek Campus or mail to: Fennell Campus Registration Centre, Mohawk College, P.O. Box 2034, Hamilton, ON, L8N 3T2 Fill in the Course Code and Course Name and click ADD. APPRENTICESHIP COURSES Submit your Exemption Request form and official transcript in person to: Student Services Office, Room A111, Stoney Creek Campus or mail to: Stoney Creek Student Services, 481 Barton Street, Stoney Creek, ON, L8G 3Y4 Step 7: Check status of your exemption request User Documentation Student 17
To check the status of your exemption request, go to the Exemption Menu and click on Student Exemption Report. The following screen will display: For any exemption requests that show Approved or Confirm Pass, there will be an E grade in your Academic History (you can go to View Your Grades to verify the E grade has been put against the course). If you are currently registered in a course that you have now received an exemption for, the system will automatically drop you from the course. User Documentation Student 18
